OBJECTIVES: To describe the injuries misdiagnosed as a sprain of the wrist and to determine the approximate incidence of misdiagnosis in patients diagnosed as having a sprain of the wrist.
METHODS: All diagnostic errors in an accident and emergency (A&E) department were noted on a computerised database. Injuries initially misdiagnosed as wrist sprain are described and compared with the number of diagnoses of sprained wrist on the A&E department computer.
RESULTS: 57 injuries initially diagnosed as a sprained wrist had a different diagnosis (1.76% of all diagnoses of sprained wrists). This is

This study reports the causes, characteristics and treatment of injuries to the hand and wrist presented to five accident and emergency departments in a two-year survey of 13% of the Danish population. The rate of injury to the hand or wrist was 28.6% of all injuries, or 3545 per 100,000 inhabitants per year. Thirty-four percent of the accidents were domestic, 35% were leisure accidents, 26% were occupational and 5% were traffic accidents. Only 2% of the patients were admitted to hospital for further treatment or observation and 13% were referred to a hospital as out-patients. The most frequen

Hand and wrist injuries in sports are some of the most common injuries reported. This review discusses common overuse injuries of the wrist including tendon injuries such as de Quervain's syndrome, subluxation of the extensor carpi ulnaris, and the common dorsal carpal impingement syndrome. The main focus of this section is the discussion of traumatic injuries to the hand in the athlete. Included is a discussion and review of fractures of the phalanges and metacarpals, common proximal interphalangeal joint injuries, and thumb carpal metacarpal and metacarpophalangeal joint injuries. Emphasis i…

Pitching, catching, and base-running may predispose professional baseball players to injury-particularly in the hand and wrist. In this study, we outline the diagnosis, management, and rehabilitation of commonly encountered injuries among these high-level athletes. Wrist injuries discussed include hook of hamate fractures, extensor carpi ulnaris tendon disorders, scapholunate instability, and triangular fibrocartilage complex tears. Hand injuries discussed include thumb ulnar collateral ligament injury, flexor tendon pulley rupture, hypothenar hammer syndrome, and blisters.

Evaluation of the hand and wrist requires a thorough understanding of the anatomic alignment of various bones, tendons, and ligaments along with the physiologic function in order to make an accurate diagnosis that portends appropriate healing and function while minimizing future disability. This article provides the latest research-based evidence of important trends in the treatment and rehabilitation of hand and wrist injuries.

Wrist imaging is currently used for diagnosis of sport injuries as part of a global strategy of rapid recovery. Standard x-rays are the first step in this procedure. Although arthrography is still the reference for the diagnosis of intrinsic ligament and cartilaginous lesions, MRI can sometimes be sufficient. Ultrasonography is a dynamic process and is accurate in detecting tendon injuries. Wrist sport injuries are frequent and often asymptomatic. Here we review the usual aspects of bone, ligament, and tendon lesions encountered in each sport, while providing advice on the most appropriate ima

Primary care physicians not only have an important role in the diagnosis and initial treatment of wrist injuries, but also play a key role in the education of families about prevention. Children and adolescents are often competitive in sports throughout the year. Periods of rest can be important in prevention of overuse injuries in the very active, developing athlete. Protective gear such as wrist guards, used during activities such as inline skating and snowboarding, has been shown to prevent acute injuries that often require surgery or lead to prolonged disability [84,85].A primary care phys…

The aim of the study was to assess acute-phase multidetector CT (MDCT) findings in wrist injuries. We retrieved all emergency room MDCT requests processed in the period from August 2000 to May 2003. All patients with a wrist injury who underwent MDCT initially were included. Imaging studies were evaluated in relation to injury mechanism, fracture location, and fracture type. A total of 6422 MDCT examinations were performed during this 34-month period, and 38 patients (24 male, 14 female, age range 21-73 years, mean age 40 years) met the inclusion criteria. MDCT revealed 56 fractures and 7 disl…

Because sports-related injuries to the hand and wrist are relatively frequent and lead to significant disability if misdiagnosed or improperly treated, it is important to identify the specific nature of such an injury to reach an accurate diagnosis and administer appropriate treatment. The author advocates an initial three-step process that may help the clinician to effectively diagnose and treat injuries: listen to the description of injury mechanisms; observe the site of injury; palpate the area for tenderness or instability.

Golf injuries of the wrist are rare, but when they occur they can be devastating for the avid golfer, competitive amateur golfer, or the professional golfer, as the hand and wrist are so integral to the game. The majority of golf injuries are overuse injuries of the wrist flexor or extensor tendons. The left wrist (in the right-handed golfer) is the most common location. Analysis of the golf swing finds excessive motion of the left wrist (in the right-handed golfer), along with a catapulting function, accounting for vulnerability of the left wrist to injury. Hyperextension and radial deviation

Although nerve injury rarely accompanies wrist trauma or disease, the long-term functional result frequently depends more on resolution of the nerve deficit and symptoms than on resolution of the basic orthopedic pathology. The origins of nerve injury associated with orthopedic problems are reviewed, and a rational approach to their management is described.

A prospective study of 10 patients with acute complex carpal dislocations was undertaken. Preoperative wrist arthrograms were done in these patients. We then observed the ligamentous tear through both volar and dorsal approaches at the time of open reduction. Perilunate dislocation was always an integral part of the pathology in all patients, except in one patient with radiocarpal dislocation. Communication between the radiocarpal and midcarpal joint was found in all patients, except in one patient with radiocarpal fracture-dislocation. Volar extrusion of the contrast material occurred in all

Any disabling injury of the elbow or wrist should be studied roentgenographically for evidence of fracture which may not be otherwise evident but which may cause permanent disability unless the joint is immobilized for healing."Tennis elbow" may be treated with physical therapy and analgesic injection but may require splinting or tendon stripping. Elbow sprain can occur in the growing epiphysis but is rare in adults. A jarring fall on the hand may cause fracture or dislocation at the elbow. Full extension of the joint should be restored gradually by active exercise rather than passive or forci

About one fifth of all sports accidents involve the hand. Many injuries can be diagnosed and treated easily. However, some of them such as fractures of the scaphoid or the hamate of the hamulus are difficult to detect with conventional radiographs and are often missed without a CT scan. Ligament injuries such as the skier's thumb must be recognized and treated properly, often surgically. There are also sport-specific injuries, such as the closed rupture of a flexor tendon pulley in climbers, which otherwise occur rarely and are little known. These topics, pitfalls and tricks will be discussed.

High-energy injuries to the wrist gather complex fractures of the distal radius, radiocarpal dislocations, perilunate dislocations, and other intracarpal dislocations. Depending on the energy of the injury and the position of the wrist at the time of impact, the patient, often a young male with a high functional demand, presents one of these injuries associating fracture(s) and ligament injury. The trauma is often bilateral, with proximal lesions (elbow) very often associated with contusion or compression of the median nerve. Diagnosis is confirmed by wrist X-rays, which are sufficient to dete
