The contribution of cervical spine disorders to cochleovestibular symptoms was based on the evaluation of patients complaining of dizziness and/or inner ear hearing loss, treated in the ENT Department of Tubingen University between 1977 and 1982. Cervicogenic vertigo was provoked by movements of the cervical spine, but the latter rarely caused tinnitus and never induced hearing loss. The diagnosis between a vascular or proprioceptive nystagmus may be made on the presence of lesions of the cervical spine. These are disorders of the cranio-cervical or the thoraco-cervical region especially and m

After a short introduction of theoretical principles of tomography, the advantages of this technique--especially the representation of the dog's spinal column--are discussed with five case histories. This technique allows to produce isolated pictures of special parts of the spinal column which may be superpositioned by other body parts like: atlantooccipitalregion (especially dens axis), caudal cervical spine, thoracical spine and lumbosacral region. Particularly the evaluation of the intervertebral spaces, the small vertebral joints and the contours and structures of the vertebrae is enhanced

OBJECTIVE: The primary driver (PD) of cervical malalignment is important in characterizing cervical deformity (CD) and should be included in fusion to achieve alignment and quality-of-life goals. This study aims to define how PDs improve understanding of the mechanisms of CD and assesses the impact of driver region on realignment/outcomes.
METHODS: Inclusion: radiographic CD, age >18 years, 1 year follow-up. PD apex was classified by spinal region: cervical, cervicothoracic junction (CTJ), thoracic, or spinopelvic by a panel of spine deformity surgeons. Primary analysis evaluated PD groups me

The discovery of a craniocervical junction malformation requires management in three steps: (1) The patterns must be recognized using tomographic measurements (Chamberlain's line, Wackenheim's line). Dynamic flexion-extension studies are necessary to assess stability or instability. Stable patterns range from platybasia to basilar invagination, with gradual deformation, and are frequently associated with Chiari malformation. Unstable patterns characterized by odontoid instability are the equivalent of an odontoid fracture. The origin is malformative (hypoplasia, aplasia of the dens, os odontoi

We report on a 52-year-old man who committed suicide by jumping in a 380 cm long rope with a diameter of 12 mm. The loading forces resulted in complete decapitation. Our biomechanical experiments show that traction-forces of about 12000 Newton lead to complete decapitation irrespectively of the diameter of the used halter. The biomechanical results made it possible to show the possible fall-hights in dependence upon bodymass and distance of negative acceleration "s" graphically. The curves are valid for a constant deceleration during the distance "s", which is determined by lengthening of the

In imaging the atlantoaxial region in injured patients, the initial modality is plain radiography. The lateral C-spine as well as the open-mouth view are essential in this regard. On these views, it is not only important to examine the bony contour but also to look for indirect signs of injury such as prevertebral soft-tissue swelling, air in the prevertebral space, an increased width of the anterior atlantodental interval, and overriding of the C1-C2 joint on one side (the so-called wink sign of atlantoaxial rotatory subluxation). In patients in whom there is a high index of suspicion for occ

A 26-year-old female developed a unilateral hypoglossal nerve palsy in association with tuberculosis of the first cervical vertebra. The extensive soft tissue and bony abnormalities were demonstrated only by computerized axial tomography. Involvement of the hypoglossal nerve within the skull by tuberculous granulation tissue was the most likely mechanism for the nerve palsy.

A 4-month-old female with homozygous achondroplasia and daytime apnea was found to have a small foramen magnum by computerized tomography. Following suboccipital craniectomy and C1-C2 laminectomy respiratory problems did not recur during a 7 month post-operative period. We suggest that the respiratory problems in homozygous achondroplasia may be due to brainstem compression from an abnormally small foramen magnum.

In normal, physiological circumstances there is ample room in the spinal canal to accommodate the spinal cord. Our study aimed to identify the degree of compromise of the spinal canal which could be anticipated in various atlantoaxial pathological states. We examined paired atlas and axis vertebrae using high-definition radiography and simultaneous photography in both normal and simulated pathological orientations in order to measure the resultant dimension of the spinal canal and its percentage occlusion. At the extreme of physiological axial rotation (47 degrees) the spinal canal is reduced

Cranial settling is a condition in which erosive changes of the atlantal lateral masses result in downward telescoping of the atlas onto the axis body, anterior displacement of the atlantal posterior arch, and subsequent ventral and dorsal cervicomedullary compression. Cranial settling is described in conjunction with rheumatoid basilar invagination and atlanto-axial instability, possibly representing the most life-threatening abnormality associated with rheumatoid arthritis. The authors describe a case of symptomatic cranial settling in a nonrheumatoid patient with chronic, severe, spasmodic

Factors such as cause of the trauma, concomitant injuries, and cervical sagittal spinal canal size were compared between survivors and nonsurvivors of cervical spine injuries. Traffic accidents were more commonly the cause of the trauma among nonsurvivors (76%) than among survivors (50%). Concomitant injuries were of a more serious degree among nonsurvivors than in survivors. Cervical sagittal spinal canal size was measured at the atlas level. Patients without any neurologic deficits had significantly (p less than 0.001) wider canals (24.1 mm) than fatalities (20.7 mm). The energy level at the

Involvement of the cervical spine by rheumatoid disease is common, but lateral subluxation at the atlanto-axial level has not been recorded previously. The condition is due to asymmetrical erosion of the lateral atlanto-axial facet joint, and may be complicated by collapse of the lateral mass of the axis. The condition should be suspected in patients with rheumatoid arthritis (RA) who present with occipital, auricular, and/or facial pain.
