Neuroendocrine Neoplasms of the Lungs, Thyroid, and Thymus.

Neuroendocrine neoplasms (NENs) comprise a group of tumours that can develop in various internal organs, but in this review, we will describe only those arising in the lungs, thyroid, and thymus. Pulmonary neuroendocrine neoplasms (pulmonary NENs) account for approximately 25% of all lung cancers. They are classified into four groups of tumours: typical carcinoids (TCs), atypical carcinoids (ACs), small cell lung carcinoma, and large cell lung carcinoma. This review focuses on TC and AC. The treatment consists mainly of radiotherapy, chemotherapy, and surgical resection, but novel drugs like a

Follicular neoplasms of the thyroid: predictors of malignancy?

Follicular neoplasms of the thyroid present a therapeutic challenge. Initial limited thyroidectomy may result in some patients requiring completion thyroidectomy for malignancy. In the current study we examined age, gender, race, time from nodule discovery to operation in months, history of radiation exposure, tumor size (cm), and cell type in patients with follicular neoplasms identified at the time of thyroidectomy in two socioeconomically diverse settings from 1993 through 2000 to identify possible factors associated with a greater chance of malignancy. Of 36 follicular lesions identified i

Thyroid cancer: a lethal endocrine neoplasm.

This conference focuses on the controversies about managing thyroid cancer, emphasizing the possibility that the treatment of patients with potentially fatal thyroid cancer may be improved. Although the mortality rate from thyroid cancer is low, it is the highest among cancers affecting the endocrine glands (excluding the ovary). Exposure to radiation during childhood in the 1930s and 1940s increased the incidence of but not the mortality from thyroid cancer, because these tumors are mainly papillary cancers developing in young adults. These rates may change as the exposed cohort ages. Risk fa

BRAF T1796A transversion mutation in various thyroid neoplasms.

A high prevalence of activating mutation of the B type Raf kinase (BRAF) gene was recently reported in papillary thyroid cancer (PTC). However, the frequency of this mutation in several other types of thyroid neoplasms was not thoroughly investigated. In the present study, in addition to PTC, we evaluated various thyroid tumor types for the most common BRAF T1796A mutation by direct genomic DNA sequencing. We found a high and similar frequency (45%) of the BRAF T1796A mutation in two geographically distinct PTC patient populations: one composed of sporadic cases from North America, and the oth…

[Thyroid neoplasms in children and adolescents--personal experience].

Between 1974-1992 we operated 12,344 patients at the Clinic of Surgery of the Institute of Endocrinology of the Lódź Medical Academy for various forms of goiter. Of these, 295 patients were between the ages of 9-18 years, including 268 (90.8%) girls and 27 (9.2%) boys. Malignant thyroid neoplasm was diagnosed in 19 of these children (6.4%). From among the 268 girls, malignant tumors were discovered in 15 (5.6%), while among the 27 boys, in 4 (14.8%). Ten of these neoplasms were carcinoma papillare, 9 carcinoma folliculare. All of the children are alive without signs of metastases or recurrence

Hurthle cell neoplasms of the thyroid gland revisited.

Hurthle cell tumours (benign and malignant) have been regarded as lesions with uncertain biological behaviour. However recent clinico-pathological studies have shown that they should be categorized as benign or malignant on the basis of capsular and/or vascular invasion, like other differentiated thyroid neoplasms. The fact that the tumours are composed of Hurthle cells is irrelevant. Currently, histological parameters do not seem to predict biological behaviour of Hurthle cell carcinomas.

[Lymphocytic infiltration in thyroid neoplasms. Preliminary prognostic assessments].

INTRODUCTION: It is known that the immune system is involved in several thyroid diseases and in the reaction against cancer progression. We have therefore evaluated lymphocytic infiltration in the slices of surgically removed thyroids in patients affected by thyroid carcinomas, to further clarify the anatomic and clinical characteristics of this pathology and the possible prognostic correlations. MATERIALS AND METHODS: Thirty-eight patients, 7 men and 31 women, aged 23-73 years, were studied. They underwent total or subtotal thyroidectomy for thyroid carcinoma. The histopathological findings

Hyalinizing trabecular tumor in a background of lymphocytic thyroiditis: a challenging neoplasm of the thyroid.

OBJECTIVE: To describe a case of hyalinizing trabecular tumor (HTT) in a background of lymphocytic thyroiditis that was misdiagnosed as papillary thyroid carcinoma (PTC) based on fine-needle aspiration (FNA) cytologic findings and overtreated with total thyroidectomy. METHODS: We present a case report, including the imaging and pathologic findings, of a 68-year-old woman who presented with a multinodular goiter that was suspicious for PTC. RESULTS: On the basis of FNA cytologic findings, she underwent a total thyroidectomy, and histologic examination of the thyroid gland revealed HTT in a ba

[Surgical tactics in thyroid nodular neoplasms associated with chronic autoimmune thyroiditis].

