The amylase level of bile and various cholangiograms was studied in 36 cases of congenital biliary dilatation. (C.B.D.) The amylase level above 10000 U/L was considered to suggest the existence of choledocho-pancreatic long common channel formation. Radiologically, the dilatation of intrahepatic bile duct was seen in 9 out of 19 (47%) Type I cases and in all (100%) Type II cases. The cystic dilatation of intrahepatic bile duct was seen in all Type I cases which have cystic choledochal dilatation. On the other hand, fusiform dilatation was seen in all Type II cases with fusiform choledochal dil

Spontaneous perforation of the bile duct (SPBD) is a rare disease in children. Pancreatico-biliary maljunction (PBM) has been postulated to contribute to its etiology. We have treated three children with SPBD over 30 years, two of whom had PBM. There was one boy and two girls aged 10 months to 2 years with symptoms of abdominal distension, vomiting, abdominal pain, jaundice, and acholic stools. The diagnosis of SPBD was made by paracentesis showing biliary ascites, and primary biliary and intra-abdominal drainage was performed in all cases. The site of perforation was at the connection of the

BACKGROUND: One anastomosis gastric bypass (OAGB) is now the third most common bariatric surgery worldwide. This procedure is garnering increasing attention, but its complication of bile reflux and the associated risk of gastric carcinogenesis remains controversial.
OBJECTIVE: The study aims to assess the impact of bile reflux on the gastric mucosa by comparing pathological and immunohistochemical results of gastric mucosa before and 2 years after OAGB surgery.
METHODS: This retrospective study analyzed gastric lesions observed in gastroscopy before and after OAGB surgery. Pathological exami…

BACKGROUND & AIMS: Bile acid reflux has been implicated as a cause for esophagitis, pulmonary fibrosis, and other aerodigestive disorders. The goal of this study was to determine the relationship between gastric and lung bile acid concentrations and if diet was a modifiable risk factor to reduce bile acid reflux.
METHODS: We prospectively recruited children with oropharyngeal dysphagia and respiratory symptoms undergoing aerodigestive testing. Gastric and, when available, lung bile acid concentrations were quantified using liquid chromatography-mass spectrometry. In a subgroup of enterally fe

Pancreaticobiliary maljunction (PBM) is a congenital malformation in which the pancreatic and bile ducts join anatomically outside the duodenal wall. Because of the excessive length of the common channel in PBM, sphincter action does not directly affect the pancreaticobiliary junction, which allows pancreatic juice to reflux into the biliary tract. According to the results of a nationwide survey, bile duct and gallbladder cancers were found in 6.9 and 13.4 % of adult patients with congenital biliary dilatation, respectively, and in 3.1 and 37.4 % of those with PBM without biliary dilatation, r

Regeneration of canine oesophageal mucosa was studied under basal conditions and in the presence of gastro-oesophageal reflux. In normal circumstances mucosal defects in the oesophagus regenerate by squamous epithelium. In the presence of gastro-oesophageal reflux of either acid or a combination of acid and bile, regeneration was frequently by columnar epithelium (Barrett's oesophagus). This columnar regeneration was not seen with bile reflux alone. By the use of squamous barriers to proximal migration of columnar epithelium in the stomach, it was demonstrated that columnar re-epithelializatio…

BACKGROUND: The endoscopic observation of a bile lake in the stomach, antral gastritis, or ulcerations and the histologic finding of foveolar hyperplasia or chronic gastritis have been implicated as indicators of excessive duodenogastric reflux. The accuracy of these criteria was evaluated in 135 patients with nonspecific symptoms in the foregut suggestive of duodenogastric reflux and no evidence for alcohol- or drug-induced gastric mucosal injury.
METHODS: The presence of excessive duodenogastric reflux was objectively determined by means of both gastric pH monitoring and cholescintigraphy w

The specialised regions of the stomach include the cardia, fundus, body, antrum and the pylorus. Not only do these regions have specific secretory functions, but also, they have specific motor functions as well. For example, the proximal stomach is most important in regulating emptying of liquids and the distal stomach and pylorus, the emptying of solids. Disorders of gastric emptying can be classified into two major categories: mechanical obstruction due to increased resistance and functional obstruction (gastroparesis) due to pump failure. Gastroparesis is best diagnosed using a solid test m

