This study tests the hypothesis that glutamate receptors are altered in the brains of alcoholics as a result of chronic alcohol neurotoxicity. Excessive release of the excitatory neurotransmitter glutamate may damage postsynaptic neurons by increasing calcium flux through N-methyl-D-aspartate (NMDA) receptor-gated ion channels. Alcohol has opposite effects on the NMDA receptor, depending on the duration of exposure. Acute exposure to alcohol inhibits ion flow through NMDA receptors, whereas chronic exposure upregulates the number of these receptors and thereby increases ion flow. Acute withdra…

Standardized investigations on resting heart rate variability (HRV) should provide more information on acamprosate's human pharmacodynamic properties because acamprosate interacts with several neurotransmitter systems which are also involved in maintaining autonomic neurocardiac balance. We performed HRV measurements prospectively in 69 healthy controls and 19 chronic alcoholics to prove the hypotheses that: (1) compared to healthy controls, chronic alcoholics show disturbances in neurocardiac vagal function; and (2) in alcoholics, acamprosate treatment (6-8 days) should further decrease paras…

The possible effects of acute and long-term treatment of the antidipsotropic agent acamprosate on psychomotor performance and driving ability were studied in a prospective open clinical trial involving 5 alcoholic patients without any clinical evidence for cognitive impairement. Acamprosate 1995 mg/day was given for 6 months for relapse prevention with all patients being abstinent throughout the study. No side effects were reported during treatment. Psychomotor performance and driving ability were assessed using the ART 90, a standardized and computerized neuropsychological test battery. A num

OBJECTIVES: Relationships of nutritional status with ethanol consumption and diet were studied in 33 chronic alcoholics with no clinical or laboratory evidence of liver disease.
METHODS: Nutritional assessment included subjective global assessment, weight-height index, body mass index, and serum albumin measurements. Dietary intake included estimates of daily intake of substrates, folic acid, vitamins B1, B5, B6, and B12. Circulating concentrations of folate, pyridoxal-phosphate and vitamin B12 were evaluated as well.
RESULTS: Only 18.1% of patients were considered malnourished, with body ma…

OBJECTIVES: To establish better operational criteria for the diagnosis of Wernicke's encephalopathy. Current criteria for diagnosing Wernicke's encephalopathy require the presence of three clinical signs (oculomotor abnormalities, cerebellar dysfunction, and an altered mental state), although it has often been reported that most patients do not fulfil all these criteria.
METHODS: The clinical histories of 28 alcoholics with neurological and neuropsychological assessments and definitive neuropathological diagnoses were examined to determine clinical signs for use in a screening schedule. Opera…

Examination of the change strategies associated with successful long-term sobriety remains an understudied area in addiction research. The following study recruited individuals in various stages of sobriety (range: 1 month to 27 years continuous abstinence). Subjects (N = 191) were surveyed on demographic, problem history, degree of self-help utilization, current process of change use and self-efficacy measures. Subjects were differentiated based on varying experience with AA, including exposure, frequency of meeting attendance and degree of affiliation. Analyses included comparisons on demogr

The application of the novel antialcoholic drug Medichronal mainly results in normalization of dyscatecholaminemias, by lowering or elevating noradrenaline or adrenaline levels. This effect is similar to that produced by unitiol that is conventionally used in nacrological care. The changes in catecholamine levels occur in various dyscatecholaminemias characterized by a high or low excretion of biogenic monoamines. Before and after therapy, in some patients, the concentrations of catecholamines do not differ from those in healthy persons.

Sixty subjects (20 alcoholic inpatients, 20 outpatient alcoholics, and 20 surgical controls from two Veterans Administration hospitals) were tested on field independence-dependence (measured by the Rod-and-Frame Test), realistic/unrealistic self-expectations (on the Level of Aspiration Board), and degree of alcohol dependence (on the Alcohol Dependence Scale). Response bias and current intelligence were controlled. As predicted, alcoholic inpatients were more alcohol dependent than outpatients, both groups of alcoholics were found to be more field dependent and more unrealistic in their self-e…

Alcoholism has been associated with a high prevalence of anxiety and phobic disorders. The authors ascertained the current prevalence of panic disorder and the lifetime prevalence of infrequent panic attacks in 154 male alcoholics in an inpatient alcohol treatment program. Thirteen percent (N = 20) gave a lifetime history of panic attacks and 45% (N = 9) of these had current panic disorder. Panic attacks preceded or coincided with the onset of problem drinking in 50% (N = 9) of the 18 patients with both diagnoses. Only two patients with histories of panic had been previously diagnosed, and non…

Alcoholic subjects (453) were randomly assigned to disulfiram or placebo therapy and followed for up to 12 months for drinking. Drinking status was determined from interviews of the subject and a household contact each 2 months and from the analysis of eight blood samples or 39 urines submitted at intervals during the year. Liver status was monitored each 2 months by obtaining serum alkaline phosphatase, bilirubin, and AST. Sensitive criteria were arbitrarily selected to identify about 1/5 of the patients with episodic elevations of liver tests. There was no relationship of liver test elevatio

It has been shown that a specific liver lesion--that is, pericentral sclerosis associated with pericellular fibrosis--is the precursor of alcoholic liver sclerosis. It is, however, difficult to diagnose this hepatic lesion in chronic alcoholics, using only clinical data without liver biopsy. To investigate the possibility of a clinical test reflecting the presence of this hepatic lesion, ethanol (0.75 g/kg body weight) was given orally to chronic alcoholics, and serum glycoprotein levels (prealbumin, alpha HS glycoprotein, haptoglobin, alpha 2-macroglobulin) were measured before and six hours …

1. A group of 129 patients with chronic alcoholism were assessed for their nutritional status with respect to certain minerals and vitamins, and compared with control subjects. 2. In all subjects the plasma values were normal for calcium, magnesium and zinc. 3. As in other studies a seasonal variation was found in the plasma levels of 25-hydroxyvitamin D in the control subjects and the alcoholic subjects; in all seasons lower levels were found in the alcoholics than in the controls, but none of the alcoholic patients had results in the range found in osteomalacia. 4. The alcoholic subjects had…

The estimate of drug abuse by alcoholics and problem drinkers in the literature varies from a low of 1% to a high of 90%. These studies, coupled with data from the National Drug/Alcohol Collaborative Project (NDACP), led us to estimate the figure to lie somewhere between 60 and 80%, being somewhat higher for individuals under 40 than over 40. Some explanations for the increased occurrence of this form of multiple substance abuse are provided. Problems associated with investigating this phenomenon are discussed and illustrated. Physiological and medical complications associated with drug and al…

Abstinence behaviour after disulfiram implantation has been investigated in 21 chronic alcoholics. The blood levels of disulfiram and its metabolites, carbon disulfide (CS2) and reduced disulfiram (diethyldithiocarbamate) were determined and the blood levels of patients with implants were compared with those of patients receiving disulfiram orally. The blood levels in the implanted patients were significantly lower than those of the group taking disulfiram orally. No metabolites were detectable after 3 months, despite the sensitive method employed. Nevertheless, 14 of the 21 chronic alcoholics…

Alcohol is the main risk factor for death and disability. The treatment of alcohol dependence (AD) is a complex activity as the variables are numerous; however, those which must necessarily be taken into account are the type of AD, the internal comorbidities and the presence of any psychiatric comorbidity. Liver problems are one of the most common causes of alcohol-related liver damage. 45% of deaths from cirrhosis are alcohol-related. Thus, the treatment of AD must often deal with a more or less severe liver disease, which influences the choice of anticraving drug. As chronic liver disease is
