BACKGROUND: Left atrial appendage aneurysm (LAAA) is a rare condition mostly due to congenital malformations or secondary causes (i.e. mitral regurgitation).
CASE SUMMARY: We present a case of a 47-year-old male with a history of atrial fibrillation treated with propafenone presented to our emergency department for palpitation and epigastric pain. The electrocardiogram showed atrial fibrillation at high ventricular rate and a new-onset left bundle branch block. Urgent coronary angiogram excluded coronary artery disease. Echocardiography and cardiac magnetic resonance revealed a giant LAAA. Th…

Left atrial appendage occlusion (LAAO) is an increasingly used alternative to oral anticoagulation in patients with atrial fibrillation, especially in patients with absolute/relative contraindications to these therapies. This review will cover three main aspects of the procedure. In the fist part of the manuscript, we focus on patient selection. We describe three main categories of patients with primary indication to LAAO, namely patients with previous or at a high risk of intracerebral bleeding, patients with a history of major gastrointestinal bleeding and patients with end-stage renal disea…

BACKGROUND: Transcatheter left atrial appendage (LAA) closure is an alternative therapy for stroke prevention in atrial fibrillation (AF) patients. However; real-world efficacy, safety and complications have yet to be investigated. We sought to determine the procedural outcomes and potential complications of LAA closure in routine clinical practice at a high-volume center.
METHODS AND RESULTS: The study group comprised 179 patients (105 males; 72.7±9.0 years) with AF undergoing LAA closure at a single center in Germany. The rate of successful implantation was 98.9% (2 patients did not undergo…

We report a rare case of an inverted left atrial appendage without prior cardiac surgery. A left atrial mass was incidentally found during routine echocardiography in a 19-year-old man with mitral valve prolapse. Echocardiography revealed a hyperechoic mass in the left atrium, and a neoplastic lesion could not be excluded. On magnetic resonance imaging (MRI), this mass consisted of fat tissue that showed continuation to epicardial fat, indicating an inverted left atrial appendage mimicking a tumor in the left atrium. When a mass in the left atrium is observed on echocardiography, there are sev…

Juxtaposition of the atrial appendages (JAA) is an uncommon anomaly of the heart that is frequently associated with other cardiac abnormalities, such as transposition of the great arteries and tricuspid atresia. We encountered 7 patients with JAA from July 1984 to July 1989 at National Taiwan University Hospital. The JAA was right-sided in 1 patient and left-sided in 6. The patients' ages ranged from 10 months to 12 years. The cardiac anomalies in the patient with right JAA were double-outlet right ventricle and ventricular septal defect, which was repaired with a patch. Of the 6 patients with…

The possible diagnostic value of an enlarged left atrial appendage (LAA) on the posterior-anterior or right anterior oblique chest film as a means of implicating a rheumatic etiology for mitral valve disease in children was investigated. Chest films were examined without prior knowledge of clinical or laboratory data, and the results were later correlated with this information in 113 children and adolescents. The clinical and laboratory data included application of the modified Jones criteria for the diagnosis of acute rheumatic fever, streptococcal antibody titers and clinical and cardiac cat…

BACKGROUND: The potential for thromboembolism in atrial flutter (AFL) is different from atrial fibrillation. AFL cycle length (AFL-CL) may be related to reduced left atrial appendage (LAA) function. Very rapid AFL-CL can lead to mechanical and electrophysiological disorders that contribute to lower LAA emptying velocity (LAEV). The aim of this study is to relate atrial flutter cycle length with LAEV and its role in thrombogenesis.
METHODS: Cross-sectional study of patients with atrial flutter AFL who underwent transoesophageal echocardiography (TEE) before catheter ablation or electric cardio…

BACKGROUND: The long-term effects of catheter ablation (CA) on the left atrium and left atrial appendage (LAA) are unknown in persistent atrial fibrillation (AF). This study investigated left atrial (LA) reverse remodeling and evolution of LA/LAA function after successful CA for persistent AF and identified predictors for maintenance of sinus rhythm (SR) and LA reverse remodeling.
METHODS AND RESULTS: CA was performed in 123 patients with persistent AF. LA volumes, LA strain and LAA wall velocity were assessed both at baseline and at 12 months after ablation. Patients who maintained SR were d…

BACKGROUND: Concomitant atrial fibrillation (AF) and gastrointestinal (GI) bleeding present a clinical challenge due to recurrent bleeding risk associated with anticoagulation for AF-associated stroke prevention. Left atrial appendage occlusion (LAAO) offers an alternative stroke prevention strategy, but its impact on recurrent GI bleeding remains unknown.
METHODS: This retrospective, multicenter cohort study used the TriNetX database to identify adults with AF on oral anticoagulation and a history of GI bleeding. Patients were stratified by treatment with or without LAAO. 1:1 propensity scor…

