Hypothermic treatment for neonatal asphyxia in low-resource settings using phase-changing material-An easy to use and low-cost method.

AIM: To evaluate whether phase-changing material can be used for therapeutic hypothermia of asphyxiated newborns in low-resource settings. METHODS: Prospective interventional study of asphyxiated term infants fulfilling criteria for hypothermia treatment at Vietnam National Children's Hospital from September 2014 to September 2016. Hypothermia was induced within 6 hours after birth and maintained for 72 hours by a phase-changing material mattress with melting point of 32°C. Rectal temperature was continuously measured, and deviations from target temperature range 33.5-34.5°C were recorded. R

Determining the contribution of asphyxia to brain damage in the neonate.

Studies in the research laboratory have demonstrated the complex relationship between fetal and newborn asphyxia and brain damage, a balance between the degree, duration and nature of the asphyxia and the quality of the cardiovascular compensatory response. Clinical studies would support the contention that the human fetus and newborn behave in a similar manner. An accurate diagnosis of asphyxia requires a blood gas and acid base assessment. The clinical classification of fetal asphyxia is based on a measure of metabolic acidosis to confirm that fetal asphyxia has occurred and the expression o

Benefits of neonatal resuscitation training in high- and low-resource settings.

Globally, around 1.9 million stillbirths occur each year, and around 2.3 million newborns die in the first 28 days after birth each year. Many of these deaths are preventable. The burden of stillbirths is likely even higher, as many are unreported. A disproportionate number of fresh stillbirths and neonatal deaths occur in low-resource settings. About 1 million neonatal deaths occur in the first 24 h after birth, with preterm birth and birth asphyxia/intrapartum-related death being the leading causes. For newborns who have difficulty establishing effective breathing at birth, prompt and effect

Global, regional, and national burden of neonatal encephalopathy due to birth asphyxia and trauma, 1990-2019.

BACKGROUND: Neonatal encephalopathy (NE) due to birth asphyxia and trauma has become the second leading cause of global neonatal and under-5 deaths. However, comprehensive data on the global NE burden are scarce. This study aimed to determine the global, regional, and national trends in NE burden from 1990 to 2019, utilizing the 2019 Global Burden of Disease (GBD) study data. METHODS: Annual incident cases, deaths, disability-adjusted life years (DALYs), and age-standardized rates (incidence [ASIR], mortality [ASMR], DALYs) of NE were collected from the 2019 GBD study. The percentage of relat

The Complex Interrelationship Between Mechanical Ventilation and Therapeutic Hypothermia in Asphyxiated Newborns. A Review.

Asphyxiated newborns often require both therapeutic hypothermia (TH) and mechanical ventilation (MV) and the complex interrelationship between these two therapeutic interventions is very interesting, which could not only have several synergistic positive effects but also some risks. Perinatal asphyxia is the leading cause of neonatal hypoxic-ischemic encephalopathy (HIE) and TH is the only approved neuroprotective treatment to limit brain injury, improving the mortality rate and long-term neurological outcomes. HIE is often associated with severe respiratory failure, requiring MV, due to diffe

Development of a novel mobile application, HBB Prompt, with human factors and user-centred design for Helping Babies Breathe skills retention in Uganda.

BACKGROUND: Helping Babies Breathe (HBB) is a life-saving program that has helped reduce neonatal morbidity and mortality, but knowledge and skills retention after training remains a significant challenge for sustainability of impact. User-centred design (UCD) can be used to develop solutions to target knowledge and skills maintenance. METHODS: We applied a process of UCD beginning with understanding the facilitators of, and barriers to, learning and retaining HBB knowledge and skills. HBB Master Trainers and frontline HBB providers participated in a series of focus group discussions (FGDs) t

Activity Recognition From Newborn Resuscitation Videos.

OBJECTIVE: Birth asphyxia is one of the leading causes of neonatal deaths. A key for survival is performing immediate and continuous quality newborn resuscitation. A dataset of recorded signals during newborn resuscitation, including videos, has been collected in Haydom, Tanzania, and the aim is to analyze the treatment and its effect on the newborn outcome. An important step is to generate timelines of relevant resuscitation activities, including ventilation, stimulation, suction, etc., during the resuscitation episodes. METHODS: We propose a two-step deep neural network system, ORAA-net, ut

[Critical Outcome and Hypoxic Ischemic Encephalopathy - A quality Assurance Issue].