There were analyzed the results of surgical treatment of 628 patients with different thyroid nodular neoplasms combined with autoimmune thyroiditis, who were operated in the surgical department of the Mandryka Central Military Clinical Hospital in 1989-2001. The use of organ-preserving operations is substantiated by the necessity of maximal decrease of the post-operative complication level and preservation of thyroid hormonogenic function. Resection with node removal within the limits of healthy tissues is substantiated by the results of analysis of morphologic structure of nodular neoplasm an

Overview of the 2022 WHO Classification of Thyroid Neoplasms.

This review summarizes the changes in the 5th edition of the WHO Classification of Endocrine and Neuroendocrine Tumors that relate to the thyroid gland. The new classification has divided thyroid tumors into several new categories that allow for a clearer understanding of the cell of origin, pathologic features (cytopathology and histopathology), molecular classification, and biological behavior. Follicular cell-derived tumors constitute the majority of thyroid neoplasms. In this new classification, they are divided into benign, low-risk, and malignant neoplasms. Benign tumors include not only…

[Iodine deficiency and thyroid neoplasms in the Krakow region].

The aim of the study was to evaluate the correlation between incidence rate (IR) of thyroid cancer and severity of iodine deficiency. The influence of iodine prophylaxis on IR was also investigated. The suspension of iodine prophylaxis in 1980 resulted in goitre prevalence increase in schoolchildren (up to 60%) and elevated TSH levels in neonates observed in early 1990-ties. In 1990 rise in thyroid cancer IR was observed. IR of papillary and follicular carcinoma in 1995 were 1.6 and 1.5 respectively. Papillary to follicular ratio was about 1.0. Such a high prevalence of follicular cancer was s

ENDOCRINE TUMOURS: Calcitonin in thyroid and extra-thyroid neuroendocrine neoplasms: the two-faced Janus.

An increased calcitonin serum level is suggestive of a medullary thyroid cancer (MTC), but is not pathognomonic. The possibility of false positives or other calcitonin-secreting neuroendocrine neoplasms (NENs) should be considered. Serum calcitonin levels are generally assessed by immunoradiometric and chemiluminescent assays with high sensitivity and specificity; however, slightly moderately elevated levels could be attributable to various confounding factors. Calcitonin values >100 pg/mL are strongly suspicious of malignancy, whereas in patients with moderately elevated values (10-100 pg/mL)

Malignant thyroid neoplasm with ectopic Cushing's syndrome.

An adult man in his 30s with newly diagnosed diabetes mellitus and hypertension presented with a 33 lb weight gain, generalised swelling and classic Cushingoid features, including proximal muscle weakness, easy bruisability, wide violaceous striae and a painless, progressively enlarging right-sided neck swelling over the past month. Physical examination revealed a 4×4 cm thyroid mass, facial plethora and dorsocervical fat pads. Laboratory investigations confirmed hypercortisolism with elevated cortisol and Adrenocorticotropic Hormone(ACTH), with non-suppression on dexamethasone suppression tes

A delayed teflonoma of the neck simulating a thyroid neoplasm.

Pseudotumoral granulomatous foreign body reaction to Teflon particles is a rare complication of intracordal Teflon paste injection for unilateral vocal cord paralysis. We report a case of pseudotumor of the thyroid area, which appeared 3 years after intracordal Teflon paste injection and presented as a cold thyroid nodule. At frozen section examination the presence of a giant-cell foreign body granuloma containing numerous refringent particles suggested a 'teflonoma'. The diagnosis was confirmed on permanent sections, after comparison of the intratissular particles with fresh Teflon paste, und

Palpable parathyroid adenomas presenting as clinical solitary thyroid nodules and cytologically as follicular thyroid neoplasms.

BACKGROUND: Marfan Syndrome (MFS) and Ehlers-Danlos Syndrome (EDS) are hereditary connective tissue disorders with significant clinical variability, predominantly affecting cardiovascular, musculoskeletal, and vascular systems. Surgical interventions play a pivotal role in managing life-threatening complications such as aortic dissections in MFS and vascular ruptures in EDS. However, the long-term effectiveness of surgical strategies remains debated, necessitating a comprehensive evaluation of their outcomes to guide clinical decision-making. METHODOLOGY: A systematic review adhering to PRISM

Poorly differentiated ("insular") carcinoma of the thyroid gland: an aggressive subset of differentiated thyroid neoplasms.

Four patients with a histologically distinctive thyroid carcinoma--which recently has been referred to as poorly differentiated ("insular") carcinoma--are reported. This study confirms the previous conclusions that patients with this neoplasm often experience an aggressive clinical course, with focal recurrences and distant metastases common, which results in death in the majority of patients. Such aggressive behavior may occur even when the insular component accounts for only a small percentage of an otherwise well-differentiated carcinoma, as seen in one of our patients. After subtotal or to
