Anaphylaxis after contact with a jellyfish.

We report a case of an anaphylactic reaction to a jellyfish sting. The episode was manifested by hypotension and bronchospasm. The patient's basophils released histamine in response to nematocyst venom from the Chesapeake Bay sea nettle; this sensitivity could be passively transferred by a heat labile serum factor. This appears to be the first case report of such a reaction.

Disodium cromoglycate in anaphylaxis and pollinois.

Because disodium cromoglycate is being used in treating the symptoms of allergic patients and its mode of action remains uncertain, morphological experiments were conducted in an attempt to further elucidate this compound's mode of action. Rabbits with an ear chamber underwent anaphylaxis, lethal or sublethal, with microscopic observations of the microcirculation during anaphylaxis. At death or a chosen interval tissue of the ear chamber was secured for histological examination. The mast cells and the state of their granules were counted for each section. In no instance did disodium cromoglyca…

Acute systemic anaphylaxis in the horse.

1. Histamine in small doses caused systemic depressor responses in horses, whereas greater doses caused biphasic effects. All doses of 5-hydroxytrypt-amine (5-HT) were pressor and all doses of bradykinin depressor. All three active substances raised pulmonary artery pressure and lowered central venous pressure. 5-HT reduced ventilation volume. Histamine caused brief apnoea followed by hyperpnoea only.2. Acute anaphylaxis in the horse was accompanied by a severe systemic arterial depressor response, a pressor response in the pulmonary artery and vena cava, and alternating phases of apnoea and d…

Phenotype and risk factors of venom-induced anaphylaxis: A case-control study of the European Anaphylaxis Registry.

BACKGROUND: Venom-induced anaphylaxis (VIA) is a common, potentially life-threatening hypersensitivity reaction associated with (1) a specific symptom profile, 2) specific cofactors, and 3) specific management. Identifying the differences in phenotypes of anaphylaxis is crucial for future management guidelines and development of a personalized medicine approach. OBJECTIVE: This study aimed to evaluate the phenotype and risk factors of VIA. METHODS: Using data from the European Anaphylaxis Registry (12,874 cases), we identified 3,612 patients with VIA and analyzed their cases in comparison wi…

Factors increasing the risk for a severe reaction in anaphylaxis: An analysis of data from The European Anaphylaxis Registry.

BACKGROUND: Preventive measures to decrease the frequency and intensity of anaphylactic events are essential to provide optimal care for allergic patients. Aggravating factors may trigger or increase the severity of anaphylaxis and therefore need to be recognized and avoided. OBJECTIVE: To identify and prioritize factors associated with an increased risk of developing severe anaphylaxis. METHODS: Data from the Anaphylaxis Registry (122 centers in 11 European countries) were used in logistic regression models considering existing severity grading systems, elicitors, and symptoms to identify t…

Patterns of anaphylaxis after diagnostic workup: A follow-up study of 226 patients with suspected anaphylaxis.

BACKGROUND: Most published studies on anaphylaxis are retrospective or register based. Data on subsequent diagnostic workup are sparse. We aimed to characterize patients seen with suspected anaphylaxis at the emergency care setting (ECS), after subsequent diagnostic workup at our Allergy Center (AC). METHODS: Prospective study including patients from the ECS, Odense University Hospital, during May 2013-April 2014. Possible anaphylaxis cases were daily identified based on a broad search profile including history and symptoms in patient records, diagnostic codes and pharmacological treatments. …

Food-dependent exercise-induced anaphylaxis to lentil and anaphylaxis to chickpea in a 17-year-old boy.

We report the case of a 17-year-old boy who experienced 4 episodes of exercise-induced anaphylactic reaction after ingestion of lentil and 2 episodes of anaphylaxis following ingestion of chickpea. His medical history revealed that he had allergic rhinitis with positive results after skin prick tests (SPT) with mites. His SPTs and specific immunoglobulin E antibody testing with lentil and chickpea were positive. Oral challenge with chickpea was not performed due to patient refusal. Treadmill exercise challenge tests in the fasting state and 1 hour after a meal not containing lentil were negati…

[How I treat anaphylaxis in children].

Anaphylaxis is a rapid-onset, life-threatening systemic hypersensitivity reaction. Insufficient awareness of this condition in China and delayed or inappropriate use of epinephrine-the core treatment-are major causes of severe outcomes. Based on two clinical cases and the latest domestic and international guidelines, this article systematically describes early recognition and diagnosis of anaphylaxis in children, epinephrine-centered emergency management strategies, and the treatment protocol for refractory anaphylaxis. For classic anaphylaxis, early intramuscular injection of epinephrine into…

Urine mast cell mediators and anaphylaxis.

PURPOSE OF REVIEW: Anaphylaxis is a severe type 1 hypersensitivity reaction which is the result of mast cell degranulation causing multiorgan dysfunction. Currently, the most accepted biomarker measurement for mast cell mediated anaphylaxis is serum tryptase measurement. Serum tryptase measurement has limitations including an ideal window of collection during anaphylaxis and must be obtained by venipuncture collection. Urinary mast cell mediators are emerging mast cell biomarkers that can be used for evaluation of mast cell mediated anaphylaxis and mast cell activation. This review will highli…

Rate of Patch Testing Induced Anaphylaxis.

Background: Although allergic contact dermatitis is a type IV hypersensitivity reaction, type I hypersensitivity reactions, such as anaphylaxis, have been reported during patch testing. Objective: The aim of this study was to identify reported cases of anaphylaxis from patch testing and estimate its rate. Methods: A literature review was conducted on PubMed to identify previously reported cases of anaphylaxis after patch testing and suspected allergens. In addition, a survey was distributed to expert patch testing dermatologists to determine the rate of anaphylaxis after patch testing. Results…

Rare presentation of anaphylaxis: pancake syndrome.

A 43-year-old woman presented with oral discomfort, sneezing, urticaria, eyelid angioedema, abdominal pain, diarrhoea, dyspnoea and wheeze soon after eating a Japanese flour pancake (okonomiyaki, containing wheat, egg, yam, pork, prawn and squid). Subsequent analysis of the flour used in the pancake revealed the presence of Dermatophagoides farinae (4500 mites/g). The patient tested positive for specific IgE to D. farinae (15.2 kU/L) and D. pteronyssinus (14.0 kU/L) with negative responses to other ingredients in the pancake. Oral ingestion of dust mite in poorly stored foods can cause anaphyl

Alpha-Gal-containing biologics and anaphylaxis.

Cetuximab, the IgG1 subclass chimeric mouse-human monoclonal antibody biologic that targets the epidermal growth factor receptor (EGFR), is used worldwide for the treatment of EGFR-positive unresectable progressive/recurrent colorectal cancer and head and neck cancer. Research has shown that the principal cause of cetuximab-induced anaphylaxis is anti-oligosaccharide IgE antibodies specific for galactose-α-1,3-galactose (α-Gal) oligosaccharide present on the mouse-derived Fab portion of the cetuximab heavy chain. Furthermore, it has been revealed that patients who are allergic to cetuximab als…

Pediatric anaphylaxis and hyper IgE syndrome.

Patients with autosomal-dominant (AD) hyper immunoglobulin E (IgE) syndrome (HIES) or Job syndrome develop frequent dermatologic and pulmonary infections. As patients have an extreme elevation of IgE levels, this database analysis study sought to study the association between AD HIES, Job syndrome, and anaphylaxis. HIES is a heterogeneous group of immune disorders characterized by extremely elevated levels of serum IgE. Although the molecular defects and clinical phenotypes found in association with Job syndrome are well characterized, the association with severe allergic reactions and anaphyl…
