Percutaneous balloon compression for trigeminal neuralgias and autonomic cephalalgia.

OBJECTIVE: This prospective study aimed to evaluate the results of percutaneous trigeminal ganglion balloon compression (BC) in patients with various types of trigeminal neuralgia (TN) and autonomic cephalalgia. METHODS: Twenty-five consecutive patients underwent BC and were followed up for 27-60 months. They were divided into 2 groups: group A (n=18) patients with idiopathic TN and group B (n=7) patients with secondary TN (n=5) and trigeminal autonomic cephalalgia (TAC) (n=2). RESULTS: Postoperatively, 15 patients in group A experienced pain relief, one required medication and 2 had no resp

Complementary and Integrative Medicine for the Treatment of Trigeminal Neuralgia and Trigeminal Autonomic Cephalalgia.

PURPOSE OF REVIEW: Trigeminal neuralgia (TN) and trigeminal autonomic cephalalgias (TACs) are both painful diseases which directly impact the branches of the trigeminal nerve, which supply the face. Patients who have experienced adverse effects, have not responded to mainstream treatments, or have a personal preference for nonmedication options, often turn to complementary and integrative medicine (CIM). The aim of this review is to discuss the efficacy and safety of CIM therapies available for the treatment of TN and TACs. RECENT FINDINGS: Not only are there limited therapeutic options for T…

Pediatric-onset trigeminal autonomic cephalalgias: A systematic review and meta-analysis.

BACKGROUND AND OBJECTIVE: There are five headache disorders composing the trigeminal autonomic cephalalgias (cluster headache, paroxysmal hemicrania, short-lasting unilateral neuralgiform headache attacks with conjunctival injection and tearing (SUNCT), short-lasting unilateral neuralgiform headache attacks with cranial autonomic symptoms (SUNA), and hemicrania continua). Little is known about these disorders in the pediatric population. The objectives of this study are to report the full age ranges of pediatric trigeminal autonomic cephalalgias and to determine if pediatric-onset trigeminal a…

Neuroimaging and clinical neurophysiology in cluster headache and trigeminal autonomic cephalalgias.

Clinical neurophysiology and neuroimaging are two non-invasive approaches used to investigate the pathophysiological basis of primary headaches, including cluster headache (CH) and other trigeminal autonomic cephalalgias (TACs). Modern neuroimaging has revolutionized our understanding of the pathophysiology of primary headaches, and of TACs in particular, focusing on a cerebrovascular dysfunction hypothesis toward a central triggering cause. The introduction of single-photon emission computed tomography (SPECT), positron emission tomography (PET), and voxel-based morphometry has allowed us new…

Trigeminal Autonomic Cephalalgia and Facial Pain: A Review and Case Presentation.

Trigeminal autonomic cephalalgias (TAC) are primary headache disorders that are characterized by severe unilateral pain along the distribution of the trigeminal nerve with corresponding activation of the autonomic nervous system. The clinical characteristics and presentation of TAC are unique; however, there may be an overlap of these features with other painful conditions affecting the orofacial region, which can be a diagnostic challenge for the clinician. This article reports a case history and discusses the differences between clinical characteristics of TAC and other painful orofacial con…

Efficacy of Nerve Blocks for Posttraumatic Trigeminal Autonomic Cephalalgias: A Case Series.

BACKGROUND: Posttraumatic headaches (PTHs) frequently occur following mild traumatic brain injury and may occasionally present with features of trigeminal autonomic cephalalgias (TACs). PTH-TAC is characterized by unilateral craniofacial pain and autonomic symptoms and can be significantly debilitating. Management of this headache subtype can be particularly challenging as it can be refractory to medical therapies. Currently, there is a paucity of evidence on the optimum medical management for this condition. OBJECTIVES: This case series evaluated the effectiveness of nerve blocks in managing…

Trigeminal Autonomic Cephalalgias in Children and Adolescents: Cluster Headache and Related Conditions.

Cluster headache is a primary headache disorder that can occur in children and adolescents, and is a member of the broader diagnostic group of trigeminal autonomic cephalalgias. It is characterized by repeated attacks typically lasting between 15 and 180 minutes of severe unilateral side-locked headache with cranial autonomic features. Acute treatment of the cluster attack can include the use of triptans or high-flow oxygen. Preventive measures typically start with the use of verapamil. The other trigeminal autonomic cephalalgias, paroxysmal hemicrania, short-lasting unilateral neuralgiform he…

Management of cluster headache and other trigeminal autonomic cephalalgias in pregnancy and breastfeeding.

Many clinicians lack experience in managing trigeminal autonomic cephalalgias (TACs) in pregnancy and lactation. In addition to cluster headache, TACs include hemicrania continua, paroxysmal hemicrania, and short-lasting unilateral neuralgiform headache with conjunctival injection and tearing/autonomic symptoms (SUNCT/SUNA). Treating these rare, severe headache conditions often requires off-label drugs that have uncertain teratogenic potential. In the last few years, several new treatment options and safety documentation have emerged, but clinical guidelines are lacking. This narrative review …

[The hypothalamus in SUNCT syndrome: similarities and differences with the other trigeminal-autonomic cephalalgias].

