[Combination therapy for fetal supraventricular tachycardia with flecainide and digoxin].

Persistent fetal supraventricular tachycardia (SVT) with more than 210 bpm frequently leads to congestive heart failure. We report on a case with SVT and congestive heart failure that converted into sinus rhythm within 19 days of therapy with flecainide and beta-acetyldigoxin. A 32-year-old II gravida I para (25 + 1 weeks of gestation) presented with fetal SVT of 267 bpm. A non-immunologic hydrops fetalis was diagnosed by ultrasound showing ascites, pleural and pericardial effusion and tricuspid regurgitation. Within 19 days of combination therapy with flecainide and digoxin, cardioversion was

[Drug therapy of atrial fibrillation].

Atrial fibrillation can, clinically, be differentiated into several subgroups. Paroxysmal atrial fibrillation lasts less than 24 hours and is often caused by an autonomic dysregulation. Chronic atrial fibrillation occurs in a reversible and an irreversible form. Atrial fibrillation is frequently observed transiently after cardiac surgery. The therapeutic goal must be clearly defined before treatment is initiated. In patients with a reversible form of chronic atrial fibrillation, sinus rhythm can be restored with class Ia, Ic and III antiarrhythmic drugs. These patients with sinus rhythm benefi

Sleep, circadian rhythms, and the pathogenesis of Alzheimer disease.

Disturbances in the sleep-wake cycle and circadian rhythms are common symptoms of Alzheimer Disease (AD), and they have generally been considered as late consequences of the neurodegenerative processes. Recent evidence demonstrates that sleep-wake and circadian disruption often occur early in the course of the disease and may even precede the development of cognitive symptoms. Furthermore, the sleep-wake cycle appears to regulate levels of the pathogenic amyloid-beta peptide in the brain, and manipulating sleep can influence AD-related pathology in mouse models via multiple mechanisms. Finally

Flow of aqueous humor in humans [The Friedenwald Lecture].

Based on clinical experiments with fluorophotometry, several observations can be made about aqueous flow through the chambers of the human eye. 1. The rate of flow is 2.75 +/- 0.63 microliters/min in normal subjects, as derived from measurements averaged during normal office hours. The normal range (95%) is 1.8 to 4.3 microliters/min. 2. There is a circadian rhythm of flow, with the highest rates during morning hours, slightly lower rates during afternoon hours, and rates during sleep that are approximately one half of those during the morning. The hormonal basis for this rhythm is unknown, bu

Rhythmic neural activity is comodulated with short-term gait modifications during first-time use of a dummy prosthesis: a pilot study.

BACKGROUND: After transfemoral amputation, many hours of practice are needed to re-learn walking with a prosthesis. The long adaptation process that consolidates a novel gait pattern seems to depend on cerebellar function for reinforcement of specific gait modifications, but the precise, step-by-step gait modifications (e.g., foot placement) most likely rely on top-down commands from the brainstem and cerebral cortex. The aim of this study was to identify, in able-bodied individuals, the specific modulations of cortical rhythms that accompany short-term gait modifications during first-time use

Lamina-specific cortical dynamics in human visual and sensorimotor cortices.

Distinct anatomical and spectral channels are thought to play specialized roles in the communication within cortical networks. While activity in the alpha and beta frequency range (7 - 40 Hz) is thought to predominantly originate from infragranular cortical layers conveying feedback-related information, activity in the gamma range (>40 Hz) dominates in supragranular layers communicating feedforward signals. We leveraged high precision MEG to test this proposal, directly and non-invasively, in human participants performing visually cued actions. We found that visual alpha mapped onto deep corti

REM sleep and emotional face memory in typically-developing children and children with autism.

Relationship between REM sleep and memory was assessed in 13 neurotypical and 13 children with Autistic Spectrum Disorder (ASD). A neutral/positive/negative face recognition task was administered the evening before (learning and immediate recognition) and the morning after (delayed recognition) sleep. The number of rapid eye movements (REMs), beta and theta EEG activity over the visual areas were measured during REM sleep. Compared to neurotypical children, children with ASD showed more theta activity and longer reaction time (RT) for correct responses in delayed recognition of neutral faces.

The effects of external picoTesla range magnetic fields on the EEG in Parkinson's disease.

We report a 68 year old man with a 7 year history of Parkinson's disease (PD) who obtained little benefit from treatment by dopaminergic and anticholinergic agents. During the six months prior to presentation, he experienced more rapid deterioration in symptoms including memory functions, increasing depression, and dystonia of the foot. External application of picoTesla range magnetic fields (MF) resulted in rapid attenuation of tremor and foot dystonia with improvements in gait, postural reflexes, mood, anxiety, cognitive, and autonomic functions. Plasma prolactin and luteinizing hormone (LH)

Complementary and Synergic Role of Combined Beta-blockers and Ivabradine in Patients with Chronic Heart Failure and Depressed Systolic Function: A New Therapeutic Option?

