Blepharospasm and hemifacial spasm are the two most common craniofacial movement disorders. Blepharospasm is a syndrome characterized by excessive or continuous eye closure related to overactivity of the orbicularis oculi and adjacent muscles bilaterally. PURPOSE: To investigate the response to supramaximal doses of botulinum toxin in patients with refractory blepharospasm.
DESIGN: Prospective, nonrandomized, open-label interventional case series.
PARTICIPANTS: Eight consecutive patients with blepharospasm requiring injections every 2 months despite receiving 100 U of botulinum toxin per session.

Abnormalities in dopamine neurotransmission are thought to underlie the generation of dystonic movements. The authors performed a case-control allelic association study in patients with the focal dystonia blepharospasm, using polymorphisms in the dopamine receptor and transporter genes. Allele 2 of a DRD5 dinucleotide repeat was significantly associated with blepharospasm. This may indicate a pathogenic role for this receptor.

The clinical effects of botulinum toxin A were evaluated in eight patients with idiopathic blepharospasm and hemifacial spasm. A substantial reduction or disappearance of muscle spasms were observed after the treatment. The action of botulinum toxin became noticeable 2 days after administration with a peak effect at 2 weeks. This effect diminished with muscle spasms returning about 12 weeks after administration. No serious adverse events were noted. Benign essential blepharospasm is a focal dystonia consisting in involuntary closure of the eyelids. Until early 80's, therapeutic modalities included only psychotropic drugs, biofeedback and surgery, which showed limited efficiency. Recently, it has been suggested to inject botulinum toxin in affected palpebral orbicularis muscles. The effects of botulinum A toxin injections for the treatment of facial spasm were analyzed for 22 patients. Sixteen patients had unoperated on essential blepharospasm, three had essential blepharospasm with residual spasm following previous surgical treatment, and three had unoperated on hemifacial spasm. Motion pictures of 15 patients with essential blepharospasm were studied. Previously unrecognized signs indicated multiple cranial nerve involvement. These signs include impersistence of gaze, lid retraction, tongue thrust, head tilts, head jerks, vertical gaze spasms, and asymmetry.