BACKGROUND: Dense spontaneous echo contrast (SEC) and sludge in the left atrial appendage (LAA) complicate differentiation between sludge and thrombi during transoesophageal echocardiography (TOE). We prospectively evaluated the feasibility, safety, and efficacy of dobutamine to reduce sludge and improve thrombus detection in patients with atrial fibrillation (AF).
METHODS AND RESULTS: This was an open-label, single-arm, single-center clinical study. A total of 20 patients with LAA sludge on TOE received intravenous dobutamine infusion, starting at 5 μg/kg/min and increasing to 10 and 20 μg/k…

BACKGROUND: Focal atrial tachycardias (ATs) originating from the left atrial appendage (LAA) apex are rare and challenging to treat given the high risk of cardiac perforation with conventional radiofrequency ablation.
CASE SUMMARY: A 15-year-old girl with an incessant AT arising from the LAA apex was successfully and safely treated using a novel contact force-sensing focal pulsed-field ablation (PFA) catheter with 10 applications. The patient remained asymptomatic, without tachycardia recurrence throughout a 4-month follow-up.
DISCUSSION: This first report demonstrates the safety and efficie…

BACKGROUND: Left atrial appendage occlusion (LAAO) is a treatment option to prevent thromboembolic events in patients with atrial fibrillation. However, it remains unclear whether LAAO offers superior efficacy and safety compared with oral anticoagulation for these patients.
OBJECTIVES: This study aimed to assess the efficacy and safety of LAAO vs direct oral anticoagulants (DOACs).
METHODS: We reviewed randomized controlled trials comparing oral anticoagulation and LAAO, using MEDLINE and CENTRAL. The primary outcome was a composite of stroke, systemic embolism, and cardiovascular death, wh…

Background/Objectives: Left atrial appendage (LAA) morphology has been implicated in atrial fibrillation (AF) recurrence following catheter ablation. However, the predictive value of specific anatomical shapes remains unclear. We aimed to evaluate the association between distinct LAA morphologies and AF recurrence post-ablation. Methods: In this retrospective, single-center study, 463 patients with AF undergoing first-time catheter ablation were included. Pre-ablation contrast-enhanced cardiac computed tomography was performed to classify LAA morphology into chicken-wing, windsock, cauliflower…

BACKGROUND: Focal atrial tachycardia (AT) originating from the left atrial appendage (LAA) presents clinical challenges.
CASE SUMMARY: The patient was a 45-year-old man who presented with drug-refractory AT. Activation mapping indicated focal AT originating from the LAA, and endocardial ablation failed. Ethanol infusion via the vein of Marshall (VOM) terminated the focal AT.
DISCUSSION: This is to our knowledge the first report of ethanol ablation via the VOM as an approach when endocardial ablation fails.
TAKE-HOME MESSAGE: Ethanol infusion via the VOM can be a viable method to eliminate f…

BACKGROUND: The left atrial appendage (LAA) is a trigger for atrial fibrillation (AF) in nearly one-third of patients with persistent AF undergoing repeat ablation, and LAA isolation (LAAI) has been shown to improve arrhythmia outcomes. Previous studies of LAAI using an ostial approach without vein of Marshall ethanol ablation (VOM) resulted in variable rates of acute and durable LAAI and an elevated risk of stroke.
OBJECTIVE: This study aimed to assess the feasibility and effect of LAAI using an anterior mitral isthmus line (AML) and posterior mitral isthmus line (PML), with adjunctive VOM, …

BACKGROUND AND OBJECTIVES: Although left atrial appendage closure (LAAC) is performed in patients with non-valvular atrial fibrillation (NVAF) at increased risk of intracranial haemorrhage (ICH), outcome data are scarce. We assessed the detailed neurological indications for LAAC and outcomes after LAAC in high ICH risk patients.
METHODS: Study population included consecutive patients with NVAF who underwent LAAC in a single hospital network between January 2015 and October 2021 because of prior ICH or the presence of high ICH risk imaging markers on brain MRI (cerebral microbleeds (CMBs)). Pr…

Atrial fibrillation (AF) is associated with an increased risk of stroke and systemic embolism, and the left atrial appendage (LAA) has been identified as a principal source of thromboembolism in these patients. While oral anticoagulation is the current standard of care, LAA closure (LAAC) emerges as an alternative or complementary treatment approach to reduce the risk of stroke or systemic embolism in patients with AF. Moderate-sized randomized clinical studies have provided data for the efficacy and safety of catheter-based LAAC, largely compared with vitamin K antagonists. LAA device iterati…