We conducted a literature review to identify elements of duodenogastroesophageal reflux (DGER)--namely pancreatic fluids, hydrochloric acid, pepsin, and bile--as to the effects each has when refluxed to the extraesophageal structures. Further, we wished to acquaint clinicians with the possibilities that, in addition to hydrochloric acid, the other components of DGER are likewise contributing to disease in the extraesophageal areas. Our review included studies that have indicated reflux of the above mentioned components of DGER to the pharynx, larynx, tracheobronchial tree, oral cavity, nasopha

This study assessed the effect of pancreatico-biliary reflux (PBR) as co-factor in the process of chronic cholecystitis by measurement of the levels of active pancreatic enzyme amylase in gallbladder bile and serum of patients undergoing cholecystectomy. Pancreatic Amylase levels in bile from the gallbladder and serum were measured during surgery in 68 patients with chronic calcular cholecystitis subjected to elective open or laparoscopic cholecystectomy in the National Hepatology and Tropical Medicine Research Institution and Theodore Bilharz Research Institute. Bile amylase was detected in 6

BACKGROUND/AIMS: Choledochocele is a rare abnormality of cystic or diverticular dilatation of the terminal intramural portion of the common bile duct. Because its anatomic form does not fit the criteria for pancreaticobiliary maljunction, reflux of bile or pancreatic juice has not been discussed in relation to choledochocele. Considering the flow of bile and pancreatic juice, the authors developed a new classification of choledochocele, and investigated associated pancreaticobiliary diseases.
METHODOLOGY: We reviewed 82 cases of choledochocele, and classified them into three groups on cholang

The papilla of Vater is involved in the regulation of bile flow (inhibition and facilitation) into the duodenum and the prevention of reflux into the bile and pancreatic duct system. These functions are achieved by a special muscular, neural and mucosal structure of the papilla of Vater. The mucosa of the papilla of Vater consists of a complicated system of mucosal folds which can function as an anti-reflux valve or might even participate in the regulation of bile outflow. The smooth muscle of the sphincter of Oddi differs anatomically and embryologically from the surrounding duodenal muscle.

In five anesthetized cats the excretion of Tc-99m-DAIDA into the gallbladder and the small bowel and the d.g. reflux were visualized by a gamma camera and analyzed by a digital computer. A d.g. reflux index was calculated following stimulation of the gallbladder by ceruletide and test meals, before and after a Heineke-Mikulicz pyloroplasty. It could be demonstrated that tracing of bile with TC-99m-DAIDA can be used as a noninvasive method for the evaluation of the d.g. reflux. Gastric intubation increases the d.g. reflux rate. Ceruletide is a much more effective choleretic agent than test meal

Postoperative pneumonia still occurs and is a poor prognostic factor despite advances in minimally invasive gastrectomy. Aspiration is considered a principal mechanism of postoperative pneumonia after gastrectomy. Because the reconstruction method after gastrectomy influences bile reflux, postoperative reflux may contribute to aspiration and thereby increase the risk of pneumonia. Hill classification is an endoscopic grading system used to evaluate the function of the gastroesophageal flap valve and has been reported to reflect the severity of gastroesophageal reflux.

150 years after Beaumont duodenogastric reflux is still under discussion. The cytotoxic effect of bile acids and lysolecithin is proved experimentally, its clinical role, however is called in question. Whereas a pathogenetic role is demonstrated in stress ulcer formation, its role in chronic ulcer is still questionable. Whereas its role in secondary alkaline reflux oesophagitis can be shown clinically and experimentally, the function of duodenogastric reflux in primary reflux oesophagitis still remains to be elucidated. The question of duodenogastric reflux in the operated stomach is even more

Laryngeal granuloma is a benign neoplasm located on the vocal processes of the arytenoid cartilage. The causes of development may be prolonged or traumatic intubation (23%), gastroesophageal reflux disease leading to chronic cough (30%), excessive vocal load (33%); respectively, post-intubation, contact and idiopathic granulomas are distinguished. In laryngopharyngeal reflux, an extraesophageal manifestation of gastroesophageal reflux disease, morphological changes in the mucous membrane of the larynx and pharynx occur as a result of direct exposure to acidic stomach contents, pepsin, bile and
