Implications of object-relations theory for the behavioral treatment of agoraphobia.

In this paper I review recent developments in analytic object-relations theory and show how such a frame of reference can formulate and contribute useful clinical insights for the implementation of in-vivo exposure treatment. Since agoraphobic patients tend to relate to significant others, including therapists, in either a safety-producing or potentially anxiety-provoking fashion, knowledge of developmental object-relations theory can aid the behavior therapist in avoiding the enactment of one or the other side of the split. In-vivo exposure, is structured to guard against such enactments. How

The relationship of plasma imipramine and N-desmethylimipramine to improvement in agoraphobia.

Plasma tricyclic concentrations were assessed in 15 agoraphobic patients receiving combined imipramine and behavioral treatments. Imipramine but not N-desmethylimipramine plasma levels were found to significantly correlate with improvement. The results suggest that imipramine's effect in agoraphobia might be mediated predominantly through the serotonergic action of the parent drug. There was also suggestive evidence for differential antipanic and antiphobic-antidepressant effects. Implications for future studies of the mechanism of action of drug effects in agoraphobia are discussed.

The relative efficacy of imaginal and in vivo desensitization in the treatment of agoraphobia.

Claims have often been made that in vivo procedures are generally superior to imaginal approaches in the clinical management of anxiety and … tions that in vivo exposure is necessarily superior to imaginal desensitization in the treatment of agoraphobia. Encouraging follow-up results provided some confirmation of previous findings indicating the effectiveness of self-management procedures in the long-term control of agoraphobia.

Tricyclic antidepressants and monoamine oxidase inhibitors in the treatment of agoraphobia.

The efficacy of tricyclic antidepressants and monoamine oxidase inhibitors for the treatment of agoraphobia and other panic disorders is reviewed with an emphasis on controlled prospective studies. After methodological biases are taken into account, there is strong evidence that antidepressants suppress panic attacks. This effect is not dependent on the presence of concomitant depressive symptoms. The clinical issues of dosage, delay in response, and relapse with discontinuation of treatment are also reviewed.

Cognitive-behavioral group treatment for panic disorder with agoraphobia.

Cognitive-behavioral therapy (CBT) is well documented in the treatment of panic disorder with or without agoraphobia; however, little is known about the efficacy of group treatment. The purpose of this open study is to investigate the benefits of a combination of the major cognitive and behavioral techniques used in the several specific versions of CBT thus far developed, in a psychotherapeutic group approach for panic and agoraphobia. Seventy-six outpatients meeting the Diagnostic and Statistical Manual of Mental Disorders, third edition, revised (DSM-III-R; American Psychiatric Association, …

Cognitive-behavioral group psychotherapy for agoraphobia and panic disorder.

Agoraphobia is a complex disorder characterized by substantial personal, social, and occupational disability. This paper presents a cognitive-behavioral model of group psychotherapy for agoraphobia treatment wherein group members are taught active coping skills to reduce anxiety and behavioral avoidance. Therapy occurs in the context of a supportive group, which aids in reducing the social isolation and alienation attendant on this severe anxiety disorder. The model of treatment is amenable to both time-limited and open-ended group therapy formats.

Agoraphobia without panic: clinical reappraisal of an epidemiologic finding.

OBJECTIVE: In the United States, the consensus among clinicians and researchers, reflected in DSM-III-R, is that agoraphobia is a conditioned response to panic attacks and almost never occurs without panic attacks. The predominant view in the United Kingdom is that agoraphobia frequently occurs in the absence of panic. While clinicians report that they rarely see patients with agoraphobia who have no history of panic disorder, community studies report that agoraphobia without panic disorder is common. For example, the Epidemiologic Catchment Area (ECA) study found that 68% of 961 persons with …

The relationship between panic disorder/agoraphobia and personality disorders.

This selective review of the relationship between panic disorder/agoraphobia and DSM-III personality disorders points to a preponderance of dependent, avoidant, and histrionic features and reveals a certain degree of covariation between severity of Axis I disorder and personality functioning. However, the link between panic/agoraphobia and Axis II disorders does not appear to be specific because (1) general features such as neuroticism, stress, dysphoric mood, and interpersonal sensitivity, rather than duration and severity of panic attacks and phobias, emerge as unique predictors or determina…

Plasma biogenic amine levels in agoraphobia with panic attacks.

Eight patients with agoraphobia and panic attacks as defined by DSMIII and 11 controls were included in this study which measured the circulating biogenic amine levels. The method of assay used a high performance liquid chromatograph with electrochemical detection. Measurement of dopamine, noradrenaline and adrenaline showed no differences between the groups. Levels of serotonin (5-HT) were significantly lower in the agoraphobic group. The authors discuss the implication of this finding.
