BACKGROUND: Female urethral stricture (FUS) is a rare entity that causes great morbidity and suffering in those affected. As the available scientific data is sparce, there are no formal guidelines or standard of care for this disease.
METHODS: This is a narrative review of the surgical management for female urethral stricture. The literature review was performed on PubMed. Articles were limited to English, but there was no limitation in terms of date.
RESULTS: Management of FUS is divided between endoscopic and open surgical repair. Urethral dilation with or without urethrectomy can be offer…

Great progress has been seen in the treatment of urethral strictures since the first endoscopic urethrotomy was performed in 1893 by Felix Martin Oberländer in Dresden, Germany. With the introduction of endoscopic laser therapy and the variety of urethral reconstruction methods other ways to treat this important urologic entity became available. Despite this progress, urethrotomy still represents the preferred treatment concept for primary, short and bulbar urethral strictures. In this study we performed a 2-year retrospective analysis of 20 patients undergoing primary endoscopic urethrotomy b…

OBJECTIVE: To study the relationship between urethral stricture and infertility, which has not been well studied in the past.
PATIENTS AND METHODS: In this study in West Africa, the mean semen values from 90 fertile controls and 32 infertile controls were compared to 40 men with incomplete urethral strictures (IUS) and 56 men with complete urethral strictures after treatment. Testicular biopsies were performed on 30 patients. Sexually transmitted urethritis accounted for 79% of the strictures within this group of patients. Repair of the strictures restored ejaculate in 39 men, and 22 men had …

After 101 transurethral resections, iatrogenic postoperative urethral strictures have occurred in 10 cases due to pathologic changes in the bladder and bladder neck. Urethral stricture after transurethral resection (TUR) thus has represented the most common postoperative complication. No morbidity increase of urethral strictures after TUR could be demonstrated in one of the analysed groups. Fifty per cent of the incidence rate of strictures was localized in the pars pendulans urethrae. Factors causing the stricture have been demonstrated, and the incidence rate of strictures was reduced when t…

Radiation-induced urethral stricture occurs most often due to radiation for prostate cancer. It is one of the most common side effects of radiotherapy. Stricture rates are lowest in patients undergoing external beam radiation therapy, occur more frequently in those who require brachytherapy and show highest stricture rates in patients receiving a combination of external beam radiation and brachytherapy. Strictures are mostly located at the bulbomembranous part of the urethra. Diagnostic work-up should include basic urologic work-up, ultrasound, uroflowmetric assessment, urethroscopy, retrograd…

Accurate imaging of urethral strictures is critical for preoperative staging and planning of reconstruction. The current gold standard, retrograde urethrography (RUG), allows for accurate diagnosis, staging, and delineation of urethral strictures, and remains a cornerstone in the management of urethral stricture disease. In complex situations, the RUG can be combined with voiding cystourethrogram (VCUG) in order to better visualize the posterior urethra or complex distraction defects. Direct visualization of the stricture by cystoscopy, either retrograde or antegrade, can provide additional in…

The surgical treatment of urethral stricture diseases is continually evolving. Although various surgical techniques are available for the treatment of anterior urethral stricture, no one technique has been identified as the method of choice. This article provides a brief updated review of the surgical options for the management of different sites and different types of anterior urethral stricture. This review also covers present controversies in urethral reconstruction. Among the various procedures available for treating urethral stricture, one-stage buccal mucosal graft urethroplasty is curre…

An unusual case of a 5-year-old boy with duplication of the glans, a rare variety of diphallia, is reported. The two glandes were arranged one above the other, the dorsal glans had only a blind pit while the ventral glans had a patent urethra with glanular hypospadias. The child also had right renal agenesis and a posterior urethral stricture.

We review our experience with urethral stricture disease in 25 children who were seen between 1970 and 1979. Trauma was the most common etiology (48 per cent). Excellent results were obtained by 1-stage repair and multistage skin inlay urethroplasty. An average of 3.8 procedures per patient was required for multistage repairs versus 1.1 procedures per patient for 1-stage repair. Dilation alone averaged 1.7 procedures per patient and was successful in only 20 per cent. Dilation is unacceptable for management of most strictures in children and urethroplasty should be considered early in the trea…

In this 3-part study the distribution of the post-gonococcal stricture has been related to the disposition of mucous glands in paraurethral glands by mapping the distribution of these glands in the human male urethra. Experimental study in the rat has indicated the role of extravasation of urine in the pathogenesis of urethral stricture. Lastly, the ultrastructure of urethral stricture tissue has revealed features that suggest why some strictures are fibrous and others resilient.

PURPOSE OF THE REVIEW: The review is an attempt to present the current literature regarding strictures in children to provide an evidence-based recommendation regarding diagnosis and treatment.
RECENT FINDINGS: The level of evidence upon which to base therapy of strictures in children is low and consists mostly of case series from single institutions. Hence most of the information is extrapolated from the adult literature. A consensus committee of the Societe Internationale d'Urologie reviewed the literature, and the committee report (which is as yet unpublished) provides the basis for most o

Urethral strictures represent a relevant problem, particularly among urological patients, since they can also develop in conjunction with endourological instrumentation. After functional and radiological diagnostics, the choice of an individualized treatment concept is foremost while taking into consideration minimally invasive endourological procedures and techniques of surgical reconstruction. The best long-term results for this disorder, which has a considerable tendency to recur, are achieved with open reconstructive methods whereas urethrotomy pursues a curative approach only in cases of …

Thirty-six children have been treated for a non-hypospadiac urethral stricture. Of 12 patients with meatal or submeatal stenosis, 10 had undergone circumcision for balanitis xerotica obliterans. The strictures were successfully treated by meatoplasty or meatal dilatation. Twenty-four children had a more proximal urethral stricture: 16 were caused by urethral catheterisation, 4 were post-traumatic, 2 were congenital and 2 were idiopathic. Sixteen children were treated by visual urethrotomy; this was successful in 12 after a maximum of 2 urethrotomies. Two children required 4 or more urethrotomi…

Our experience with urethral strictures in boys during a 15-year period confirms the findings of others that the most common etiology is iatrogenic. Traumatic and inflammatory strictures are rare. The congenital stricture differs fundamentally from acquired types of urethral strictures and would be termed more appropriately congenital urethral membrane. Urethral dilation and/or urethrotomy was unsuccessful in 47% of our patients, leading to secondary formation of a new stricture in 2 instances. However, urethroplasty was successful in 83% of our cases and seems to be indicated when 2 or more d…

A post-mortem study of the urethral anatomy following gonorrhoea is presented. The different specimens examined demonstrated lesions ranging from microscopic submucosal scars to fully developed urethral strictures. In every specimen extensive squamous metaplasia of the surface epithelium was present. It is postulated that overstretching of this unsupported squamous epithelium during voiding causes microscopic epithelial tears and that the accumulation of the subsequent microscopic scars leads to the formation of a urethral stricture. Bacterial infection does not seem to play any part in the ge…

BACKGROUND: Urethral strictures have affected men for millennia, and they are commonly iatrogenic, idiopathic or traumatic in nature. When left untreated, urethral strictures can devastate the health and function of the urinary tract. For centuries, simple urethral dilation and endoscopic urethrotomy have provided temporary relief. However, in only the past 20 years, urethroplasty has emerged as a curative treatment.
OBJECTIVE: The aims of this article are to review the aetiology and clinical manifestations of urethral strictures, evaluate the traditional but temporising treatments for urethr…