UNLABELLED: The study evaluates the predictive value of the critical status of a newborn as to the risk of developing hypoxic ischemic encephalopathy (HIE). METHODS: On the basis of the data set from the perinatal survey in Hesse, Germany, in the year 2016, including 52,122 live births (singleton, 37+0 GA), cases of critical newborns were identified. A conjoined analysis with the data set of the neonatal survey from the identical period provided the basis to evaluate the relationship to cases compromised by HIE. RESULTS: The incidence of cases with a critical outcome (n=11) and those with HI

Perinatal hypoxic-ischemic brain injury in large animal models: Relevance to human neonatal encephalopathy.

Perinatal hypoxia-ischemia resulting in death or lifelong disabilities remains a major clinical disorder. Neonatal models of hypoxia-ischemia in rodents have enhanced our understanding of cellular mechanisms of neural injury in developing brain, but have limitations in simulating the range, accuracy, and physiology of clinical hypoxia-ischemia and the relevant systems neuropathology that contribute to the human brain injury pattern. Large animal models of perinatal hypoxia-ischemia, such as partial or complete asphyxia at the time of delivery of fetal monkeys, umbilical cord occlusion and cere

Hypoxic-Ischaemic Encephalopathy and the Blood-Brain Barrier in Neonates.

This review aims to highlight a possible relationship between hypoxic-ischaemic encephalopathy (HIE) and the disruption of the blood-brain barrier (BBB). Inflammatory reactions perpetuate a large proportion of cerebral injury. The extent of injury noted in HIE is not only determined by the biochemical cascades that trigger the apoptosis-necrosis continuum of cell death in the brain parenchyma, but also by the breaching of the BBB by pro-inflammatory factors. We examine the changes that contribute to the breakdown of the BBB that occur during HIE at a macroscopic, cellular, and molecular level.

Stress Induced Gastric Ulcers: Presenting as Massive Rectal Bleeding in a Newborn.

Severe gastrointestinal bleeding in newborn period is a serious but uncommon phenomenon that has a broad differential diagnosis. Primary duodenal ulcers are rare in children but stress induced ulceration in stomach occurs more often in neonatal period due to birth asphyxia, prolonged labour, cesarean deliveries, instrumentations, respiratory distress syndrome and sepsis. These present as acute onset of gastrointestinal bleeding commonly as altered gastric aspirate, hematemesis or malena. We report a case of a neonate with stress induced gastric bleeding following birth asphyxia who presented w

Urinary NGAL to define AKI in asphyxiated infants.

Acute kidney injury (AKI) is independently associated with poor outcomes in the critically ill patient. The standard kidney function biomarker, serum creatinine, shows a demonstrable rise in concentration many hours to days after insult to the kidney. Thus, creatinine-based AKI diagnosis is likely delayed, rendering treatments to mitigate or prevent AKI ineffective. Neonatal AKI is further confounded by the fact that infant serum creatinine concentrations reflect maternal levels. The past 15 years has seen a massive research effort to identify early damage markers of AKI, with the hope that ea

[REACTION OF STRUCTURAL ELEMENTS OF HEMATOENCEPHALIC BARRIER IN NEWBORN RATS TO NORMOBARIC HYPOXIA].

For the last time a particular interest of investigators has been attracted to the period of early newborn state when active process of adaptation of the organism to new life conditions occur defining its increased sensitivity to the effect of unfavorable environmental factors. An important place among these processes belongs to formation of homeostasis mechanisms and, primarily, the barrier mechanisms. The purpose of the present study was to investigate reactions of the hematoencephalic barrier (HEB) to action of perinatal normobaric hypoxia (a model of incomplete human pregnancy). Using ligh

Angiogenesis dysregulation in term asphyxiated newborns treated with hypothermia.

BACKGROUND: Neonatal encephalopathy following birth asphyxia is a major predictor of long-term neurological impairment. Therapeutic hypothermia is currently the standard of care to prevent brain injury in asphyxiated newborns but is not protective in all cases. More robust and versatile treatment options are needed. Angiogenesis is a demonstrated therapeutic target in adult stroke. However, no systematic study examines the expression of angiogenesis-related markers following birth asphyxia in human newborns. OBJECTIVE: This study aimed to evaluate the expression of angiogenesis-related protei

[Assessment of levels of otoacoustic emission response in neonates with perinatal asphyxia].

OBJECTIVE:: To evaluate the effects of perinatal asphyxia on the level of the response to transient otoacoustic emissions in infants. METHODS:: Otoacoustic emissions in 154 neonates were performed: 54 infants who suffered asphyxia at birth, measured by Apgar score and medical diagnosis, and 100 infants without risk were compared. Scores less than 4 in the first minute and/or less than 6 in the fifth minute were considered as "low Apgar". Statistical analysis of the data was performed using the Kruskal, Wilcoxon, and Mann-Whitney nonparametric tests. RESULTS:: Lower levels of response were ob
