Stapedectomy and fatal meningitis. A human temporal bone study.

Temporal bone histopathology is reported on a case of acute suppurative otitis media, acute suppurative labyrinthitis, and meningitis with death of the patient 20 days after stapes surgery. Purulent exudate invaded the vestibule by the oval window through a displaced footplate. The main pathway of infection to the menings was extension along the branches of the vesticular nerve to the internal auditory meatus and to the subarachnoid space.

Three-dimensional fluid-attenuated inversion recovery magnetic resonance imaging findings in a patient with relapsing polychondritis.

OBJECTIVE: We report three-dimensional fluid-attenuated inversion recovery magnetic resonance imaging findings in a patient with relapsing polychondritis. METHOD: Case report. RESULT: A 76-year-old woman initially presented with bilateral auricular swelling together with dyspnoea. Three months later, she experienced left hearing loss and recurrent vertigo. A biopsy of the auricle was performed and relapsing polychondritis was diagnosed. The patient underwent three-dimensional fluid-attenuated inversion recovery magnetic resonance imaging 4 hours after intravenous injection of a standard dose

The significance for postoperative hearing of preserving the labyrinth in acoustic neurinoma surgery.

Among 186 patients with preoperative hearing, a total of 189 acoustic neurinomas were removed through a lateral suboccipital approach with anatomical preservation of the cochlear nerve. Functional hearing was preserved in 92 (49%) of these patients; despite anatomical preservation of the cochlear nerve, deafness was the result in 51% of the series. Many factors have been considered to cause hearing loss in patients whose cochlear nerve was intact after surgery; these include nerve retraction, nerve or cochlear ischemia, overheating and vibration damage to the nerve, and opening of the labyrint

Cholesteatoma extending into the internal auditory meatus.

We report our experiences in managing a patient with cholesteatoma complicated by meningitis, labyrinthitis and facial nerve palsy. The antero-inferior half of the tympanum was aerated but the postero-superior portion of the tympanic membrane was tightly adherent to the promontry mucosa. An attic perforation was present at the back of the malleolar head. High-resolution computed tomography also uncovered a fistula in the lateral semicircular canal. Surgical exploration of the middle ear cavity demonstrated that both the vestibule and cochlea were filled with cholesteatoma, and the cholesteatom

The vascular anatomy of the vestibular labyrinth in man.

1. A survey of the literature on the vascular anatomy of the vestibular labyrinth in Man is presented, and the methods of investigation used to visualize the vessels are evaluated. A generally correct picture of the subject is provided by the various studies published since Siebenmann's monograph. There are, however, some conflicting points as well as some lack of data with regard to the course and relationships of the large vessels and the capillary areas of the cristae and maculae. 2. This study presents a systematic description of the vascular anatomy of the vestibular labyrinth in Man, fro

Schwannoma in the vestibule and cochlea.

Schwannoma of the vestibule or the cochlea is an unusual lesion. In the past, most examples have been found at autopsy or as unsuspected findings at surgery for vertigo. The symptoms of isolated labyrinthine schwannoma may be indistinguishable from advanced Meniere's disease. Magnetic resonance imaging has led to pre-operative diagnosis in some cases. Two cases of schwannoma within the labyrinth from a series of 339 symptomatic acoustic tumours, are presented and the imaging findings are discussed.

Imaging of endolymphatic and perilymphatic fluid at 3T after intratympanic administration of gadolinium-diethylene-triamine pentaacetic acid.

By optimizing the inversion time of a 3D inversion-recovery turbo spin-echo sequence at 3T, we obtained separate images of endolymphatic and perilymphatic space 24 hours after intratympanic administration of gadolinium contrast material. In patients with Ménière disease, endolymphatic hydrops were detected not only in the cochlea but also in the vestibule. Fusion of the 2 types of images visualized the entire fluid space of the labyrinth and the spatial relationship of the 2 spaces.

Three-Dimensional High-Resolution Temporal Bone Histopathology Identifies Areas of Vascular Vulnerability in the Inner Ear.

OBJECTIVES: Hypothesized causes of vestibular neuritis/labyrinthitis include neuroinflammatory or vascular disorders, yet vascular disorders of the inner ear are poorly understood. Guided by known microvascular diseases of the retina, we developed 2 hypotheses: (1) there exist vascular vulnerabilities of artery channels in cases of hypothetical nerve swelling for the superior, inferior, and vestibulocochlear artery and (2) there are arteriovenous crossings that could compromise vascular flow in disease states. METHODS: Two fully mounted and stained temporal bones were used to render three-dim

Carbon dioxide laser occlusion of the guinea pig posterior semicircular canal.

