BACKGROUND: Isolated trigeminal neuropathy is uncommon; causes include trauma, inflammation, or neoplasm.
METHODS: We report a patient who fell and struck his head during a myocardial infarction, was treated with streptokinase, and developed symptoms and signs of an isolated trigeminal sensory neuropathy.
RESULTS: Imaging showed hemorrhage in the trigeminal nerve root; follow-up imaging showed resolution of the hemorrhage, but no underlying structural lesion.
CONCLUSION: A combination of head trauma plus thrombolysis resulted in an isolated trigeminal neuropathy.

Pure trigeminal motor neuropathy is represented by trigeminal motor weakness without signs of trigeminal sensory or other cranial nerve involvement. We present a case of 45-year-old male complaining of difficulty in chewing with facial asymmetry on right side. He had no sensory disturbances. History, neurological examination, CT and MRI led to the diagnosis of unilateral trigeminal motor nerve neuropathy. The cause may have been an autoimmune reaction to the viral infection.

The artery to trigeminal nerve ganglion has been identified as a branch that arises from the extracranial segment of middle meningeal artery prior to entry into the foramen spinosum. The role of the artery in being the arterial supply to trigeminal nerve ganglion is supported by electrophysiological and clinical suppression of trigeminal nerve activity during selective intra-arterial injection of anesthetics.

Injuries to the trigeminal nerve branches are a known and accepted risk in oral and maxillofacial surgery. It is prudent for the practitioner to explain the risks to patients as part of the informed consent process and to recognize and document the presence of nerve injury postoperatively. Patients should be referred to a surgeon experienced in microsurgical techniques in a timely fashion for evaluation and possible surgical intervention if an injury is not resolving.

Microvascular decompression of the root of the trigeminal nerve was carried out in 35 patients with trigeminal neuralgia. The principles of the surgical interventions and the intraoperative findings are described. The peculiarities of neurovascular conflict are analysed in comparison with the neurovascular conflict are analysed in comparison with the of the trigeminal root caused by the main trunk and branches of the superior cerebellar artery, the anterior inferior cerebellar artery, and the superior petrosal vein, relieved pain completely in all cases. The advantages of microvascular decompr…

Injuries to the inferior alveolar nerve (IAN) and the lingual nerve are relatively uncommon. However, there are certain routine surgical and non-surgical procedures that have the potential, albeit small, for injuring these neural structures. A review of the diagnosis, classification and management of these injuries is presented.

BACKGROUND: While high-resolution imaging is increasingly used in guiding decisions about surgical interventions for the treatment of trigeminal neuralgia, direct assessment of the extent of vascular contact of the trigeminal nerve is still considered the gold standard for the determination of whether nerve decompression is warranted.
OBJECTIVE: To compare intraoperative and magnetic resonance imaging (MRI) findings of the prevalence and severity of vascular compression of the trigeminal nerve in patients without classical trigeminal neuralgia.
METHODS: We prospectively recruited 27 patients…

OBJECTIVE AND IMPORTANCE: Sarcoidosis most commonly presents as a systemic disorder. Infrequently, sarcoidosis can manifest itself in the central nervous system, with granulomas involving the leptomeninges and presenting with facial nerve weakness. Sarcoid of the trigeminal nerve is exceedingly rare and can mimic trigeminal schwannoma. We review the literature on sarcoid granulomas of the trigeminal nerve and compare their radiological features with the more common schwannoma.
CLINICAL PRESENTATION: A 33-year-old woman presented with a history of left-sided facial pain and numbness for 11 mon…

The trigeminal nerve is the fifth cranial nerve and is a sensory-motor nerve that provides the innervation to the face with its three roots. The trigeminal nerve can be affected by several diseases, such as vascular conflict, congenital malformation, inflammatory or neoplastic diseases. Magnetic Resonance Imaging plays a crucial role in its evaluation. This article aims to review the trigeminal nerve anatomy, discuss the best magnetic resonance imaging techniques to evaluate each nerve segment, and demonstrate the imaging aspect of the diseases that most commonly affect it.

The objective of this study is to review surgical anatomy of the trigeminal nerve. We also demonstrate some pictures involving the trigeminal nerve and its surrounding connective and neurovascular structures. Ten adult cadaveric heads were studied, using a magnification ranging from 3× to 40×, after perfusion of the arteries and veins with colored latex. The trigeminal nerve is the largest and most complex of the cranial nerves. It serves as a major conduit of sensory input from the face and provides motor innervation to the muscles of mastication. Because of its size and complexity, it is ess…

We studied the trigeminal nerve in the middle cranial fossa in 20 cadaveric specimens (10 fixed skull base). Specifically, we analysed the relationships among the gasserion or trigeminal ganglion (herein referred to as the gasserion ganglion), the internal carotid artery, and the petrous bone. The dimensions of the trigeminal ganglia, the length from the ganglia to the exit foramina, and the width of the ophthalmic, maxillary, and manidibular nerves were also studied. The foramina of the middle cranial fossa were measured for their diameter and their distance from each other. Our paper compare…

Traumatic injury to the peripheral nerves often results in persistent discomfort. Substance P has been implicated as a mediator of pain, and depletion of this neurotransmitter has been shown to reduce pain. Subjects suffering from traumatic dysesthesia of the trigeminal nerve were treated with capsaicin, a substance P depleter with significant long-term effects. This form of therapy may be used individually or in combination with other pharmacologic interventions in the treatment of traumatic trigeminal dysesthesia.