INTRODUCTION: SUNCT belongs to the group of trigeminal-autonomic cephalalgias (TAC) --cluster headache and paroxysmal hemicranias--, since its shares a series of features with them. SUNCT was finally included in this group when the hypothalamus was proved to play a key role in its pathophysiology, an aspect that it has in common with other TAC. However, its clinical resemblance to trigeminal neuralgia of the first branch is notable, although it is accepted that the genesis of the trigeminal neuralgia is peripheral. DEVELOPMENT: The article presents the evidence available to date that has made

Symptomatic trigeminal autonomic cephalalgia associated with allodynia in a patient with multiple sclerosis.

A patient with symptomatic trigeminal autonomic cephalalgia (TAC) provides a chance to understand the pathophysiology and anatomic correlates of TAC. A 28-year-old woman experienced intermittent sharp and excruciating pain over her right temporal, ear and neck regions for 3 days. The headaches lasted 10-20 minutes each, occurred 1-2 times a day, and were accompanied by prominent ipsilateral lacrimation and conjunctival injection. The patient had hiccups, 4-limb numbness and impaired visual acuity in both eyes. She had also had 3 episodes of left-side optic neuritis in the past half year. Neuro

CT-Guided Thermocoagulation of the Pterygopalatine Ganglion for Refractory Trigeminal Autonomic Cephalalgia.

INTRODUCTION: Trigeminal autonomic cephalalgia (TAC) is a type of one-sided cerebral painful headache, with attacks regularly accompanied by autonomic responses, such as tearing, runny nose, panic, nausea and vomiting on the affected side. Currently, the most common treatment strategies are drugs, nerve grafts and surgery. Clinical understanding of TACs is limited. Here, we report the case of thermocoagulation treatment of the pterygopalatine ganglion in an uncommon TAC under local anesthesia. CASE PRESENTATION: A rare case of TAC was treated with computed tomography (CT)-guided thermocoagula

Aura in trigeminal autonomic cephalalgia is probably mediated by comorbid migraine with aura.

OBJECTIVE: The presence of aura is rare in cluster headache, and even rarer in other trigeminal autonomic cephalalgias. We hypothesized that the presence of aura in patients with trigeminal autonomic cephalalgias is frequently an epiphenomenon and mediated by comorbid migraine with aura. METHODS: The study retrospectively reviewed 480 patients with trigeminal autonomic cephalalgia in a tertiary medical center for 10 years. Phenotypes and temporal correlation of aura with headache were analyzed. Trigeminal autonomic cephalalgia patients with aura were further followed up in a structured teleph…

TAC-Tic syndromes: a case series on the complex overlap between trigeminal autonomic cephalalgias and trigeminal neuralgia.

INTRODUCTION: Trigeminal autonomic cephalalgias (TACs) and trigeminal neuralgia (TN) are classified as distinct disorders, although overlap syndromes are increasingly recognized. CASE REPORTS: We retrospectively describe five women with TAC-tic overlap: two with SUNCT-tic, two with cluster-tic, and one with paroxysmal hemicrania-tic syndrome. Each had distinguishable neuralgic and autonomic pain phenotypes, appearing either simultaneously or sequentially. In both SUNCT-tic cases, repeat imaging disclosed neurovascular compression after an initially negative study; microvascular decompression …

Ventral tegmental area deep brain stimulation for trigeminal autonomic cephalalgias: a comprehensive literature review.

Trigeminal autonomic cephalalgias (TACs) - including chronic cluster headache (CCH), short-lasting unilateral neuralgiform headache attacks (SUNHA, encompassing SUNCT and SUNA), and paroxysmal hemicrania - are rare but severely disabling primary headache disorders. A systematic search of PubMed, Embase, and Cochrane (inception to December 2024) identified studies on deep brain stimulation (DBS) for TACs. A meaningful proportion of patients remain refractory to optimized pharmacotherapy, and neuromodulation has emerged as a promising avenue. The ventral tegmental area (VTA), a dopaminergic midb…

Tolosa-Hunt syndrome presenting with features of a trigeminal autonomic cephalalgias and pituitary enlargement.

Tolosa-Hunt syndrome is understood as a steroid-responsive, relapsing-remitting, unilateral headache disorder associated with ipsilateral cranial neuropathies, of a probable granulomatous aetiology. The diagnosis is made clinically from the history and examination, supported by appropriate imaging. Here the authors report a case of Tolosa-Hunt syndrome with a headache phenotype mimicking a trigeminal autonomic cephalalgias (hemicrania continua), and serial MRI studies showing a stable enlarged pituitary. Due to her initial lack of clinical signs, she was diagnosed with chronic migraine, revise…

Diagnostic and therapeutic errors in trigeminal autonomic cephalalgias and hemicrania continua: a systematic review.

Trigeminal autonomic cephalalgias (TACs) and hemicrania continua (HC) are relatively rare but clinically rather well-defined primary headaches. Despite the existence of clear-cut diagnostic criteria (The International Classification of Headache Disorders, 2nd edition - ICHD-II) and several therapeutic guidelines, errors in workup and treatment of these conditions are frequent in clinical practice. We set out to review all available published data on mismanagement of TACs and HC patients in order to understand and avoid its causes. The search strategy identified 22 published studies. The most f…
