Factors leading to long-term hospitalization after laparoscopic appendectomy.

BACKGROUND: Randomized studies demonstrate that laparoscopic appendectomy yields better results compared with open techniques. We sought to identify factors that determine an extended hospital stay among patients undergoing laparoscopic appendectomy. METHODS: This was a prospective study including 669 patients undergoing laparoscopic appendectomy. We analyzed variables that can predict the length of hospital stay. RESULTS: Of 669 patients undergoing laparoscopic appendectomy, 141 stayed in the hospital for > or = 5 days (Group 1), and 97 stayed in the hospital for < or = 1 day after surgery …

Results of laparoscopic vs. conventional appendectomy in complicated appendicitis.

BACKGROUND: Although many trials show some advantages of laparoscopic appendectomy over open appendectomy, the value of laparoscopic appendectomy is still controversial. Specifically the question of whether there are benefits of laparoscopic appendectomy over open appendectomy in complicated appendicitis remains to be answered. METHODS: Of 1,106 consecutive appendectomies (717 laparoscopic appendectomies, 330 open appendectomies, and 59 conversions) between 1989 and 1999, the results of 299 patients with complicated appendicitis (defined by perforation, abscess, or peritonitis) were analyzed …

Intra-abdominal abscess after laparoscopic appendectomy for perforated appendicitis.

HYPOTHESIS: The incidence of postoperative intra-abdominal abscess is higher after laparoscopic compared with open appendectomy for perforated appendicitis. METHODS: A historical cohort study of pediatric patients operated on for suspected appendicitis by open appendectomy or laparoscopic appendectomy compares the incidence of postoperative intra-abdominal abscess for each procedure. SETTING: A tertiary care center. PATIENTS: Five hundred thirty-eight pediatric patients were operated on for suspected appendicitis at our institution between 1974 and 1999. Of these, 453 were included in the s…

Laparoscope-assisted appendectomy in children: the two-trocar technique.

UNLABELLED: The conventional cannulae position in laparoscopic appendectomy does not permit good visualization of the appendix, especially if it is in a retrocaecal or pelvic position. In children, the smaller dimension of the peritoneal cavity aggravates this problem. METHOD: In order to optimize appendix visualization, we propose a modification of the cannulae position associated with extraabdominal appendectomy. Two trocars are inserted, one in the umbilicus under direct vision and one in the midline above the pubic symphysis. The telescope is inserted in the second cannula. The appendix i…

[Laparoscopic appendectomy in pediatric patients: experience of 2300 operations].

The experience of 2300 laparoscopic appendectomies in children aged between 7 days and 14 years is presented. Uncomplicated forms of acute appendicitis were observed in 1611 patients (70.1%). Complicated forms were observed in 689 (29.9%) patients: general peritonitis in 196, local peritonitis--in 334, periappendicular abscess--in 122 and friable infiltrate--in 37 patients. Laparoscopic appendectomy according to F. Götz modified and adapted by us to children's age was used for all variants of the appendix location, including atypical. Optimal surgical approaches for children of early age and c…

Computed tomography of superior mesenteric vein thrombosis following appendectomy.

During a 5-year period, superior mesenteric vein (SMV) thrombosis was detected with computed tomography (CT) in six patients shortly after an appendectomy. No sign of SMV was present at appendectomy, and a period of more than 2 weeks free of clinical symptoms had elapsed between the appendectomy and the onset of the SMV thrombosis. In four cases, the appendicitis was complicated. These patients had nonspecific signs and symptoms, although two of them had elevation of blood hepatic enzyme levels. In all cases, postcontrast CT demonstrated enlargement of the SMV, with well-defined enhancement of…

Prophylactic metronidazole in appendectomy: a double-blind controlled trial.

