BACKGROUND: Transitions of care often result in fragmented care, leading to unmet patient needs and poor satisfaction with care, especially in patients with multiple chronic conditions. This project aimed to understand how experiences of patients with hip fracture, caregivers, and healthcare providers differ across different points of transition.
METHODS: A secondary analysis of 103 qualitative, semi-structured interviews was conducted using emergent coding techniques, to gain an understanding of how transitional care experiences may differ across varying settings of care. Following the secon…

BACKGROUND: Transitional care management (TCM) supports timely postdischarge follow-up and is associated with decreased emergency department visits and 30-day readmissions. However, delivering TCM in rural settings remains challenging, and staff frequently report frustration with existing processes.
OBJECTIVE: This quality improvement study aimed to maximize the interdisciplinary team's expertise by applying a design thinking process. The study assessed current TCM practices, co-developed solutions, and iteratively implemented and evaluated interventions. This process enabled the identificati…

ESKD incidence rates are increasing, and mortality rates remain high. Fewer than 15% of patients use home dialysis modalities or receive preemptive kidney transplants. To address these shortcomings, executive order 13,879 (the Advancing American Kidney Health Initiative) directs the Centers for Medicare & Medicaid Services to encourage home dialysis and increase access to kidney transplants. Transitional care units (TCUs) have the potential to promote these goals by filling the gaps in care. TCUs are outpatient dialysis units for patients initiating dialysis with little or no predialysis care …

BACKGROUND: after hospitalisation for cardiac disease, older patients are at high risk of readmission and death.
OBJECTIVE: the cardiac care bridge (CCB) transitional care programme evaluated the impact of combining case management, disease management and home-based cardiac rehabilitation (CR) on hospital readmission and mortality.
DESIGN: single-blind, randomised clinical trial.
SETTING: the trial was conducted in six hospitals in the Netherlands between June 2017 and March 2020. Community-based nurses and physical therapists continued care post-discharge.
SUBJECTS: cardiac patients ≥ 70 …

BACKGROUND: The long-term effects of Hirschsprung disease are clinically variable. An improved understanding of challenges patients may face as adults can help inform transitional care management.
OBJECTIVE: To explore the outcomes and transitional care experiences in adult patients with Hirschsprung.
DESIGN: Cohort study.
SETTING: Single center.
PATIENTS: All patients treated for Hirschsprung between 1977 and 2001 (aged older than 18 years at the time of survey distribution in July 2018-2019). Eligible patients were sent validated multidomain surveys and qualitative questions regarding th…

OBJECTIVE: The indices to measure disease activity of chronic arthritis in adulthood and childhood are different. Therefore, assessing the status of the disease in young patients with juvenile idiopathic arthritis (JIA) can be tricky, especially when the transition to adult care is ongoing. The aim of our study was to assess the level of correlation between adult and juvenile scores in the measurement of disease activity in JIA patients during transitional care.
METHODS: We estimated the disease activity by using the Juvenile Arthritis Disease Activity Score 71 (JADAS71), clinical JADAS, adul…

The "transitional mobile team" has been developed to optimize the management of the transition from prison to the community for people with psychiatric disorders in France. The objectives are to limit the risk of relapse and death during this high-risk period and to ensure the links between psychiatry in prison and community psychiatry.

Background The objectives of this study were to develop and test in real-world clinical practice the effectiveness of a comprehensive postacute stroke transitional care (TC) management program. Methods and Results The COMPASS study (Comprehensive Post-Acute Stroke Services) was a pragmatic cluster-randomized trial where the hospital was the unit of randomization. The intervention (COMPASS-TC) was initiated at 20 hospitals, and 20 hospitals provided their usual care. Hospital staff enrolled 6024 adult stroke and transient ischemic attack patients discharged home between 2016 and 2018. COMPASS-T…

CONTEXT: Transition from the pediatric to the adult health care system is a complex process that should include medical, psychosocial, educational, recreational, and vocational considerations.
OBJECTIVE: In this systematic review, we aim to synthesize the evidence on transitional care interventions (TCIs) to improve the quality of life (QoL) for adolescents and young adults with childhood-onset disabilities, including neurodevelopmental disorders.
DATA SOURCES: Four electronic databases (Medline, Embase, PsycINFO, and Cumulative Index to Nursing and Allied Health Literature) were searched.

While the benefits of growth hormone (GH) therapy in adult hypopituitary patients with GH deficiency (GHD) are established, the role of continued GH therapy after final height in adolescent GH-deficient patients remains unclear. Preliminary data suggest that cessation of GH on completion of linear growth may be associated with impairment of somatic development and adverse changes in body composition. For the present time, the decision whether to continue GH treatment in adolescent patients with GHD is best made on an individual basis. For such patients, continuity of care is crucial. Children

BACKGROUND: Transitional care requires effective coordination between hospital and community services to ensure continuity of care and appropriate post-acute rehabilitation pathways. In this context, physiotherapists may contribute to discharge planning through standardized assessment of patients' functional status and rehabilitation needs.
OBJECTIVE: This study aimed to assess patients' functional status at hospital discharge, investigate variability in post-acute care allocation across three hospitals, and explore the contribution of physiotherapist-led functional assessment to hospital-to-…

BACKGROUND: Congestive heart failure (CHF) is a leading cause of adult hospitalization, with 30-day readmission rates remaining disproportionately high at approximately 25% despite medical advances.
PROJECT RATIONALE: Transitional-care interventions are critical for bridging the gap between inpatient and outpatient settings. Real-world data are necessary to validate the efficacy of dedicated CHF coordinators and guide resource allocation.
PROJECT SUMMARY: A retrospective cohort study evaluated 11,269 adults admitted with acute decompensated CHF from January 2020 through October 2025. Compari

People released from prison are at risk of adverse health outcomes in the weeks after release. This article describes cases that illustrate the complexity of caring for this population. It is important to address medical and psychological needs as well as factors that contribute to social determinants of health.

INTRODUCTION: Over 75% of adults in the United States live with at least one chronic condition, many of whom require surgery and need to see clinicians from multiple specialties. Literature on chronic care rarely describes the role of surgical teams in chronic disease management and, similarly, literature on transitions of care in surgery rarely examines long-term patient care needs. The objective of this study was to review how models of chronic disease management and transitions of care have been applied to surgical populations.
METHODS: Two reviewers independently screened the results from

PURPOSE: The purpose of this study was to identify elements of a stroke population that may affect transitions of care (TOC).
DATA SOURCES: A retrospective analysis of the demographic characteristics of patients from an urban primary stroke center with an admitting diagnosis of transient ischemic attack, acute ischemic stroke, subarachnoid hemorrhage, or intracerebral hemorrhage was performed over an 8-month period (N = 276). A subset of this patient sample participated in a telephone survey 1 month after discharge.
CONCLUSION: Hospital length of stay, age, insurance status, discharge dispos

Cystic fibrosis (CF) is a chronic childhood illness with gradually improving survival and significant burden of disease during adult life. Transition of CF care from pediatric to an adult based multidisciplinary team is a complex process and careful coordination with a transition key worker is necessary for successful transition without adverse outcome. Transition is associated with a key change in CF management with shift from family-centred care to self-reliance and independence on part of the patient. Readiness and skills of self-managed care play central part in successful transition. Reso