Preservation of hearing is possible with selective ablation of the vestibular system and mechanical occlusion of the semicircular canals. Complete ablation of all three canals would improve exposure of the internal auditory canal fundus (e.g., for acoustic tumor exposure), but mechanical packing of the vestibule would disrupt normal sound transduction. This study was designed to assess the feasibility of preserving hearing with CO2 laser occlusion, without mechanical packing of the posterior semicircular canal membranous labyrinth. Twenty adult Hartley guinea pigs underwent occlusion of the ri

Hearing Preservation in Patients Who Undergo Labyrinthectomy and Translabyrinthine Procedures: A Case Report and Systematic Review.

IMPORTANCE: Preservation of endolymphatic fluids, maintenance of a fluid-filled vestibule, and preservation of the cochlear nerve and its vasculature are believed to be necessary to retaining hearing after an inner-ear operation. However, some studies have reported no hearing loss despite the violation of the vestibule, questioning the importance of maintaining a fluid-filled vestibule in preserving hearing. OBJECTIVE: To report on the preservation of hearing after a complete labyrinthectomy for Meniere disease and after disruption of the vestibule. EVIDENCE REVIEW: This systematic review ad

Pathogenesis of cytomegalovirus-associated labyrinthitis in a guinea pig model.

Cytomegalovirus infects fetuses through the placenta, resulting in various congenital disorders in newborns, including hearing loss. We developed a monoclonal antibody to guinea pig cytomegalovirus (GPCMV) that was available for immunohistochemistry, and investigated the expression of the GPCMV antigen in animal models of direct and congenital infections. Injection of GPCMV, directly to the inner ear, increased the sound pressure level and resulted in labyrinthitis with severe inflammation. Immunohistochemistry detected GPCMV-infected cells mainly in the scala tympani, scala vestibule and spin

[Cholesteatoma and labyrinthine fistula].

43 cases of large labyrinthic fistulas caused by a cholesteatoma of the middle ear were studied (33 External Semi-circular Canal fistulas, 4 External Semi-circular Canal amputations with opening of the vestibule, 3 of the promontory, 2 of the oval window, 1 of the Semi-circular Canal). The audiometric, preoperative, radiological data was analyzed. The radiological exam is often disappointing for the average-sized fistulas. Preoperative labyrinthization is moderate for External Semi-circular Canal fistulas and even for certain vestibular amputations; it is often incomplete in the other location

[Endolymphatic sac tumor with intralabyrinthine hemorrhage: a case report].

A 27-year-old female patient suffering endolymphatic sac tumor with intralabyrinthine hemorrhage was reported. The patient had hearing loss in the left ear with continuous tinnitus, and MRI showed the soft tissue shadow of endolymphatic sac. Considering that the tumor involved semicircular canal and vestibule, endolymphatic cyst tumor resection was performed by labyrinth route. After surgery, there was no cerebrospinal fluid leakage and facial nerve function was normal. More importantly, enhanced MRI of temporal bone showed no tumor recurrence 1 year after surgery.

Extracting the Cochlea from a Human Temporal Bone: A Cadaveric Protocol.

The extraction of the cochlea from a cadaver human temporal bone may be required for different studies of the inner ear. For histological evaluations, the inner ear must be extracted from the temporal bone to facilitate histologic processing; likewise, some micro-computed tomography devices are too small to accommodate the complete temporal bones; additionally, the image quality can be enhanced when the cochlea is isolated. The inner ear is located within the petrous part of the temporal bone. The inner ear can be divided into the osseous labyrinth or otic capsule and the membranous labyrinth

A practical work-up for vertigo.

Specific elements in the history and physical examination direct the work-up of the "dizzy" patient toward or away from a vestibular disorder. When a disorder of this type is suspected, the differential diagnosis includes benign positional vertigo, vestibular neuritis, vertebrobasilar insufficiency, migraine, and endolymphatic hydrops, which is the pathologic condition thought to underlie Meniere's disease. Anxiety is well recognized as a potential component of vestibular abnormalities or as the primary problem.