Ninety-eight patients with suspected nonperforated acute appendicitis have to date been entered into a continuing, prospective, blinded randomized trial to determine the role of prophylactic metronidazole in postoperative wound infection. Twenty-six patients were excluded for the following reasons: diagnosis other than acute appendicitis (16), perforation of the appendix (8), administration of other antibiotics (1), and refusal to enter study (1). Seventy-two patients, 47 men and 25 women, with a mean age of 27.5 years (range 15 to 60 years), underwent appendectomy and were studied. Thirty pat…

Bilateral ovarian masses, massive ascites, and prior appendectomy: must it be ovarian cancer? --a case report of pseudomyxoma peritonei 24 years after appendectomy.

This case report presents a patient who underwent an appendectomy 24 years ago and was later admitted with bilateral ovarian masses and massive ascites. Ovarian cancer was suspected, and surgical intervention was prompted. During the operation, the abdominal cavity was found to be filled with yellow, jelly-like material. Both intraoperative frozen section and postoperative paraffin-embedded pathology confirmed that the ovarian masses were actually mucinous tumors originated from gastrointestinal duct and was clinically diagnosed as pseudomyxoma peritonei (PMP). This case highlights that even i…

LAPAROSCOPIC APPENDECTOMY DOES NOT INCREASE THE RATE OF NEGATIVE APPENDECTOMY along with a lower rate of perforated appendicitis - RESULTS IN 1899 PATIENTS at Zagreb UHC.

- Laparoscopic appendectomy is the method of choice of many professional societies owing to its many advantages. The question arises whether surgeons urge more easily to laparoscopic exploration due to its less invasiveness, faster recovery and adequate exploration of the entire abdominal cavity than to observation in unequivocal cases. This retrospective analysis (2009-2016) included 1899 patients undergoing laparoscopic (lap) or gridiron intra-abdominal approach treated at Zagreb University Hospital Centre. The analysis included total negative appendectomy, negative-negative appendectomy (no…

Transumbilical single-port appendectomy in children: a retrospective study.

OBJECTIVE: Acute appendicitis is the leading cause of abdominal surgery. This study evaluated outcomes of transumbilical single-incision laparoscopic appendectomy (TSILA) compared with conventional laparoscopy and open surgery. METHODS: A retrospective study of 550 pediatric patients (0-18 years) undergoing an appendectomy (2020-2023) at a tertiary center was conducted. Patients were stratified by technique and appendicitis grade (I-IV). Data were analyzed using Pearson's Chi-squared and ANOVA tests (p < 0.05). RESULTS: TSILA accounted for 34.5% of cases (n = 190) and showed shorter hospital…

[A Case of Appendiceal Neuroendocrine Tumor Diagnosed after Appendectomy].

A 49‒year‒old woman presented to our hospital with right lower abdominal pain that had been presenting for about 6 days without any improvement. An abdominal CT examination demonstrated an enlarged appendix and stercolith of appendix with inflamed surrounding adipose tissue. Antibiotic treatment was administered, but an abdominal CT examination taken 3 days later showed worsening inflammation around the appendix. An appendectomy was laparoscopically performed. Upon resection, the enlarged appendix was found. In immunohistochemical staining, the resected tissue was positive for neuroendocrine m…

Towards ambulatorization of appendectomy: Lessons learned during the pandemic.

BACKGROUND: Acute appendicitis is the most common abdominal surgical emergency worldwide. Researchers from various countries have evaluated the impact of the COVID-19 pandemic on its diagnosis and treatment, as well as a possible change in its management, such as outpatient treatment. The objective of this study was to describe the characteristics of acute appendicitis treated during the COVID-19 State of Emergency (SOE) and to assess whether it could be included in an ambulatory surgery program. METHODS: Retrospective observational study involving patients treated at public hospitals for app

Acute purulent enteritis after appendectomy: report of a case.

Acute purulent enteritis is uncommon, which occasionally occurs in association with mechanical injuries from foreign bodies of helminth parasites and the contaminated food, leading to bacterial invasion. Herein we report a case of acute purulent enteritis after appendectomy. A 44-year-old male was diagnosed with right lower abdominal pain and vomiting. Acute appendicitis was diagnosed, and an open appendectomy was performed. Postoperatively, the patient developed symptoms of small intestine obstruction. A laparotomy revealed necrosis of the small intestine, and resection was performed. Patholo…
