[Trauma center management].

Every year, more than 20,000 patients with polytrauma are treated in Germany. The term polytrauma refers to simultaneous injury to several body regions that are individually or collectively life-threatening for the patient. However, this assessment is made based on appropriate scoring systems. Adequate treatment of these patients requires not only medical care at the highest level, but also coordination of organizational/logistical processes. The link between preclinical and clinical care is treatment in the shock room, which should be led by a defined, experienced "trauma leader". Treatment a

Practice Variation in Vena Cava Filter Use Among Trauma Centers in the National Trauma Database.

BACKGROUND: Agreement regarding indications for vena cava filter (VCF) utilization in trauma patients has been in flux since the filter's introduction. As VCF technology and practice guidelines have evolved, the use of VCF in trauma patients has changed. This study examines variation in VCF placement among trauma centers. MATERIALS AND METHODS: A retrospective study was performed using data from the National Trauma Data Bank (2005-2014). Trauma centers were grouped according to whether they placed VCFs during the study period (VCF+/VCF-). A multivariable probit regression model was fit to pre

Improving outcomes at Level I trauma centers: an early evaluation of the Trauma Survivors Network.

BACKGROUND: The Trauma Survivors Network (TSN), a program developed to help patients and families manage the psychosocial impact of their injuries, combines information access, self-management training, peer support, and online social networking. The purpose of this study was to evaluate the effectiveness of the TSN in improving patient reported outcomes among orthopedic trauma patients at a Level I trauma center. METHODS: We prospectively enrolled 251 patients with either severe lower-extremity injuries or polytrauma in two cohorts: one group (n = 125) before implementation of the TSN and on

Expanding the Public Health Role of Pediatric Trauma Centers: Drug Screening for Adolescent Trauma Patients.

BACKGROUND: Injury and overdose are leading causes of adolescent death in the United States, with high rates of problematic substance use among injured adolescents. We sought to determine whether drug screening prevalence and positivity at pediatric trauma centers have changed with the ongoing adolescent overdose crisis. STUDY DESIGN: This study evaluated trends in biochemical drug screening prevalence and positivity among injured adolescents (12-17 years) from the 2017-2022 Trauma Quality Improvement Program dataset. These were compared with contemporaneous trends in national adolescent over

Management of Pediatric Lower Extremity Vascular Trauma: Adult vs Pediatric Level I Trauma Centers [RETRACTED].

Источник пометил публикацию как отозванную

BACKGROUND: Pediatric lower extremity vascular injury (PLEVI) is uncommon. Though epidemiologic studies exist, the availability of granular data is sparse. Additionally, few studies compare the management between adult (ATC) and pediatric (PTC) trauma centers. The objective of this study was to analyze the surgical management of PLEVIs between a Level I ATC and a Level 1 PTCs in our metropolitan area. METHODS: We performed a retrospective review of all PLEVIs (age < 18 years) managed surgically between 01/2009–12/2022. Demographics, clinical and outcome data were obtained and compared between

Nationwide community based emergency centers reveal scale of hand trauma.

INTRODUCTION: This study describes the characteristics of hand trauma treated in community-based emergency centers (CBECs) in Israel. It was hypothesized that the rate of hand trauma, as well as patient and injury characteristics, would differ from a recent study of patients treated in hospital emergency departments (EDs) in Jerusalem. METHODS: Data on all hand injury patients treated at any of the CBECs belonging to a large nationwide chain in 2017 were retrieved from the electronic medical records of the treatment centers, including demographic and clinical characteristics. RESULTS: Over t

Closed midshaft femur fractures: are they only for trauma centers?

Trauma centers are limited resources, particularly in rural areas, and availability of emergency care in some parts of the United States may be inadequate. The declining number of orthopedic surgeons willing to care for injured patients has limited access to fracture repair in some communities. We studied the management of closed midshaft femur fractures in both trauma centers (TCs) and nontrauma centers (NTCs) to evaluate outcome for this common orthopedic injury and determine if these issues have affected fracture care in Kentucky. All patients 16-years-old and older who suffered femur fract

Penile Fracture Management at Trauma Centers in the United States.