While substantial advances have been made in the treatment of chronic heart failure (CHF) in the past decade, the prevalence of CHF is increasing. CHF represents a growing financial burden on healthcare systems and, despite therapeutic advances, mortality remains high. There is a need for new therapeutic targets and treatment strategies. Beta-blockers remain the drugs of choice for reducing heart rate (HR) in CHF with reduced ejection fraction (EF), but evidence suggests that their use is suboptimal; a substantial proportion of patients with heart failure do not tolerate the doses of beta-bloc

Common and specific genetic influences on EEG power bands delta, theta, alpha, and beta.

It is difficult to study the genetic basis of psychological function/dysfunction due to its etiological complexity. Instead, we studied a biological marker, EEG power, which is associated with various psychological phenotypes and is closer to gene function. Previous studies have consistently demonstrated high heritability of EEG band power, but less is known about how common or specific genes influence each power band. For 519 adolescent twin pairs, spectral powers were calculated for delta, theta, alpha, and beta bands at bilateral occipital and frontal sites. All four bands were entered into

Cortical modulation through robotic gait training with motor imagery brain-computer interface enhances bladder function in individuals with spinal cord injury.

Neurogenic bladder (NB) dysfunction in individuals with complete spinal cord injury (SCI) is a condition that significantly affects quality of life. Despite the prevalence of interventions, there is a substantial gap in effective treatments for this dysfunction. This study proposes robotic-assisted gait training combined with motor imagery (MI)-based brain-computer interface (BCI) to induce improved cortical modulation, and consequently improve bladder function in patients with SCI. The study involved seven men with complete and chronic SCI in a protocol comprising 24 sessions of robotic-assis

[Heart frequency control in chronic and paroxysmal atrial fibrillation. Critical evaluation of traditional therapy concepts].

BACKGROUND: The loss of the "atrial kick" associated with atrial fibrillation results in a diminished cardiac output. With normal atrial pressures, this latter may decrease initially by 20 to 30%, and an increase in the ventricular rate fails to achieve full compensation, while the irregular rhythm may be felt as bothersome palpitations. RELEVANT STUDIES: The results of all the studies investigating the role of ventricular rate control are inconsistent and contradictory. Despite its widespread use, the value of digitalis remains controversial. Although it is the drug of choice in atrial fibri

The role of parietal beta-band activity in the resolution of visual crowding.

Visual crowding is the difficulty in identifying an object when surrounded by neighbouring flankers, representing a bottleneck for object perception. Crowding arises not only from the activity of visual areas but also from parietal areas and fronto-parietal network activity. Parietal areas would provide the dorsal-to-ventral guidance for object identification and the fronto-parietal network would modulate the attentional resolution. Several studies highlighted the relevance of beta oscillations (15-25 Hz) in these areas for visual crowding and other connatural visual phenomena. In the present

Effects of T-type and L-type calcium currents on synchronized activity patterns in a model subthalamo-pallidal network.

Synchronized rhythmic oscillatory activity in the beta frequency band in the basal ganglia (BG) is a hallmark of Parkinson's disease (PD). Recent experiments and theoretical studies have demonstrated the crucial roles of T-type and L-type calcium currents in shaping the activity patterns of subthalamic nucleus (STN) neurons. However, the role of these currents in the generation of synchronized activity patterns in BG networks involving STN is still unknown. In this study, using an STN model incorporating T-type and L-type calcium currents, we examined how these currents shape the patterns of n

Development of infant sustained attention and its relation to EEG oscillations: an EEG and cortical source analysis study.

The current study examined the relation between infant sustained attention and infant EEG oscillations. Fifty-nine infants were tested at 6 … ed to emerge at 10 months and became well established by 12 months. No difference was found for the beta rhythm between different attention phases. The theta synchronization effect was localized to the orbital frontal, temporal pole, and ventral temporal areas. The alpha desynchronization effect was localized to the brain regions composing the default mode ne …

Microdialysis of melatonin in the rat pineal gland: methodology and pharmacological applications.

The present study describes the development of a new technique to measure melatonin contents in the pineal gland of freely moving rats, by means of on-line microdialysis. The transcerebral cannula was modified, and a sensitive assay of melatonin, using HPLC with fluorimetric detection, was set up. With this system it is possible to monitor the melatonin levels on-line in the pineal gland during day- and nighttime. The nightly increase in melatonin release was recorded. Tetrodotoxin had an inhibitory effect on nighttime levels, whereas even high concentrations did not alter the daytime level. F

[Atrial fibrillation].

Atrial fibrilation is the most frequent arrhythmia, the occurrence increasing with age and associated diseases. The incidence at the age below 60 years is markedly lower than one per cent, whereas in persons above 80 years of age it exceeds six per cent. The occurrence in patients with heart failure is from 10% (NYHA II) up to 50% (NYHA IV). Atrial fibrillation is classified into that observed for the first time and permanent, respectively, while transient forms include paroxyzmal and persistent atrial fibrillation. The diagnosis is based on ECG recording, while echocardiography is most signif
