Alpha-thalassaemia and population health in Southeast Asia.

Alpha-thalassaemia mutations are common. In Southeast Asia, they cause Hb H disease and Hb Barts hydrops fetalis. Fetuses with the devastating Hb Barts hydrops fetalis due to the complete lack of alpha-globin gene die in utero or shortly after birth, often during the second or third trimesters. Recent findings on patients with Hb H disease who have only one active alpha-globin gene suggest that it is not necessarily a benign disorder as previously thought. The disease burden of these syndromes and their public health importance have been largely neglected. We review the population carrier freq…

Coltiviruses and seadornaviruses in North America, Europe, and Asia.

Coltiviruses are tickborne viruses of the genus Coltivirus. The type species, Colorado tick fever virus (from North America), has been isolated from patients with flulike syndromes, meningitis, encephalitis, and other severe complications. Another coltivirus, Eyach virus, has been isolated from ticks in France and Germany and incriminated in febrile illnesses and neurologic syndromes. Seadornaviruses are endemic in Southeast Asia, particularly Indonesia and China. The prototype virus of the genus, Banna virus (BAV), has been isolated from many mosquito species, humans with encephalitis, pigs, …

Smokeless tobacco and health in India and South Asia.

South Asia is a major producer and net exporter of tobacco. Over one-third of tobacco consumed regionally is smokeless. Traditional forms like betel quid, tobacco with lime and tobacco tooth powder are commonly used and the use of new products is increasing, not only among men but also among children, teenagers, women of reproductive age, medical and dental students and in the South Asian diaspora. Smokeless tobacco users studied prospectively in India had age-adjusted relative risks for premature mortality of 1.2-1.96 (men) and 1.3 (women). Current male chewers of betel quid with tobacco in c…

Update on the prevalence of malnutrition among children in Asia.

Approximately 70.0% of the world's malnourished children live in Asia, resulting in the region having the highest concentration of childhood malnutrition. About half of the preschool children are malnourished ranging from 16.0% in the People's Republic of China to 64.0% in Bangladesh. Prevalence of stunting and underweight are high especially in South Asia where one in every two preschool children is stunted. Besides protein-energy malnutrition, Asian children also suffer from micronutrient deficiency. Iron deficiency anaemia affects 40.0-50.0% of preschool and primary school children. Nearly …

Training and delivery of pediatric surgery services in Asia.

PURPOSE: The purpose of this report is to obtain an overview of pediatric surgery training in Asia and to have a glimpse of its delivery. METHODS: A questionnaire survey was conducted, and the returns from 41 (67%) pediatric surgeons from 14 (88%) Asian countries were reviewed. RESULTS: The number of pediatric surgeons per million population is lowest in Indonesia (0.1) and highest in Japan (25). Two cities in China have 100 or more pediatric surgeons. The number of staff surgeons per center ranges from 1 to 36. Centers with 5 or fewer neonatal surgical operations per staff surgeon per year …

The epidemiology of diabetes mellitus in the Asia-Pacific region.

The Asia-Pacific region is at the forefront of the current epidemic of diabetes. There are currently more than 30 million people with diabetes in the Western Pacific region alone. The World Health Organization predicts that this number will rise dramatically by the year 2025, by which time India and China may each face the problem of dealing with 50 million affected individuals. The problem in the region results from a combination of large population size with rapidly rising prevalence rates, particularly of type 2 diabetes mellitus. Although much heterogeneity exists, rising prevalence rates …

Solving the micronutrient problem in the Asia Pacific region.

Over two billion people, or more than one out of three individuals throughout the world, are at risk of iron, vitamin A and iodine deficiencies. Although countries of the Asia Pacific region have generally shown a remarkable decline in the proportion of malnourished children, micronutrient deficiencies remain significant public health problems. The World Summit for Children in 1990, and the FAO/WHO International Conference on Nutrition held in Rome in 1992, affirmed that the elimination of the various forms of micronutrient malnutrition would constitute a significant contribution to social, ec…

Social and cultural aspects of organ donation in Asia.

In Asian countries, it is more difficult to obtain cadaver kidneys for renal transplantation because of certain socio-cultural beliefs and customs. The issues affecting living related kidney donation are more social than cultural. This is due to the web of family pressures and personal conflicts for both donor and recipient surrounding the donation. Drug control policies in some parts of Asia.

The Asian region has experienced drug problems ever since opium was exported to China in the eighteenth century. Today, in some parts of the region, high priority is placed on drug control policies relating, for example, to the eradication of opium poppy cultivation, criminal sanctions against drug trafficking, and treatment programmes for drug-dependent persons. Some good results have been achieved in recent years: according to estimates, opium cultivation in at least one country in the region has decreased; the number of new cases involving drug-dependent persons is falling in a number of co…

Historical review and current status of gnathostomiasis in Asia.

The genus Gnathostoma contains at least twelve distinctive species. Among these, 6 species, namely G. spinigerum, G. hispidum, G. doloresi, G. vietnamicum, G. malaysiae and G. nipponicum, are found in wild animals in Asia. Related to human gnathostomiasis, G. spinigerum has long been believed as the only causative species. Until the early 1980s, all gnathostomiasis cases found in major endemic areas in Asia, such as Thailand, China and Japan, were caused by infection with G. spinigerum. In the early 1980s in Japan, new gnathostomiasis cases appeared in urban areas and these cases were shown to…

Septic melioidosis after a visit to Southeast Asia.

A case is reported of a fifty-seven year old man with fever, who was admitted to hospital after a recent visit to Southeast Asia. Among the clinical findings prostatitis and broncho-pneumonia were noted. Within twenty-four hours irreversible fulminant sepsis developed although he was treated with cefotaxime, tobramycin and erythromycin. Post mortem Pseudomonas pseudomallei was cultured from blood and aspirate collected by bronchoscopy. It is important to consider melioidosis as a cause of septic illness in patients who have been visiting Southeast Asia.

Energy dilemmas in Asia: the needs for research and development.

Outside of China, the countries of southern and eastern Asia contain 30 percent of the world's population but only 2 percent of the known fossil fuel resources. Economic growth has resulted in increasing imports of petroleum and petroleum products. Because of the tenfold rise in oil prices since 1972, several of these countries are faced with two dilemmas-one short range and one long range. Unless they can discover more fossil fuel resources within their own borders, they must either incur dangerously growing foreign exchange deficits or drastically slow their economic growth. Research and dev…
