[Hypertension].

This 40 year old man had been treated adequately for arterial hypertension over the last 10 years. Within one year this arterial tension rose continuously in spite of treatment. A low blood-potassium value found on routine chemical analysis prompted an investigation on secondary causes for hypertension. The results of a peripheral renin-study were typical for secondary aldosteronism. An abdominal aortography and selective angiography of the renal arteries visualized an intrarenal arterial aneurysm with stenoses as causative factor of the renovascular hypertension. Antihypertensive treatment wi…

Abdominal aortic aneurysm of unknown origin and renovascular hypertension in a 25 year old male.

A 25 year old male had an abdominal aortic aneurysm involving bilateral renal arteries. Aortography revealed no irregularity in the aortic wall but a slight kinking at the tenth thoracic level followed by a fusiform aneurysm. Computed tomography (CT) showed an enlarged aortic lumen with an intraluminal thrombus. Plasma renin activity (PRA) was markedly increased (11 ng/ml/h), and the administration of captopril caused a further increase in PRA with a significant reduction in blood pressure, indicating the presence of renovascular hypertension. Abdominal aneurysm in a young adult is very rare. …

Transcatheter embolization of hepatic arteriovenous fistula in Osler-Weber-Rendu disease--a case report.

Hereditary hemorrhagic telangiectasia (Osler-Weber-Rendu disease) is associated with arteriovenous fistulas throughout the body that can cause hemodynamic abnormalities. Owing to their size and extent, surgical repair is often not feasible. A patient referred for presumed valvular heart disease is described. On the basis of oximetry data at cardiac catheterization, a large intrahepatic arteriovenous fistula was discovered by aortography. This finding and a history of recurrent epistaxis were consistent with Osler-Weber-Rendu disease. Owing to the size of the fistula, embolization of the right …

Takayasu's disease. A report of 3 cases.

Three patients with Takayasu's disease are described. In 2 cases aortography demonstrated an occlusion of the great vessels at the origin of the arch of the aorta. In 1 patient there was concomitant aneurysmal dilatation of the brachiocephalic trunk. Common and internal carotid artery stenosis occurred in 1 patient and was associated with hemiplegia and blindness. All 3 patients had constitutional symptoms and signs of the disease as well as markedly elevated erythrocyte sedimentation rates; 2 patients had moderate clinical responses to steroid administration in the short term and 1 developed …

[A new (?) multiple abnormality complex similar to the Poland syndrome].

The report describes a case Poland's Syndrome - like in which the hypoplasia of the left major pectoralis muscle was associated with: subtot … istics of the syndrome as well as the specific contribution provided in the case by Ultrasounds and aortography. Furthermore, the analysis of the case seems to indicate a strict relationship between the abnormalities of diaphragm, pectoralis muscle and homolateral upper limb.

Chronic visceral ischemia. An often overlooked cause of abdominal pain.

Chronic intestinal ischemia is usually caused by atherosclerosis of the visceral branches of the abdominal aorta. It classically presents with severe postprandial abdominal pain, weight loss due to fear of eating, and altered intestinal motility. Aortography is the definitive diagnostic study and usually reveals occlusive disease of at least two major visceral vessels. Revascularization is indicated in symptomatic patients but should be undertaken in asymptomatic patients only when aortic reconstruction is indicated that would threaten intestinal collateral blood flow. Refinements in operative…

Dissection and rupture of the ascending aorta. Unusual complications of aortocoronary bypass surgery.

This is a report concerning two cases of unusual complications, dissection and rupture of the ascending aorta, secondary to saphenous vein bypass surgery. We recommend that evaluation of patients undergoing saphenous vein bypass surgery include careful image-intensifier fluoroscopic examination during coronary arteriography. If aneurysmal dilatation of the aorta or calcification is observed, ascending aortography should be performed to better evaluate the aorta, and surgeons should be alerted to use appropriate maneuvers and techniques to minimize the risk of complications in the aortic site o…

[Angiographic study of congenital aortic stenosis (author's transl)].

Left ventriculography is of great interest in the diagnosis and prognosis of congenitalaortic stenosis. It permits one to distinguish valvular stenosis, which is the commonest type, from sub-valvular stenosis, of which there exist 5 main type; type I and II are the commonest, they have a better prognosis than types III and IV. Sub-sigmoid aortography shows the appearance of the sinus of Valsalva, thus whether there exists a tricuspid or bicuspid aortic valve. In all cases, one should seek stenosis at various levels of the aortic outlet and possible myocardiography of the left ventricle, the fr…

[Congenital supravalvular aortic stenosis; report of a case].

We experienced a successful surgical case of extended aortoplasty by means of Doty's method (two sinus reconstruction) for congenital supravalvular aortic stenosis (SAS). Case was 12-year-old boy, who had no complaint except heart murmur. The retrograde aortography demonstrated localized stenosis just above the aortic valve, and it was an hour-glass type. The preoperative peak systolic pressure gradient between the left ventricle and ascending aorta was 56 mmHg, which was improved postoperatively. This case showed excellent results. Doty's aortoplasty was favorable method for SAS without defor…

[A case of carcinoma of the pancreas head associated with celiac axis compression syndrome].