INTRODUCTION: Current AUA urological trauma guidelines recommend prompt surgical intervention for patients presenting with acute penile fracture to minimize potential risks of erectile dysfunction and penile curvature. Given its relatively rare occurrence, there have been few large population-based studies evaluating real-world management of penile fracture. We hypothesized that there is variability in intervention for patients presenting with penile fracture associated with patients' clinical and demographic characteristics and hospital characteristics. METHODS: The Trauma Quality Improvemen

Epidemiology of suicide and the impact on Western trauma centers.

BACKGROUND: Suicide represents the eleventh leading cause of death in the United States. Most suicides are a result of self-directed injury and are commonly treated in trauma centers. The purpose of this article is to characterize the epidemiology of suicide in the Western Mountain states and to describe the impact of these injuries on trauma centers. METHODS: Data were accumulated from the National Trauma Data Bank, the American Association of Suicidology, and the Centers for Disease Control and Prevention Web-based Injury Statistics Query and Reporting System. Variables analyzed included th

Epidemiology of trauma at a level 1 trauma center.

We reviewed trauma registry data (2004-06) from a Level 1 trauma center to describe the epidemiology of injury. There were 5,900 trauma patients. Most admissions were among white men, age 20-29 years (mean age = 48 years). Most admissions occurred in the summer, on Saturday and Sunday evenings. Blunt injuries (89%) and fractures (49%) predominated. Mean ISS = 9.7. ISS = 9-15 was highest among those 70+ years and lowest among patients 30-39 years (P < 0.01). Motor vehicle crashes and falls were the most common mechanism of injury. Violent assaults ranked highest in the injury prevention priorit

Impact of trauma level designation on mortality in trauma patients with sepsis: an observational study across US trauma centers.

BACKGROUND: Sepsis is a major complication in trauma patients, leading to increased morbidity and mortality. Given the varying resource allocation across trauma center levels, the impact of trauma center designation on sepsis-related mortality remains unclear. This study examines the association between trauma center level and sepsis outcomes in trauma patients using data from the National Trauma Data Bank (NTDB) 2017 dataset. METHODS: A retrospective cohort study was conducted using the NTDB 2017 dataset at the American University of Beirut (AUB). Trauma patients who developed sepsis as a ho

Heterogeneity in managing rib fractures across non-trauma and level I, II, and III trauma centers.

BACKGROUND: We aimed to elucidate management patterns and outcomes of high-risk patients with rib fractures (elderly or flail chest) across non-trauma and trauma centers. We hypothesized highest-capacity (level I) centers would have best outcomes for high-risk patients. METHODS: We queried the 2016 National Emergency Department Sample to identify adults presenting with rib fractures. Multivariable regression assessed ED and inpatient events across non-trauma and level III/II/I trauma centers. RESULTS: Among 504,085 rib fracture encounters, 46% presented to non-trauma centers. Digit replantation: experience of two U.S. academic level-I trauma centers.

BACKGROUND: Despite advances in microsurgery, digit replantation now is performed less frequently in the U.S. compared with fifteen years ago. There has been uncertainty regarding whether previously reported U.S. replantation success rates and results reported from other countries reflect the current experience in the U.S. We hypothesized that the success of digit replantation at two academic level-I referral hospitals in the U.S. would be similar to previously published results. METHODS: In this retrospective case series, we examined all cases of digit replantation that were performed from 1

The Conundrum of Maintaining Pediatric Readiness at Nonpediatric Designated Trauma Centers.

INTRODUCTION: The American College of Surgeons (ACS) has recently highlighted the need for emergency departments to maintain pediatric readiness, regardless of pediatric trauma center verification. Since sufficient emergency department volume is often required to maintain pediatric competency, a more complete understanding of pediatric trauma volume at centers without ACS verification would help inform efforts to improve pediatric trauma readiness. METHODS: We conducted a retrospective descriptive analysis of public data sets from 2017 through 2023 from the Trauma Quality Improvement Program
