Tinnitus.

Tinnitus is a common otologic symptom secondary to numerous etiologies, such as noise exposure, otitis, Meniere's disease, otosclerosis, trauma, medications, and presbycusis. A thorough evaluation is necessary to rule out less common causes, which may include acoustic neuromas, glomus tumors, atherosclerosis of the carotid arteries, arteriovenous fistulae (AVFs), arteriovenous fistulae malformations (AVMs), and intracranial hypertension. Treating physicians need to have a very compassionate attitude towards these patients, and statements such as "there is nothing that can be done" are very ina…

Tinnitus.

Sound without external stimuli may warn of serious conditions. Accurate history and physical examination along with ancillary testing, including audiograms, are essential in evaluation of these patients. Evaluation of sudden tinnitus may save hearing. Extra-auditory tinnitus may arise from respiratory, vascular, and muscular sources that are often treatable. Conductive tinnitus may indicate treatable malformation of the external or middle ear. Sensorineural tinnitus may be drug-related, noise-related, of central origin, or due to cochlear deterioration. Comprehensive diagnostic procedures can …

Tinnitus and Reactions to Tinnitus: A Cross-Sectional Survey Across Different Tinnitus Durations.

Background/Objectives: Tinnitus and reactions to the tinnitus are different dimensions that can be explored in research and in clinical settings. Notably, these dimensions can elucidate priorities and the most problematic areas for patient-centered approaches. The aim of this study is to determine how tinnitus is perceived and impacts people who have experienced tinnitus for different durations. Methods: People with tinnitus were invited to participate in a survey at the University of Iowa Tinnitus Website. 709 people responded and documented their perceived sound, problems experienced, and du…

Tinnitus evaluation using the tinnitus grading system.

We developed a system to help evaluate the likelihood of a particular patient requiring intensive tinnitus treatment. The system is based on Vernon's tinnitus grading system, in which patients assign values from 1 to 5 to each of the three subjective complaints of loudness, annoyance, and interference with life activities. In our study, we collected data from 346 of our tinnitus patients and analyzed the relationship between the three values. On the basis of these results the patients were divided into five tinnitus-type groups. Scores on a tinnitus severity scale were also determined by addin…

Categorization of Tinnitus Severity for the Mandarin Tinnitus Questionnaire.

BACKGROUND: The Tinnitus Questionnaire is commonly used to evaluate the psychological impact of tinnitus and has been translated into Mandarin. The original English version of the Tinnitus Questionnaire was translated into Mandarin (Mandarin Tinnitus Questionnaire [MTQ]). The MTQ included not the same items compared with original version. Thus, MTQ should have its own severity categorization. AIMS/OBJECTIVES: The objective of this research was to develop a method to categorize tinnitus patients by clinical severity using scores from the MTQ. MATERIAL AND METHODS: A total of 192 participants …

Acceptance of Tinnitus As an Independent Correlate of Tinnitus Severity.

OBJECTIVES: Tinnitus is the experience of sounds without an identified external source, and for some the experience is associated with significant severity (i.e., perceived negative affect, activity limitation, and participation restriction due to tinnitus). Acceptance of tinnitus has recently been proposed to play an important role in explaining heterogeneity in tinnitus severity. The purpose of the present study was to extend previous investigations of acceptance in relation to tinnitus by examining the unique contribution of acceptance in accounting for tinnitus severity, beyond anxiety and…

Associations Between Physical Activity, Tinnitus, and Tinnitus Severity.

OBJECTIVES: To examine the associations between physical activity and tinnitus development and physical activity and tinnitus severity in a large representative sample of US adults. DESIGN: Data were obtained from 3826 eligible participants (20 to 69 years) in the National Health and Nutrition Examination Survey between 2015 and 2016. Physical activity was assessed using a Global Physical Activity Questionnaire. We used multivariable logistic regression to test the associations of physical activity (without physical activity, with physical activity) and amount of physical activity (min/week, …

Chronic Tinnitus.

