Ilizarov external fixation in the management of bilateral, highly complex blast injuries of lower extremities: a report of two cases.

Because of their complexity, war injuries inflicted by a blast mechanism often require tailoring of treatment to attain a more individualized solution. We report two cases of bilateral, severely mangled lower limbs with open tibial fractures and crush injuries to the feet. In each case, one limb had to be amputated below the knee, but the other limb was saved by immediate stabilization in a tubular external fixation frame crossing the knee; the frame was later replaced by a hybrid ring fixation frame with a freely moving knee. Such incidences are rare, and the particular management detailed he

Diagnostic and Prognostic Significance of the S100 Biomarker in Blast-related Closed Traumatic Brain Injury.

INTRODUCTION: Traumatic Brain Injury (TBI) is a frequent cause of long‑term disability in service members who have been exposed to blast, and clinicians often lack reliable tools to quantify the actual burden of damage early after injury. Among available candidates, S100B is a calcium‑binding protein of astroglial origin that appears in the bloodstream when the blood-brain barrier is disrupted, and neuronal structures are injured. Its diagnostic and prognostic properties in the specific setting of blast‑related closed TBI, however, remain incompletely defined. The present study, therefore, foc

C1q dependency of retinal ganglion cell death in blast-mediated traumatic brain injury.

Traumatic brain injury is a widespread health concern that annually affects millions of people worldwide, with blast-mediated TBI (bTBI) being of particular relevance to military personnel and civilians in war zones. Current treatments primarily target symptoms, rather than the underlying neuropathology, and are ultimately limited by the need for an improved etiological understanding of TBI. Here, we utilized a pre-clinical mouse model of bTBI, previously shown to evoke a high susceptibility to retinal ganglion cell (RGC) death, to investigate whether neuroinflammatory pathways associated with

Posterior fossa penetrating traumatic brain injury from military blast: illustrative case.

BACKGROUND: Posterior fossa penetrating traumatic brain injury (pTBI) in military combat is rare but carries high neurovascular risk. OBSERVATIONS: A 51-year-old male sustained a mine explosion injury in Ukraine, in which a 4 × 1.5-cm metal fragment penetrated through the skull base into the posterior cranial fossa. The fragment was adjacent to the vertebral artery, sigmoid sinus, lower cranial nerves, and occipital condyle. Digital subtraction angiography was completed on arrival, which showed a patent right vertebral artery, occluded transverse-sigmoid sinus, and partial internal jugular ve

A Ferret Model of Blast-Induced Traumatic Brain Injury with Biochemical and Neurobehavioral Outcome Measures.

Although blast-induced traumatic brain injury (bTBI) is considered as the signature injury of recent combat operations such as Operations Iraqi Freedom and Enduring Freedom, no precise biomechanical and biological mechanisms of injury have been identified. Consequently, to date, there are no FDA-approved countermeasures for the treatment of bTBI. Animal models that are highly translatable to humans are required for studying the injury mechanisms underlying bTBI. As a small animal with a gyrencephalic cerebral cortex, the ferret has been increasingly used for studying the mechanisms of neurolog

Management and Surgical Dressing Protocol for Cutaneous Mucormycosis After a Blast Injury: A Case Report.

BACKGROUND Mucormycosis is known for being an aggressive fungal infection that is associated with high morbidity and mortality. Due to its nonspecific presentation, diagnosis is challenging, and a high level of suspicion is required to optimize patient outcomes. This report discusses a case of mucormycosis infection in a bomb blast victim who was identified early, treated promptly, and was eventually discharged 1 month after admission. CASE REPORT We report the case of a 40-year-old male patient who sustained a bomb blast injury. He arrived at a tertiary hospital 3 days after the injury, in pa

Effectiveness of Body Armor Against Shock Waves: Preventing Blast Injury in a Confined Space.

Introduction Blast injuries in modern society often occur owing to terrorist attacks in confined spaces, particularly in urban settings, indoors, and in vehicles, leading to significant damage. Therefore, it is important to focus on blast injuries in confined spaces rather than in conventional open-field experiments. Materials and methods We used an air-driven shock wave generator (blast tube) established indoors in 2017 and conducted basic research to potentially save the lives of patients with blast injuries. Under general anesthesia, pigs were divided into with body armor (BA) and without B…

(2R,6R)-Hydroxynorketamine Treatment of Rats Exposed to Repetitive Low-Level Blast Injury.

Many military veterans who experienced blast-related traumatic brain injuries (TBIs) in the conflicts in Iraq and Afghanistan suffer from chronic cognitive and mental health problems, including post-traumatic stress disorder (PTSD). Male rats subjected to repetitive low-level blast exposure develop chronic cognitive and PTSD-related traits that develop in a delayed manner. Ketamine has received attention as a treatment for refractory depression and PTSD. (2R,6R)-hydroxynorketamine [(2R,6R)-HNK] is a ketamine metabolite that exerts rapid antidepressant actions. (2R,6R)-HNK has become of clinica

Mechanism of the traumatic brain injury induced by blast wave using the energy assessment method.

Although blast-induced traumatic brain injury (b-TBI) is well recognized for its significance in the military population, the unique mechanisms of primary b-TBI remain undefined. The aim of the present study is to unveil the mechanism of the traumatic brain injury induced by the shock wave using the energy assessment method. First, the magnetic resonance images of the human head were processed to establish the finite element (FE) model of the human head including the skin, the skull, the brain and other structures. The simulation of the shock wave was implemented using the Coupled Eulerian-Lag

A novel animal model of primary blast lung injury and its pathological changes in mice.

