Complications of continuous spinal anesthesia.

The increased use of continuous spinal anesthesia (CSA) in recent years has resulted in research efforts directed at reducing the complications associated with the technique. Complications of CSA are categorized as general, the same as those associated with single-shot spinal anesthesia, and specific, those associated solely with CSA. General complications include infection, backache, hematoma, and neurological sequelae. Specific complications are all related to catheter use. The risk of developing the neurological complication of postdural puncture headache (PDPH) led to the use of microcathe

[Silexan and narcosis : case report and possibilities of preoperative and perioperative management].

The increased use of phytotherapeutic drugs means that anesthetists are more often confronted with these drugs. In this case report possible problems which can occur are demonstrated exemplified by silexan. Silexan is a phytotherapeutic anxiolytic which is used in anxiety disorders. Because of its potential mechanism of action via the neurotransmitter gamma-aminobutyric acid (GABA) receptors interactions with narcotic drugs are possible. The case of an 18-year-old girl who underwent an operation under general anesthesia while taking silexan as long-term medication is presented. The desired dep…

Anesthetic management of a patient with laryngeal amyloidosis.

A 73-year-old woman who suffered from progressive hoarseness for 6 years and dysphagia without pain for 1 year presented with a soft tissue … al amyloidosis is fragile and hemorrhagic. Therefore, massive bleeding may occur during intubation. Anesthetists should take care in intubating the tracheas of these patients and be aware of other systemic diseases in laryngeal amyloidosis.

Cholecystectomy in a patient with Broncho pleural fistula - An Anesthesiologist's tight rope walk.

Patients presenting for surgery postlobectomy with Broncho pleural fistula are prone for perioperative respiratory complications and pose a significant challenge to anesthetist. Published data to guide perioperative management of such cases especially for abdominal surgeries are scarce. We describe a 51-year-old gentleman status postlobectomy with Broncho pleural fistula posted for cholecystectomy in view of symptomatic gallstones. Laparoscopic surgery was later converted to open surgery in view of surgical difficulties. Perioperative period was uneventful and proper modes of analgesia helped

Anesthetic Management in a Case of MURCS Syndrome.

MURCS syndrome is a more severe form of Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome, an acronym meaning aplasia/hypoplasia of Müllerian ducts (MU), congenital renal agenesis/ectopia (R), and cervical somite dysplasia (CS). A common presentation is primary amenorrhea in adolescent females. An anesthetist must consider the benefits and limitations of both regional and general anesthesia for these patients based on site of surgery and severity of malformations. We report successful anesthetic management of a 21-year-old female with MURCS syndrome scheduled for a creation of neovagina under spi

Anesthetic Management of a Patient With Mast Cell Activation Syndrome: A Case Study.

Mast cell activation syndrome (MCAS) is a relatively new diagnosis for a constellation of symptoms with sometimes devastating results for patients. A 40-year old woman with MCAS underwent arthroscopic repair of her right shoulder, with successful anesthetic management. This case report discusses the basic immunologic physiology surrounding this syndrome, myriad medications often used by this patient population, and the anesthetic management of this patient. With additional knowledge of this disorder, exposure to its clinical presentation in the perioperative setting, and anesthetic considerati

Ultrasound-Guided Taylor's Approach in Ankylosing Spondylitis.

Ankylosing spondylitis (AS) is a chronic inflammatory disease of the spine which leads to ossification and formation of a classical bamboo spine. This poses a challenge to the anesthetist both in terms of administering general and regional anesthesia due to the limited mobility of the spine. With the advent of ultrasound as an aid in regional anesthesia, it has been relatively easy to perform a central neuraxial blockade in such patients though the skill requires some degree of expertise. Here, we have described the use of ultrasound as a guide for administering regional anesthesia to a patien

[Internal hemorrhage in abdominal dermolipectomy. 2 reported cases, one of which needing emergency surgical repair of aortic fissure].

The authors report two very exceptional and similar cases. In the first case, during abdominal lipectomy, the surgical team was confronted suddenly with an internal hemorrhage requiring prompt resuscitation by the anesthetist. Laparatomy revealed an aortic fissure, one centimeter above the iliac bifurcation. Bleeding was controlled by clamping the aorta, followed by suturing of the fissure. Surgery was then completed uneventfully. The second patient developed a retroperitoneal hematoma confirmed by postoperative CT scan during lipectomy and liposuction. No further treatment was required after

Challenging Airway Management in a Patient with Retrosternal Goiter Presenting in Respiratory Distress.