We report a 51-year-old woman with pancreas head cancer associated with celiac axis compression syndrome (CACS). Angiography demonstrated that the inferior pancreatico-duodenal artery and the posterior superior pancreaticoduodenal artery dilated significantly. The lateral view of aortography showed compression of the celiac axis. We performed pancreatoduodenectomy safely with cutting the median arcuate ligament. Postoperative course was uneventful. Postoperative angiography demonstrated improvement of the compression. In CACS, celiac axis is compressed by the median arcuate ligament (seichu ky…

Aortic root dissection complicating coronary bypass surgery.

Four cases are presented of aortic root dissection after aortocoronary bypass surgery in which the origin of the intimal tear was at or very near the aortic site of saphenous vein anastomosis. Two cases were documented at autopsy. In one of two cases diagnosed with aortography, the patient underwent surgical correction and survived. All patients had long-standing severe hypertensive cardiovascular disease or severe generalized atherosclerotic disease, or both. Clinical awareness of aortic dissection after coronary bypass surgery in this group of patients should make early diagnosis with succes…

Exclusion of aortic tear in the unstable trauma patient: the utility of transesophageal echocardiography.

OBJECTIVE: The goal of this study was to investigate the value of biplanar transesophageal echocardiography (TEE) as a screening tool for aortic tear in unstable trauma patients. METHODS: During a 1-year period, a prospective trial to exclude aortic tear was conducted at a level I trauma center. Ten of 53 patients (19%) sustaining severe blunt thoracic trauma were deemed too unstable to undergo safe transport to aortography and underwent TEE. Mechanism of injury was motor vehicle crash in eight patients and pedestrians struck in two. Patients had a mean Injury Severity Score = 34 (range, 17 t…

[Annuloaortic Ectasia Associated with Single Coronary Artery and Ventricular Septal Defect;Report of a Case].

Single coronary artery is rear. Various combination anomalies have been reported in patients with this anomaly undergoing heart surgery. We report a case of annuloaortic ectasia combined with single coronary artery and ventricular septal defect. The patient was a 67-year-old man with exertional dyspnea. Coronary angiography and computed tomography revealed a single coronary artery. Aortography showed severe aortic regurgitation and dilatation of Valsalva sinus. Ultrasonic cardiography showed small ventricular septal defect at the membranous septum. He underwent Modified Bentall's operation. He…

A case of aortic aneurysm hospitalized as acute coronary syndrome.

Saccular type of thoracic aortic aneurysm is a rarely seen phenomenon. Here, we present a case of saccular-type aortic aneurysm admitted to coronary care unit with a diagnosis of acute coronary syndrome. A 63-year-old woman presented to our clinic with chest pain lasting for 2-3 hours. Because her chest pain persisted despite intensive medical treatment, she underwent coronary angiography. Coronary arteries appeared normal but a saccular type aneurysm of ascending aorta was detected on aortography. The patient underwent urgent surgery. The aneurysmal segment was resected and tube graft inserte…

[Transfemoral plug closure in 45 cases of patent ductus arteriosus].

Forty five consecutive patients with simple patent ductus arteriosus were successfully closed using transfemoral catheter method originally introduced by Porstmann in 1967. However, some improvements of instrumentation and technic were made for achieving much more safety and reliability. The diameter of ductus shown in aortography was from 0.3 to 1.0 cm. Primary success was achieved after the first trial, two trials were necessary only in one case. Emphasis was put on the proper size and shape of plug in order to obtain excellent result. 1-54 months follow-up study after procedure revealed no …

Results of surgery for congenital supravalvular aortic stenosis.

Of eight children aged 3 to 15 years with surgical correction of severe supravalvular aortic stenosis, 6 were evaluated 7 to 44 months later by repeat cardiac catheterization and aortography. Prosthetic patch angioplasty was performed in all cases. Preoperative systolic gradients ranged from 40 to 90 mm Hg (average 70); postoperative gradients ranged from 0 to 20 mm Hg (average 11). The postoperative anglographic appearance of the ascending aorta was near normal in all six patients, and none had new aortic valve insufficiency. These results of surgery for supravalvular aortic stenosis are judg…

Thoracic endovascular aortic repair in a case of grade III blunt aortic injury with aberrant vertebral artery origin.

Aberrant origin of the left vertebral artery (LVA) can pose a challenge during thoracic endovascular aortic repair. We encountered such a patient who was involved in a motor vehicle accident in whom computed tomography angiography revealed a grade IIIB blunt aortic injury with an anomalous origin of the LVA distal to the origin of the left subclavian artery. On-table aortography confirmed dominance of the LVA. Hence, an open left carotid-vertebral and then left carotid-subclavian artery bypass was performed, followed by thoracic endovascular aortic repair. The patient recovered well and was di…
