BACKGROUND: The angiosomes of the body were defined in 1987. The recent popularity of skin perforator and muscle flaps designed in the thigh, together with significant major vessel anomaly and disease, has necessitated a more detailed reevaluation of the blood supply to this region.

Lipomas are common soft-tissue tumours that are usually found in the subcutaneous adipose tissue. Occasionally, they may contain mesenchymal elements other than adipose tissue, including osseous components. These ossifying lipomas are usually located near or within bone, and it is very rare for a lipoma with no connection to bone to contain mature osseous tissue. We describe a case of a symptomatic ossifying intramuscular lipoma of the thigh.

Gossypiboma, an iatrogenic mass lesion caused by a retained surgical sponge is an extremely rare event following musculoskeletal procedures. A man aged 70 during a hospital stay over a period of a few weeks developed increasing pain and swelling of the left thigh. Despite extensive diagnostic imaging and surgical drainage, initially no diagnosis could be established. After more than 3 months the tumour proved to be caused by leakage of urine from the pelvis to the soft tissues of the leg. Unfamiliarity of the clinicians with such an extension of an urinoma contributed to the considerable diagnostic delay. Introduction of a bladder catheter stopped the flow of urine to the leg and reduced the leg circumference.

Necrotising fasciitis (NF) is an extremely virulent form of infectious fasciitis. It affects skin, subcutaneous fat and superficial and deep muscular fascia by rapidly progressive necrosis. Expeditious diagnosis and radical debridement is necessary to prevent the onset of sepsis, multisystem organ failure and possible death. Perforated rectal cancer resulting in NF can spread to the perineum and genitals known as Fournier gangrene. This case describes an unusual case of NF of the right thigh as first presentation highly suggestive for rectal cancer.
