Unnecessary Needling: A Case of Iatrogenic Pneumothorax Following Dry Needling Procedure for Chronic Myofascial Pain.

This case report highlights a complication of pneumothorax associated with dry needling (DN), a technique used for the treatment of myofascial pain syndrome and musculoskeletal disorders. Despite its growing popularity and efficacy in relieving pain, dry needling can lead to adverse events. We present a case of a 35-year-old female who developed pneumothorax following a dry needling session. During the dry needling session, the patient reported sharp pain underneath the scapula, and pneumothorax was confirmed three days later on a visit to the emergency department. Even though the chances of a

Suspected common bile duct stones: reduction of unnecessary ERCP by pre-procedural imaging and timing of ERCP.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) is the procedure of choice to remove sludge/stones from the common bile duct (CBD). In a small but clinically important proportion of patients with suspected choledocholithiasis ERCP is negative. This is undesirable because of ERCP associated morbidity. We aimed to map the diagnostic pathway leading up to ERCP and evaluate ERCP outcome. METHODS: We established a prospective multicenter cohort of patients with suspected CBD stones. We assessed the determinants that were associated with CBD sludge or stone detection upon ERCP. R

Early Application of Permanent Metallic Mesh Stent in Substitution for Temporary Polymeric Ureteral Stent Reduces Unnecessary Ureteral Procedures in Patients With Malignant Ureteral Obstruction.

OBJECTIVE: To identify risk factors for metal stent failure in patients who received polymeric double J (PDJ) ureteral stents for malignant ureteral obstructions (MUOs) and review our clinical experiences using a ureteral metallic stent. PATIENTS AND METHODS: Patients who underwent metallic stent placement to replace a double J ureteral stent for nonurological MUO between January 2011 and February 2014 were included. The collected data included gender, age, laterality, cause of obstruction, PDJ ureteral stenting duration, immediate success of the metal stent, and additional procedures to reli

Role of bone single photon emission computed tomography/computed tomography in managing chronic fracture-related infections: Determining the depth of infection and avoiding unnecessary bone procedures.

PURPOSE: Fracture-related infections (FRIs) encompass a broad range of infections associated with bone fractures; they remain a significant clinical challenge. Here, we aimed to investigate the viability of focusing on soft-tissue management in patients suspected of chronic FRI, who exhibit no significant bony uptake on bone single photon emission computed tomography (SPECT)/computed tomography (CT) scans. METHODS: Between January 2016 and January 2022, we managed 25 patients with chronic FRI or post-traumatic osteomyelitis using technetium 99m-methyl diphosphonate bone SPECT/CT to assess inf

Is Radiological Reporting of 'Tension' in Spontaneous Pneumothorax Driving Unnecessary Intervention?

Decompensated (aka tension) primary spontaneous pneumothorax (PSP) is extremely rare. There is no published evidence that conservative treatment of stable patients with radiological features of so-called 'tension' (e.g., tracheal deviation and mediastinal shift) results in adverse outcomes. There is also preliminary evidence that these features do not correlate with significant clinical instability. However, anecdotally, clinicians report being uncomfortable not providing interventional treatment for stable PSP patients with radiological features of tension, in part for fear of litigation. Thi

Recommendations of «not-to-do»: Proposals of the Spanish Association of Surgeons to the project «Commitment to quality of scientific societies».

INTRODUCTION: The initiative of the Spanish Ministry of Health «Commitment to quality of scientific societies», aims to reduce unnecessary interventions of healthcare professionals. METHODS: The Spanish Association of Surgeons has selected 22 experts from the different sections that have participated in the identification of 26 proposals «do not do» to be ordered by the expected impact its implementation would have according to the GRADE methodology. From these proposals, the Delphi technique was used to select 5 recommendations presented in more detail in this article. RESULTS: The 5 select

Why do cesarean delivery rates persistently rise despite evidence-based efforts to reduce them?

BACKGROUND: Globally, cesarean delivery rates have increased steadily over the past 3 decades, beyond medically indicated limits. Research in the area tends to analyze clinical indications, with little attention to the nonclinical factors that are drivers of these increasing rates. This integrative review systematically synthesizes evidence on the structural, systemic, and sociocultural contributors to the global increase in cesarean delivery rates. OBJECTIVE: Critically identify the nonclinical determinants of global cesarean section overuse. STUDY DESIGN: A comprehensive integrative review

[Relevance of imaging procedures: a sustainable development approach].

A SUSTAINABLE DEVELOPMENT APPROACH The overuse of medical imaging is a global issue, observed across all countries regardless of income level or healthcare system, whether public or private. Establishing precise criteria to assess the appropriateness of care and quantifying unnecessary services remains a complex challenge. Overuse not only causes physical, psychological, and financial harm to patients but also diverts resources away from public health and involves social expenditures. Moreover, it contributes to an environmental burden with consequences that ultimately impact population health

Unnecessary ERCPs: Is Spontaneous Stone Passage the Sole Determinant?

Endoscopic retrograde cholangiopancreatography (ERCP) is the cornerstone in the management of choledocholithiasis. Despite continuous advancements in technique and safety, ERCP carries a risk of significant complications, underscoring the importance of avoiding unnecessary procedures. The principal contributor to potentially avoidable ERCPs in patients with known choledocholithiasis is the spontaneous passage of common bile duct stones. Small stone size and a long interval between diagnosis and the procedure have increasingly been found to favor this event. Moreover, despite the development of…

ACOG Committee Opinion No. 754: The Utility of and Indications for Routine Pelvic Examination.

The pelvic examination has long been considered a fundamental component of the well-woman visit, and many women and gynecologic care providers view this visit as an opportunity to discuss sexual and reproductive health issues. Traditionally, a pelvic examination is performed for asymptomatic women as a screening tool for gynecologic cancer, infection, and asymptomatic pelvic inflammatory disease; some obstetrician-gynecologists and patients consider it important in detecting subclinical disease, despite evidence to the contrary. Given changes in screening recommendations and the ability to scr