BACKGROUND: Chronic tinnitus is a commonly occurring symptom of the auditory system. Epidemiological studies assume a lifetime prevalence in men and women of 3.5% for chronic tinnitus requiring treatment. Almost 25% of all Germans have experienced at least one episode of tinnitus. No causal therapy is yet available, but numerous treatment strategies are being pursued. Rigorous scientific assessment of these procedures is essential. METHODS: For this exhaustive revision of the German clinical practice guideline, the literature in the medical databases PubMed and Cochrane Library, including exi…

Somatic Tinnitus.

Modulation of tinnitus characteristics such as pitch and loudness has been extensively described following movements of the head, neck and limbs, vertical or horizontal eye gaze, pressure on myofascial trigger points, cutaneous stimulation of the hands, electrical stimulation of the median nerve, and transcranial direct current stimulation. Modulation of tinnitus follows complex interactions between auditory and somatosensory afferents and can be favored by underlying somatic disorders. When tinnitus appears to be preceded or strictly linked to a somatic disorder, and therefore related to prob…

[Chronic tinnitus].

Chronic tinnitus is a frequent symptom in the current medical practice. Patients presenting with chronic tinnitus have to be evaluated by comprehensive examination, including ENT status, audiometry and complete neuro-otological evaluation if required, to exclude an organic cause of tinnitus, as an external--or middle ear lesion, or a retro-cochlear process. Most often, chronic tinnitus is only associated with a sensorineural hearing-loss. Overall, no drug treatment can be proposed. However, the patient needs help and two techniques can be proposed to make the symptom more tolerable: the bio-ps…

Pulsatile tinnitus.

Pulsatile tinnitus is an uncommon otological symptom. Objective pulsatile tinnitus has numerous causes, including benign intracranial hypertension, glomus tumours and atherosclerotic carotid artery disease. History and physical examination can give important information as to the underlying diagnosis. Radiographic evaluation is essential in all patients with pulsatile tinnitus. Many patients have a treatable underlying aetiology. Early diagnosis and appropriate intervention may save patients from unnecessary morbidity.

[Somatosensory tinnitus].

BACKGROUND: Somatosensory tinnitus represents a clinically significant subgroup of chronic tinnitus. Although not completely understood, increasing evidence suggests interactivity between the somatosensory and auditory systems is responsible for both the development and maintenance of tinnitus. OBJECTIVES: The aim of this study is to provide an overview of the evaluation of somatosensory tinnitus and to propose an examination protocol to support the diagnosis and treatment of this patient group. MATERIALS AND METHODS: In addition to patient history, various clinical examination maneuvers are…

Tinnitus Update.

This article provides an update on tinnitus for audiologists and other clinicians who provide tinnitus-specific services. Tinnitus can be attributable to hearing loss, somatosensory system dysfunction, or auditory cortex dysfunction, with hearing loss being the most common cause and serious underlying pathologies being rare. Hearing loss does not always lead to tinnitus, and patients with tinnitus do not always suffer from hearing loss. The first scenario is explained by a so-called inhibitory gating mechanism, whereas the second assumes that all tinnitus sufferers have some degree of hearing …

Tinnitus Neuroimaging.

This article reviews the use of human neuroimaging for chronic subjective tinnitus. Evidence-based guidance on the clinical use of imaging to identify relevant auditory lesions when evaluating tinnitus patients is given. After introducing the anatomy and imaging modalities most pertinent to the neuroscience of tinnitus, the article reviews tinnitus-associated alterations in key auditory and nonauditory networks in the central nervous system. Emphasis is placed on how these findings support proposed models of tinnitus and how this line of investigation is relevant to practicing clinicians.

[Objective tinnitus].

Objective tinnitus is a known but rare otologic symptom. This is a case report about a six-year-old boy, who was referred to a hearing clinic for further investigation of an audible clicking sound. He experienced clicking sounds, and his parents were able to hear the sounds when listening nearby. This objective tinnitus is caused by myoclonus of either the m. tensor tympani or the m. stapedius. We discuss methods to evaluate objective tinnitus including the clinical use of a stethoscope and also the possibility of using impedance measuring in patients with objective tinnitus.
