Cognitive treatment of pathological gamblers.

This study evaluates the efficacy of a cognitive treatment for pathological gambling. Five pathological gamblers were treated in a multiple baseline across subjects design. Cognitive correction targeted the erroneous perceptions towards the notion of randomness. Four subjects reported a clinically significant decrease in the urge to gamble, an increase in their perception of control, and no longer met the DSM-IV criteria for pathological gambling. Therapeutic gains were maintained at the 6 month follow-up. Results suggest that cognitive therapy targeting the misconception of the notion of rand

[Lithium prevention of depression 100 years ago--an ingenious misconception].

The rediscovery of C. Lange's book "Uber periodische Depressionszustände und ihre Pathogenesis auf dem Boden der harnsauren Diathese", Copenhagen 1886, in a German translation by H. Kurella in 1896, gave rise to appreciate its value, to make its misconception intelligible, and to write down the author's and the translator's course of life. In his booklet Carl Lange gives a specification of depression in a most modern form. It is thrilling to read his report on about 2000 depressed out-patients successfully treated over a 20-year-period - from today's point of view lege artis - with lithium car

Presentation delay in breast cancer patients, identifying the barriers in North Pakistan.

BACKGROUND: There is strong evidence that delayed diagnosis of breast cancer is associated with poor survival. Our objectives were to determine the frequency of breast cancer patients with delayed presentation, the reasons of delay and its association with different socio-demographic variables in our North Pakistan setting. MATERIALS AND METHODS: We interviewed 315 histologically confirmed breast cancer patients. Delay was defined as more than 3 months from appearance of symptoms to consultation with a doctor. Questions were asked from each patient which could reflect their understanding abou

Appreciation of research information in patients with bipolar disorder.

OBJECTIVE: Ethicists have debated whether patients with serious mental illness can appreciate the risks of research participation and make a … elieved that researchers would make decisions based solely on what would be the best care for them (therapeutic misconception); and in randomized medication trials, they mistakenly believed they had improved chances of receiving one treatment over another. There were no differences between mood groups on these measures. Over half of subjects (59%) indicated that their ment …

The Fluid Connection: Balancing Hydration and Overactive Bladder Symptoms in Women.

In recent years, excessive fluid intake by women has emerged as a well-being misconception without robust scientific support but may have an adverse impact on treatment outcome of overactive bladder (OAB) syndrome. Excessive fluid intake may have potential disadvantages as overhydration leads to exacerbation of symptoms of OAB particularly frequency, nocturia, and urgency. Fluid management by recommending daily intake levels is the integral, yet underutilized, component of the treatment plan and should be the initial therapeutic approach in all women suffering from OAB. This strategy will redu

Ethical Considerations in Caring for Patients with Advanced Malignancy.

Patients with advanced malignancy have decisions to make about next steps that are multifactorial and highly ramified. At each step, they, their loved ones, and their health care providers will attempt to make right decisions and avoid wrong ones. Beyond bare ethical principles, these patients face tensions between what they hope for, what is possible, and what those around them expect and advise. This article uses a case-based approach to explore the balance between prognostication and directive counsel; affective forecasting and decisional regret; hope and the therapeutic misconception; and …

Nutritional Aspects of Pediatric Gastrointestinal Diseases.

In the last decade, the role of nutritional management in pediatric gastrointestinal diseases has gained increasing popularity. Disease-specific diets have been introduced as conventional treatments by international guidelines. Patients tend to more willingly accept food-based therapies than drugs because of their relatively "harmless" nature. Apart from a diet's therapeutic role, nutritional support is crucial in maintaining growth and improving clinical outcomes in pediatric patients. Despite the absence of classical "side effects", however, it should be emphasized that any dietary modificat

Planning advanced therapy trials for Huntington's Disease: ethical considerations.

Huntington's Disease (HD) is a relentlessly progressive genetic disorder characterised by cognitive, behavioural and physical impairments. T … discrimination, trial design challenges (especially the use of surgical placebos) and the risk for therapeutic misconception need careful consideration whilst broader concerns relating to justice and equity must be recognised by researchers and funders alike. This paper will provide those involved in gene therapy trials and other advanced therapy research a comprehensive …

How Should Physicians Help Patients Understand Unknowns of Nanoparticle-Based Medicines?

When a patient wants to enroll in a clinical trial to gain early access to an apparently promising but unproven intervention, her physician should clarify differences between participating in research and receiving treatment to help her avoid therapeutic misconception, make a thoughtful decision, and consider relevant clinical and ethical details. These include a patient's disease and treatment experiences, needs, interests, values, the design and phase of the trial, and the nature of the intervention being studied. When an unproven intervention is a nanodrug, a physician's role is especially …

Dispelling misconceptions in the management of familial adenomatous polyposis.

Patients with familial adenomatous polyposis require surgical intervention at some point in their lives. The diagnosis is often apparent from their phenotype and family history, however, this is not always the case. Many factors can influence the surgical strategy although the polyposis burden and distribution remain the main consideration. While prophylactic removal of the rectum and colon is often required, sparing the rectum at the index surgery is safe in select patients. This article aims to dispel misconceptions in the diagnosis and treatment of patients with familial adenomatous polypos

Ketogenic metabolic therapy for chronic kidney disease - the pro part.

Ketogenic metabolic therapy (KMT) is a medical nutrition therapy to address certain health and disease conditions. It is increasingly used for many non-communicable diseases that are rooted in abnormal metabolic health. Since chronic kidney disease (CKD) is commonly caused by overnutrition leading to hyperglycemia, insulin resistance and diabetes mellitus, the carbohydrate restriction inherent in KMT may offer a therapeutic option. Numerous studies have found that various forms of KMT are safe for individuals with CKD and may lead to improvement of renal function. This is in contrast to the cu

Stem cell treatments and Parkinson's disease: Science and misconceptions.

Parkinson's disease (PD) is a common neurodegenerative disorder associated with the accumulation of alpha-synuclein in Lewy bodies and Lewy neurites, as well as cell loss, including ventral midbrain dopaminergic (vmDA) neurons in the substantia nigra. This degeneration is responsible for some of the characteristic motor features of PD, which, in the early stages of the disease, can be successfully treated pharmacologically. However, in later stages of the disease, the efficacy of these drugs declines, and they can cause side effects. Dopamine cell therapy, which involves replacing lost vmDA ne

Breaking the guardian of the genome: TP53 dysfunction in myeloid neoplasms.

Since its discovery more than 40 years ago, TP53 has emerged as the most iconic gene in cancer biology. Early studies of mutant constructs led researchers to misclassify TP53 as an oncogene, a misconception later resolved when wild-type p53 was shown to function as a tumor suppressor. p53 is now widely recognized as the central guardian of genomic integrity, coordinating DNA-damage responses, cell-cycle control, senescence, apoptosis, and metabolic stress adaptation. In myeloid neoplasms, TP53 mutations occur in only ∼10-15% of cases but confer a disproportionately adverse clinical impact mark

Myths and misconceptions about childhood constipation.

Many widely held beliefs and assumptions concerning childhood constipation continue to interfere with rational management of childhood constipation. Although many still believe that constipation is not a common disease, about 9.5% of the world's children suffer from chronic constipation. Most of these children live in non-Western countries. There are major misconceptions about the etiology of constipation as a significant proportion of clinicians still believe that constipation is caused by some form an organic pathology, whereas in reality, the majority have functional constipation.
