BACKGROUND: Bladder outlet obstruction (BOO) and detrusor underactivity (DU) can produce overlapping clinical and uroflowmetric presentations in men with lower urinary tract symptoms (LUTS). Pressure-flow studies (PFS) provide reference diagnoses, but their invasiveness and cost constrain access. Building on a single-center model that used only structured clinical, uroflowmetric, and ultrasonographic variables, this protocol adds uroflowmetry waveforms and cystoscopic images within multimodal artificial neural networks (ANNs). Whether these inputs improve diagnostic estimation beyond structure

AIM: To examine the symptoms and noninvasive test parameters that clinically differentiate detrusor underactivity (DU) from bladder outlet obstruction (BOO) without using a pressure-flow study (PFS) in male patients with lower urinary tract symptoms.
METHODS: We examined 909 men who underwent PFS and also had the following data: International Prostate Symptom Score (IPSS), free uroflowmetry, postvoid residual (PVR), and prostate volume (PV). Using these data, significant symptoms and noninvasive test parameters that clinically differentiate DU from BOO were examined.
RESULTS: Significant par

INTRODUCTION: Differentiating bladder outlet obstruction (BOO) from detrusor underactivity (DU) remains a major unmet need in the management of lower urinary tract symptoms in men. Although pressure-flow urodynamics provide a definitive physiological classification, their invasiveness limits their widespread use. We reviewed why current non-invasive approaches have failed to achieve consistent clinical adoption and explored a pathway toward clinically useful pre-urodynamic triage.
METHODS: This hypothesis-generating narrative review synthesized evidence on non-invasive BOO/DU assessment, usin

INTRODUCTION: Overactive bladder (OAB) and underactive bladder (UAB) could be associated with metabolic syndrome, affective disorders, sex hormone deficiency, changes in urinary microbiota, functional gastrointestinal disorders, or autonomic nervous system dysfunction.
OBJECTIVES: The aim of this Think Tank was to provide a guide on how to investigate OAB and/or detrusor underactivity (DU) patients to better clarify the underlying pathophysiology and possibly personalize the treatment.
METHODS: A compendium of discussion based on the current evidence related to phenotyping patients with OAB

PURPOSE: To establish the correlation between flow rate curve shape and video-urodynamic findings in women with lower urinary tract symptoms (LUTS).
METHODS: A retrospective review of consecutive women with LUTS who performed a free flow study immediately before undergoing video-urodynamic investigations over a 28-month period. Flow rate curve shape and video-urodynamic parameters were analysed. Free flow curves were defined into five categories: bell-shaped, prolonged, fluctuating, intermittent or plateau. Women who voided less than 150 ml on the free flow study were excluded from the analys

It is not possible to diagnose the cause of voiding difficulty on the basis of a history alone. Bladder underactivity and bladder outlet obstruction cause similar voiding symptoms and also can be associated with storage symptoms. In this mini-review, we consider the factors associated with making a symptomatic diagnosis of either underactive bladder or bladder outlet obstruction. PATIENT SUMMARY: There is overlapping of symptoms for different urinary problems. To differentiate between underactive bladder and bladder outlet obstruction as the underlying cause of lower urinary tract symptoms, an

Overactive bladder syndrome (OABS) is a widely recognized syndrome with symptoms that can include urinary urgency, frequency, nocturia, and incontinence. Although there may be several causative factors for OABS, detrusor overactivity is the most common. In addition, urinary incontinence can also be due to a distinct but equally bothersome condition underactive bladder syndrome, or detrusor underactivity. The incomplete bladder emptying that characterizes detrusor underactivity often arises from impaired contractile function of the detrusor muscle. The variations in etiologies of the two syndro

Current standard clinical options for patients with detrusor underactivity (DUA) or underactive bladder─the inability to release urine naturally─include the use of medications, voiding techniques, and intermittent catheterization, for which the patient inserts a tube directly into the urethra to eliminate urine. Although those are life-saving techniques, there are still unfavorable side effects, including urinary tract infection (UTI), urethritis, irritation, and discomfort. Here, we report a wireless, fully implantable, and expandable electronic complex that enables elaborate management of ab

Distigmine bromide (distigmine) is a carbamate cholinesterase (ChE) inhibitor, which is mainly used for the treatment of myasthenia gravis. Distigmine is also used in Japan for the treatment for underactive bladder and glaucoma. The effectiveness of distigmine for underactive bladder treatment has been confirmed by many clinical reports, and this effect is thought to be caused by potentiating urinary bladder smooth muscle contraction due to inhibition of acetylcholine degradation during micturition. However, the pharmacological effects of distigmine on urinary bladder smooth muscle have not be

OBJECTIVES: To establish an artificial intelligence diagnostic system for lower urinary tract function in men with lower urinary tract symptoms using only uroflowmetry data and to evaluate its usefulness.
METHODS: Uroflowmetry data of 256 treatment-naive men with detrusor underactivity, bladder outlet obstruction, or detrusor underactivity + bladder outlet obstruction were used for artificial intelligence learning and validation using neural networks. An optimal artificial intelligence diagnostic model was established using 10-fold stratified cross-validation and data augmentation. Correlatio

Detrusor underactivity, a condition in which the bladder muscle does not contract strongly or long enough to empty the bladder completely or within the normal time frame, is a common cause of lower urinary tract symptoms in older individuals of both sexes. Although aging is a known risk factor for detrusor underactivity, its pathophysiological mechanisms are not fully understood. Therefore, establishing animal models that closely mimic the pathophysiology of detrusor underactivity in humans is necessary to elucidate these mechanisms. Metabolic syndrome is a cluster of several risk factors, inc

PURPOSE OF REVIEW: Detrusor underactivity is a highly prevalent type of voiding dysfunction in men and responsible for residual urine and decreased voiding efficiency. Patients with detrusor underactivity have an unfavorable outcome after prostatic surgery and do not have better long-term results than untreated detrusor underactivity patients. Therefore, differentiation between detrusor underactivity and bladder outlet obstruction (BOO) is crucial for the prediction of the outcome of prostatic surgery.
RECENT FINDINGS: Patients with detrusor underactivity report more frequently about decrease

OBJECTIVES: To investigate and compare the effects of tadalafil and silodosin on lower urinary tract symptoms and voiding functions in men with non-neurogenic detrusor underactivity.
METHODS: A total of 126 treatment-naive men with lower urinary tract symptoms diagnosed as non-neurogenic detrusor underactivity received tadalafil (5 mg/day) or silodosin (8 mg/day) for 12 months. After propensity score matching, parameter changes from before administration to 12 months since treatment initiation were assessed based on subjective symptoms and urodynamic findings, including bladder contractility

AIM: Nerve growth factor (NGF) and prostaglandins (PG) in the urinary bladder can be affected by pathology of bladder, and this change can be noted in the urine. This study was performed to investigate the changes in urinary NGF and PG in male patient with overactive bladder (OAB) symptoms.
METHODS: The study group included 75 male patients with OAB symptoms and 20 males without bladder symptoms as controls. Evaluation included history-taking, urinalysis, International Prostate Symptom Score (IPSS) and urodynamic study. The NGF, PGE2, PGF(2alpha) and PGI2 levels in voided urine were analyzed
