Effect of Sequential and Simultaneous Patching Regimens in Unilateral Amblyopia.

PURPOSE: Many clinicians treat unilateral amblyopia with glasses alone and initiate patching when needed; others start glasses and patching simultaneously. In this study, we reviewed the outcomes of the two approaches at our institution. DESIGN: Retrospective nonrandomized clinical trial. METHODS: Setting: Institutional practice. PATIENT POPULATION: All patients diagnosed with amblyopia at Boston Children's Hospital between 2010 and 2014. INCLUSION CRITERIA: Unilateral amblyopia (visual acuity (VA) 20/40 to 20/200 with interocular difference ≥3 lines,) age 3 to 12 years, with a 6-month fol…

Amblyopia Affects the ON Visual Pathway More than the OFF.

Visual information reaches the cerebral cortex through parallel ON and OFF pathways that signal the presence of light and dark stimuli in vi … arks in visual perception. Here we provide support for this hypothesis in humans with anisometropic amblyopia who suffered sustained optical blur early after birth in one of the eyes. In addition, we show that the dark dominance in visual perception also increases in strabismic amblyopes that have their vision to high spatial frequencies reduced by mechanis …

Higher Order Aberration and Astigmatism in Children with Hyperopic Amblyopia.

PURPOSE: To investigate the changes in corneal higher-order aberration (HOA) during amblyopia treatment and the correlation between HOA and astigmatism in hyperopic amblyopia children. METHODS: In this retrospective study, a total of 72 eyes from 72 patients ranging in age from 38 to 161 months were included. Patients were divided into two groups based on the degree of astigmatism. Corneal HOA was measured using a KR-1W aberrometer at the initial visit and at 3-, 6-, and 12-month follow-ups. Correlation analysis was performed to assess the association between HOA and astigmatism. RESULTS: A …

The effect of occlusion therapy on motion perception deficits in amblyopia.

There is growing evidence for deficits in motion perception in amblyopia, but these are rarely assessed clinically. In this prospective study we examined the effect of occlusion therapy on motion-defined form perception and multiple-object tracking. Participants included children (3-10years old) with unilateral anisometropic and/or strabismic amblyopia who were currently undergoing occlusion therapy and age-matched control children with normal vision. At the start of the study, deficits in motion-defined form perception were present in at least one eye in 69% of the children with amblyopia. Th…

Laser in situ keratomileusis in adult patients with anisometropic amblyopia.

AIM: To evaluate the increase in corrected distance visual acuity (CDVA) after laser in situ keratomileusis (LASIK) in adults with anisometropic amblyopia. METHODS: The medical records of consecutive patients diagnosed with anisometropic amblyopia at the time of refractive evaluation who underwent LASIK were retrospectively reviewed. Patients with at least a two-line difference of visual acuity (VA) between the eyes with a spherical refractive error difference of at least 3.00 diopters (D) or an astigmatic difference of at least 2.00D were included. Patients with any other possible reason for…

Eye-hand coordination skills in children with and without amblyopia.

PURPOSE: To investigate whether binocular information provides benefits for programming and guidance of reach-to-grasp movements in normal children and whether these eye-hand coordination skills are impaired in children with amblyopia and abnormal binocularity. METHODS: Reach-to-grasp performance of the preferred hand in binocular versus monocular (dominant or nondominant eye occluded) conditions to different objects (two sizes, three locations, and two to three repetitions) was quantified by using a 3D motion-capture system. The participants were 36 children (age, 5-11 years) and 11 adults w…

Prevalence of amblyopia and strabismus in young singaporean chinese children.

PURPOSE. To determine the prevalence of amblyopia and strabismus in young Singaporean Chinese children. METHODS. Enrolled in the study were 3009 Singaporean children, aged 6 to 72 months. All underwent complete eye examinations and cycloplegic refraction. Visual acuity (VA) was measured with a logMAR chart when possible and the Sheridan-Gardner test when not. Strabismus was defined as any manifest tropia. Unilateral amblyopia was defined as a 2-line difference between eyes with VA < 20/30 in the worse eye and with coexisting anisometropia (> or =1.00 D for hyperopia, > or =3.00 D for myopia, a…

Active treatments for amblyopia: a review of the methods and evidence base.