A number of options exist for patients with anticipated difficult intubation on account of a retrosternal goiter compressing on the trachea. The chosen technique(s) to secure the airway in this delicate situation often depends on the location and degree of airway obstruction, available resources/facilities, and an anesthetist's experience and preferences. We report the case of a 68-year-old woman with severe airway obstruction from a retrosternal goiter coming for total thyroidectomy. Airway management started with an awake fiber-optic intubation, proceeded to a tracheostomy and finally to use

Anesthetic management of parasitic conjoined twins' separation surgery.

Parasitic twin is a rare form of conjoined twins with an incidence ranging from 1 in 50,000 to 1,00,000 live births. In thoracopagus type, both hearts are joined together and often are associated with underlying congenital cardiac malformations. The separation surgery is a challenging task for both the surgeon as well as anesthetist due to the complexity of the procedure and long duration of surgery, carrying mortality close to 100% in case of significant cardiac fusion. Here, we are sharing anesthetic management of successful separation of a rare type of parasitic male conjoined twins who had

Craniofacial surgery in Oman: a preliminary study of 10 cases.

Craniofacial surgery requires a highly specialized surgical team, appropriate precision delicate instruments, dedicated anesthetist, and upgraded intensive care unit facilities. This type of surgery is undertaken in a large number of congenital problems such as encephalocele, craniosynostosis, hypertelorism, tumors of the craniofacial region, and for craniofacial injuries. We present our experience with craniofacial surgery in 10 patients during a 15-month period at Khoula Hospital, Muscat, Sultanate of Oman. These operations were the first of this kind to be done in this part of the world. Th

Anesthetic Dilemma in a Full-term Parturient with Facial Trauma and Suspected Head Injury Presenting as Difficult Airway Posted for Emergency LSCS in a Limited Resource Setting: Spinal versus General Anesthesia.

Trauma frequently occurs during pregnancy and occurs 1 in 12 pregnant women affecting maternal mortality, morbidity, and also pregnancy outcome. Hence, it is essential to consider pregnancy in every female trauma patient of reproductive age. Motor vehicle accidents and domestic/intimate partner violence account for most cases of major maternal trauma. Most of the causes mentioned above can lead to facial injuries/head injuries making it challenging for the anesthesia provider. We report a parturient who presented to us with multiple facial injuries with a suspected head injury following a road

Impact evaluation of two different general anesthesia protocols (TIVA with propofol vs isoflurane) on the total number of interventions to treat cardiovascular depression or arousal/movement episodes in dogs undergoing orthopedic surgery receiving an intrathecal anesthesia.

The aim of this prospective, randomized clinical trial was to compare the total number of anesthetic interventions (TNAI) performed by the anesthetist to treat cardiovascular depression or arousal/movement episodes in dogs receiving intrathecal and general anesthesia (GA), maintained using propofol-based TIVA (group P) or isoflurane (group I). Mean arterial pressure (MAP) before (T0) and 12 min after intrathecal anesthesia (T1) and intraoperative vasoactive consumption were also compared. The TNAI to deepen the anesthetic plane or to treat hemodynamic depression in the pre-surgical and intra-s

Hypnosis for anaesthetists: a systematic review and meta-analyses.

INTRODUCTION: Therapeutic hypnosis appears to offer psychological and physiological benefits in various medical fields, but despite increasing interest, its value for anaesthesia remains inconclusive. METHODS: We searched for studies of any design in which hypnosis was used for any intervention requiring the presence of an anaesthetist, alone or in combination with any type of anaesthesia, on children and adults. Meta-analyses using random-effects models were stratified on hypnosis timing, when three or more randomised controlled trials reported on a similar outcome. Additional analyses were

A Word of Caution With Esophageal Replacement for Corrosive Esophageal Injury: A Case Report.

Corrosive ingestion remains a common problem in developing countries such as India due to the lack of strict laws that regulate the sale of caustics. Corrosives can damage any segment of the gastrointestinal tract. The chronic phase of corrosive injuries may result in several complications, with the most common among them being stricture. These chronic sequelae are frequently managed by esophageal replacement. Due to the associated injury to the larynx and the trachea, the airway management is complicated in the late phase when fibrosis and strictures distort the anatomy. We present a case of

The safety of anaesthesia delivered by rural generalist anaesthetists: a scoping review of the literature.

INTRODUCTION: Rural generalist anaesthetists (RGAs) are central to the delivery of health care in much of rural and remote Australia. This article details a systematic review of the literature specifically asking the question, 'What is the current evidence of the 'safety' of anaesthesia delivered by RGAs?' METHODS: Six databases were searched using terms including 'safety', 'rural', 'anaesthetics', 'general practitioners', and associated search terms. Relevant articles were assessed for rigour, and information was summarised using qualitative grid analysis that included information on the stu
