Liver diseases result in a re-balanced state of the haemostatic system with decreased haemostatic reserves. Increased fibrinolytic activity is commonly seen during liver transplants. The aim of this study was to assess whether ClotPro's ECA-test is able to detect hyperfibrinolysis earlier and with higher frequency than ClotPro's conventional viscoelastic assays for the intrinsic and the extrinsic coagulation pathway. From 25 liver transplant recipients, systemic blood samples were collected during surgery. Viscoelastic haemostatic assays with ClotPro's IN-test, EX-test and ECA-test were perfor

Amniotic fluid embolism is frequently associated with coagulopathy. However, the exact nature and evolution of the bleeding disorder is incompletely understood. We report a case of clinically diagnosed amniotic fluid embolism associated with major haemorrhage and coagulopathy. We measured sequential levels of all individual clotting factors, thrombin generation, fibrinogen, and D-dimer levels over the course of the event, beginning shortly after the patient's initial collapse and during the subsequent resuscitation, to identify the specific abnormalities of coagulation from stored blood sample

OBJECTIVE: The aim of this study was to determine the prevalence of acute traumatic coagulopathy (ATC) and identify associated clinical and laboratory parameters including rotational thromboelastometry.
STUDY DESIGN: Dogs presenting within 6 hours after trauma were allocated to the ATC or non-ATC group based on thromboelastometry analysis (ex-tem S, in-tem S, fib-tem S). ATC was defined as ≥2 hypocoagulable parameters in 1 profile and ≥ 1 hypocoagulable parameter in an additional profile. Parameters used were ex-tem and in-tem clotting time (CT), clot formation time (CFT), maximum clot firmne

Genetic bleeding disorders can have a profound impact on a horse's health and athletic career. As such, it is important to understand the mechanisms of these diseases and how they are diagnosed. These diseases include haemophilia A, von Willebrand disease, prekallikrein deficiency, Glanzmann's Thrombasthenia and Atypical Equine Thrombasthenia. Exercise-induced pulmonary haemorrhage also has a proposed genetic component. Genetic mutations have been identified for haemophilia A and Glanzmann's Thrombasthenia in the horse. Mutations are known for von Willebrand disease and prekallikrein deficienc

Molecular genetic tools are widely applied in inherited bleeding disorders. New genes involved in haemorrhagic disorders have been identified by genome wide linkage analysis on families with a specific phenotype. LMNA1 or MCFD in combined FV/FVIII-deficiency and VKORC1 in vitamin K coagulation factor deficiency type 2 are two examples. Identification of the causative gene mutation has become standard for most bleeding disorders. Knowledge of the causative mutation allows genetic counselling in affected families and most importantly adds to the pathophysiological understanding of phenotypes. Ha

The coagulation system is governed by a subtle balance between clotting activators and inhibitors. Many genes can contribute to the overall phenotype, and polymorphisms may act to up regulate or down regulate the generation of thrombin, the coagulation-key enzyme. An increase in coagulation factor (gain function) or/and a decrease in coagulation inhibitors (loss of function) may favor venous thromboembolism (VTE). It has been observed since a long time that VTE may be a familial disease, but it was only in 1965 that Egeberg published the first case of inherited antithrombin (AT) deficiency. Th

The study of three adults with severe bleeding due to the presence of acquired anticoagulants that inhibit procoagulant action of Factor VIII and to a lesser degree of factors IX and XI, is described. In one case, this phenomenon occurred during the course of a rheumatoid arthritis, in one case coincided with a megaloblastic anemia (pernicious anemia) and the last case did not have any coexisting disease. Corticosteroids were useless, intravenous cyclophosphamide improved one patient and given orally failed in other patient. The circulating inhibitor disappeared in the patient successfully tre

Less common than acquired bleeding disorders in the perinatal period, congenital deficiencies of the clotting proteins, particularly the hemophilias A and B, can be just as devastating. Detection of these disorders may be delayed because a suspect family history is lacking in up to one third of the cases of hemophilia, and parents are usually asymptomatic in the other congenital factor deficiencies. Physiologically low levels of the liver-dependent and contact factors can make the interpretation of the common hemostasis screens difficult. Hence, the use of age-adjusted tables such as those pre

Legg-Calve-Perthes' (LCP) disease is a noninflammatory aseptic necrosis of the femoral head and neck in small-breed dogs. The etiology of the disease is not known, but ischemia resulting from vascular compression or occlusion has been proposed. A latent ischemic phase during development of the femoral epiphysis seems to be responsible for the onset of the typical clinical features of LCP disease. Ischemia might result from insufficient oxygen supply either caused by a reduced number of afferent arterial vessels or an occlusion of the efferent venous vessels by thrombosis. In humans, LCP diseas

Platelets play an important role regarding coagulation by contributing to thrombus formation by platelet adhesion, aggregation, and α-/δ-granule secretion. Inherited platelet disorders (IPDs) are a very heterogeneous group of disorders that are phenotypically and biochemically diverse. Platelet dysfunction (thrombocytopathy) can be accompanied by a reduction in the number of thrombocytes (thrombocytopenia). The extent of the bleeding tendency can vary greatly. Symptoms comprise mucocutaneous bleeding (petechiae, gastrointestinal bleeding and/or menorrhagia, epistaxis) and increased hematoma te

PURPOSE OF REVIEW: Severe bleeding and coagulopathy remain major challenges in perioperative care, particularly in cardiac surgery, trauma, and obstetrics. Viscoelastic hemostatic assays (VHAs) have emerged as rapid, point-of-care tools to assess coagulation, but their evidence base and clinical integration remain inconsistent. This review summarizes current evidence, highlights assay limitations, and outlines opportunities for refinement.
RECENT FINDINGS: VHAs outperform conventional plasma-based tests in turnaround time and functional assessment of coagulation. International guidelines incr

Blood coagulation screening profiles were performed in 512 patients who underwent open-heart surgery with extracorporeal circulation. Severe coagulation disorders were found in 29 (5.6 per cent) patients. The most common abnormalities were low one-stage prothrombin time (PT) activities and impaired whole blood clot retractions. In the majority of patients the discrepancy between low PT's and normal or only slightly depressed factor II, V, VII, and X activities was explained by the presence of an inhibitor of the extrinsic system. Eight patients demonstrated the heparin rebound phenomenon but o

Pregnancy induces significant changes in the coagulation and fibrinolytic systems, creating a physiologic hypercoagulable state. While these adaptations protect against hemorrhage at delivery, they pose complex challenges in patients with underlying coagulation disorders. This review summarizes hemostasis in pregnancy, common inherited and acquired bleeding disorders, and the anesthetic considerations essential for safe peripartum management.

Patients with clinically significant bleeding, but with normal hemostatic investigations are identified as a distinct group known as "bleeding disorders of unknown cause" (BDUC). Though the clinical symptoms often resemble that of mild bleeding disorders (MBDs) with confirmed laboratory diagnosis, these patients have to go through multiple laboratory investigations only to end up in a diagnosis of exclusion i.e. BDUC. There are no standard protocols or general consensus on treatment of these patients, either for a generalized bleeding or prophylaxis prior to surgical procedures; patients are t
