PURPOSE OF REVIEW: The advent of anti-PD-1 therapy has transformed the treatment paradigm of advanced melanoma, enabling durable responses in a proportion of patients. However, the impact of both primary and acquired resistance remains a significant barrier to sustained disease control. This review explores the biological rationale and clinical progress of anti-PD-1 based combination strategies, with a particular emphasis on emerging triplet regimens and next-generation immunotherapeutic approaches designed to extend survival and overcome resistance.

Heterotopic pregnancy refers to the concomitant presence of an intrauterine pregnancy and an ectopic pregnancy (EP). It is rare and more frequently found in women who have undergone medically assisted procreation. An abdominal location of the ectopic gestational sac is even less common, accounting for 1.4% of all ectopic pregnancies, conferring a high risk of morbidity and mortality. INTRODUCTION: Craniosynostosis is the premature fusion of one or more cranial sutures. The cause of non-syndromic craniosynostosis has been attributed to a complex interaction among genetic, epigenetic, and environmental factors. Increased concordance rates in monozygotic twins support a genetic etiology while a concordance rate less than 100% suggests environmental and/or epigenetic influences. This study aims to compare morbidity indicators between twins and triplets. This was a retrospective, observational study, and involved a review of twins and triplets born in Basildon Hospital between 1991 and 2000. To determine the incidence of multiple births and associated morbidity and mortality, we collected in a retrospective study all the multiple births (twins excluded) in Switzerland from 1985 to 1988. In all we followed 77 sets of triplets and 9 sets of quadruplets, representing an annual incidence of 1/3968 births for the triplets and 1/33,947 births for the quadruplets. A 38-year-old woman, gravida 5, para 3, with a triplet pregnancy discordant for anencephaly, is reported. The diagnosis was made prenatally by ultrasound. Autopsy of the anencephalic member revealed small adrenal glands with absent fetal cortex, and a coincidental tetralogy of Fallot. Studies of the histocompatible lymphocyte antigens of both parents were performed and they failed to demonstrate HLA B-27. The literature on anencephaly in triplets is summarized. The association of congenital heart disease and HLA B-27 with anencephaly is discussed.