BACKGROUND: Primary blast lung injury (PBLI) is a major cause of death in military conflict and terrorist attacks on civilian populations. However, the mechanisms of PBLI are not well understood, and a standardized animal model is urgently needed. This study aimed to establish an animal model of PBLI for laboratory study. METHODS: The animal model of PBLI was established using a self-made mini shock tube simulation device. In brief, mice were randomly divided into two groups: the control group and the model group, the model group were suffered 0.5 bar shock pressures. Mice were sacrificed at

Chronicity of repeated blast traumatic brain injury associated increase in oxycodone seeking in rats.

Numerous epidemiological studies have found co-morbidity between non-severe traumatic brain injury (TBI) and substance misuse in both civilian and military populations. Preclinical studies have also identified this relationship for some misused substances. We have previously demonstrated that repeated blast traumatic brain injury (rbTBI) increased oxycodone seeking without increasing oxycodone self-administration, suggesting that the neurological sequelae of traumatic brain injury can elevate opioid misuse liability. Here, we determined the chronicity of this effect by testing different durati

Blast mild traumatic brain injury is associated with increased myopia and chronic convergence insufficiency.

While chronic visual symptom complaints are common among Veterans with a history of mild traumatic brain injury (mTBI), research is still ongoing to characterize the pattern of visual deficits that is most strongly associated with mTBI and specifically, the impact of blast-related mTBI on visual functioning. One area that has not been well explored is the potential impact of blast mTBI on refractive error. While myopic shifts have been documented following head injuries in civilian populations, posttraumatic myopic shifts have not been explored in participants with military mTBI. This study in

The Neurobehavioral Effects of Buprenorphine and Meloxicam on a Blast-Induced Traumatic Brain Injury Model in the Rat.

Previous findings have indicated that pain relieving medications such as opioids and non-steroidal anti-inflammatory drugs (NSAIDs) may be neuroprotective after traumatic brain injury in rodents, but only limited studies have been performed in a blast-induced traumatic brain injury (bTBI) model. In addition, many pre-clinical TBI studies performed in rodents did not use analgesics due to the possibility of neuroprotection or other changes in cognitive, behavioral, and pathology outcomes. To examine this in a pre-clinical setting, we examined the neurobehavioral changes in rats given a single p

Oesophageal rupture from a pneumatic blast injury: an unusual mechanism of blunt oesophageal trauma.

Oesophageal perforation is a serious condition associated with significant morbidity and mortality. Clinical suspicion of oesophageal injury in patients sustaining neck or torso trauma is essential as early diagnosis and management are associated with better outcomes. Oesophageal perforation resulting from blunt trauma is uncommon, and traumatic oesophageal perforation following blast injury is exceedingly rare. We present two cases of patients developing oesophageal perforation from this rare mechanism, review basic principles of management, and key learning points.

Activation of the Nrf2-ARE signal pathway after blast induced traumatic brain injury in mice.

Background: Treatment of blast-induced traumatic brain injury (bTBI) has been hindered. Previous studies have demonstrated that oxidative stress may contribute to the pathophysiological process. The nuclear factor erythroid 2-related factor 2 (Nrf2)-antioxidant response element (ARE) signaling pathway exhibits a protective effect after traumatic brain injury (TBI). This study explored whether the Nrf2-ARE pathway was activated in a modified bTBI mouse model. Method: Mice were randomly divided into six groups: the 6 h, 1 d, 3 d, 7 d and 14 d after bTBI groups and a sham group. The protein level

5-Year imaging sequelae of concussive blast injury and relation to early clinical outcome.

Current imaging diagnostic techniques are often insensitive to the underlying pathological changes following mild traumatic brain injury (TBI) or concussion so much so that the explicit definition of these uncomplicated mild brain injuries includes the absence of radiological findings. In the US military, this is complicated by the natural tendency of service members to down play symptoms for fear of removal from their unit particularly in combat making it challenging for clinicians to definitively diagnose and determine course of treatment. Questions remain regarding the long-term impact of t

Symptom Trajectories After Military Blast Exposure and the Influence of Mild Traumatic Brain Injury.

BACKGROUND: Blast-related mild traumatic brain injury (blast mTBI) is a signature wound of recent US military conflicts in the Middle East, but the relatedness of postconcussive symptoms (PCS) to the blast mTBI is unclear, and longitudinal symptom data are sparse. OBJECTIVES: To characterize postdeployment symptom levels and trajectories and to determine relationship to blast mTBI. METHODS: A total of 216 participants within 2 years of blast exposure during deployment underwent structured interviews or algorithmic questionnaires for blast mTBI. Detailed symptom inventories for PCS, pain, pos

Internal Jugular Vein Compression: A Novel Approach to Mitigate Blast Induced Hearing Injury.

HYPOTHESIS: Internal jugular vein (IJV) compression before blast injury will lead to reduced risk of traumatic hearing injury following exposure to a blast injury. BACKGROUND: IJV compression and its effects on not only intracranial, but also intracochlear pressure may potentiate blast induced hearing injury, therefore, precluding its use as a prophylactic therapy for blast induced traumatic brain injury. METHODS: Twenty Sprague Dawley rats were exposed to a 17.9 ± 0.4 PSI (195.8 dB SPL) right sided shock wave in which 10 had application of a custom IJV compression collar before injury. All

Pathophysiology of the inner ear after blast injury caused by laser-induced shock wave.

The ear is the organ that is most sensitive to blast overpressure, and ear damage is most frequently seen after blast exposure. Blast overpressure to the ear results in sensorineural hearing loss, which is untreatable and is often associated with a decline in the quality of life. In this study, we used a rat model to demonstrate the pathophysiological and structural changes in the inner ear that replicate pure sensorineural hearing loss associated with blast injury using laser-induced shock wave (LISW) without any conductive hearing loss. Our results indicate that threshold elevation of the au