Treatment for amblyopia commonly involves passive methods such as occlusion of the non-amblyopic eye. An evidence base for these methods is provided by animal models of visual deprivation and plasticity in early life and randomised controlled studies in humans with amblyopia. Other treatments of amblyopia, intended to be used instead of or in conjunction with passive methods, are known as 'active' because they require some activity on the part of the patient. Active methods are intended to enhance treatment of amblyopia in a number of ways, including increased compliance and attention during t…

Effectiveness of screening preschool children for amblyopia: a systematic review.

BACKGROUND: Amblyopia and amblyogenic factors like strabismus and refractive errors are the most common vision disorders in children. Although different studies suggest that preschool vision screening is associated with a reduced prevalence rate of amblyopia, the value of these programmes is the subject of a continuing scientific and health policy discussion. Therefore, this systematic review focuses on the question of whether screening for amblyopia in children up to the age of six years leads to better vision outcomes. METHODS: Ten bibliographic databases were searched for randomised contro…

Is hemiplegic cerebral palsy equivalent to amblyopia of the corticospinal system?

OBJECTIVE: Subjects with severe hemiplegic cerebral palsy have increased ipsilateral corticospinal projections from their noninfarcted cortex. We investigated whether their severe impairment might, in part, be caused by activity-dependent, competitive displacement of surviving contralateral corticospinal projections from the affected cortex by more active ipsilateral corticospinal projections from the nonaffected cortex, thereby compounding the impairment. METHODS: Transcranial magnetic stimulation (TMS) characterized corticospinal tract development from each hemisphere over the first 2 years

The prevalence of strabismus and amblyopia in Japanese elementary school children.

BACKGROUND: The purpose of this study is to elucidate the prevalence of strabismus and amblyopia in a large population of Japanese elementary school children, from Grade 1 to Grade 6, ages ranging from 6 to 12 years. The School Health Law requires that all pupils in Grade 1 to Grade 6 be examined for vision and eye problems. Visual acuity testing is done by school teachers and eye disease screening by school ophthalmologists. Pupils with suspected ocular diseases are further examined by extramural ophthalmologists and the results reported back to the schools. The schools then summarize and sen…

"Phase capture" in amblyopia: the influence function for sampled shape.

This study was concerned with what stimulus information humans with amblyopia use to judge the shape of simple objects. We used a string of four Gabor patches to define a contour. A fifth, center patch served as the test pattern. The observers' task was to judge the location of the test pattern relative to the contour. The contour was either a straight line, or an arc with positive or negative curvature. We asked whether phase shifts in the inner or outer pairs of patches distributed along the contour influence the perceived shape. That is, we measured the phase shift influence function. Our r…

The association between anisometropia, amblyopia, and binocularity in the absence of strabismus.

PURPOSE: First, to determine if thresholds exist for the development of amblyopia and subnormal binocularity with various types of anisometropia and to confirm or refute existing guidelines for its treatment or observation. Second, to delineate any association between the degree or type of anisometropia and the depth of amblyopia and severity of binocular sensory abnormalities. METHODS: Four hundred eleven (411) patients with various levels of anisometropia, no previous therapy, and no other ocular pathology were evaluated. The effect of anisometropia (both corrected and uncorrected) on monoc…

The visual-evoked potential in tobacco-alcohol and nutritional amblyopia.

Twelve subjects, nine with tobacco-alcohol amblyopia and three with nutritional amblyopia, were studied with pattern and flash visual-evoked potentials. We found abnormal steady-state responses in all 12 patients, including those with minor visual abnormalities, when the contrast of the stimulus was decreased. The latency of the first major positive wave (P100) of the flash- or pattern-evoked potential was not delayed in 11 of the 12 patients. The normal P100 latencies differed from the prolonged latencies found in other patients with loss of central nervous system myelin caused by vitamin B12…

An unexpected role for normal accommodative vergence in strabismus and amblyopia.

Accommodative vergence was measured using the Müller experimental paradigm in which target movement along the line of sight of the viewing eye results in large movement of the fellow (covered) eye. Our paper demonstrates an unexpected predominance of accommodative vergence over disparity vergence in patients with constant strabismus amblyopia, intermittent strabismus, and amblyopia without strabismus. Comparison of accommodative vergence responses in these patients with responses from normal subjects using symmetrical and asymmetrical targets under binocular and monocular conditions establishe…

Congenital third nerve palsy with amblyopia of the contralateral eye.

A child with congenital third nerve palsy using the palsied eye for fixation is presented. Apparently, amblyopia developed in the secondarily deviating eye. This is the first case report of which we are aware with such a disturbance, ie, primary ocular motor palsy in one eye and sensory amblyopia in the other.
