Thirteen consecutive referrals of bulimic patients who met DSM-III criteria for bulimia were treated in an open-label, flexible-dose study with trazodone. Three of the 13 dropped out before the fourth week of treatment, the minimum duration of treatment for evaluable subjects, and hence were not included in the analyses. Priapism associated with medication use is well known as one of the most difficult forms of priapism to treat. We report 2 cases of priapism and prolonged erection associated with trazodone therapy. The literature is reviewed and potential treatment options are discussed.

The need to maintain long-term treatment of chronic pathologies makes the appearance of interactions possible when such therapies incorporate other drugs to deal with the aggravation of the same or other intercurrent pathologies. We describe the case of a 37-year-old man who had suffered from exhibitionism for 10 years. The strength of the patient's urges to act out decreased gradually with the commencement of low-dose trazodone treatment. During the 2 years of trazodone treatment, there was no acting out. The medicine was gradually decreased and eventually discontinued. The patient's impulse manifested only once, but is responded well to trazodone resumption. Priapism is a prolonged erection lasting more than four hours. In most cases, it is a surgical emergency. Trazodone is one of the molecules that can cause priapism.
