The Future of Cardioprotection-Pointing Toward Patients at Elevated Risk as the Target Populations.

Translation of the cardioprotective effect by pharmacological and mechanical conditioning therapies into improvement of clinical outcome for the patients has been disappointing. Confounding factors like comorbidity and comedications may explain some of the loss in translation. However, the substantial improvement of outcome in disease states involving ischemia-reperfusion injury, that is, planned cardiac surgery, elective percutaneous coronary intervention, and even primary percutaneous coronary intervention for ST-segment myocardial infarction (STEMI), is the most plausible explanation for th

Maternal-fetal interfaces transcriptome changes associated with placental insufficiency and a novel gene therapy intervention.

The etiology of fetal growth restriction (FGR) is multifactorial, although many cases often involve placental insufficiency. Placental insufficiency is associated with inadequate trophoblast invasion, resulting in high resistance to blood flow, decreased availability of nutrients, and increased hypoxia. We have developed a nonviral, polymer-based nanoparticle that facilitates delivery and transient gene expression of human insulin-like 1 growth factor (hIGF1) in placental trophoblast for the treatment of placenta insufficiency and FGR. Using the established guinea pig maternal nutrient restric

Hyperventilation therapy for acute traumatic brain injury.

BACKGROUND: Because hyperventilation is often associated with a rapid fall in intracranial pressure, it has been assumed to be effective in the treatment of severe head injury. Hyperventilation reduces raised intracranial pressure by causing cerebral vasoconstriction and a reduction in cerebral blood flow. Whether reduced cerebral blood flow improves neurological outcome however, is unclear. OBJECTIVES: To quantify the effect of hyperventilation on death and neurological disability following head injury. SEARCH STRATEGY: The search strategy drew on that of the Injuries Group as a whole. The

A taxonomic review of the freshwater shrimp genus Caridina H. Milne Edwards, 1837 (Decapoda, Atyidae) from southern Thailand, with descriptions of five new species.

AIMS: IgA nephropathy (IgAN) is traditionally considered a polytypic disorder, although λ light chain predominance on immunofluorescence (IF) is common. In some cases, monotypic IgA (mIgA) staining is identified. Whether mIgA represents a variant of IgAN or should be classified as proliferative glomerulonephritis with monoclonal immune deposits (PGNMID) remains controversial, particularly in cases with isolated κ deposits or membranoproliferative (MP) or membranous patterns. We performed this study to characterize all mIgA cases in our institutional biopsy database, with emphasis on hematologi

Effectiveness of blood flow-restricted slow walking on mobility in severe multiple sclerosis: A pilot randomized trial.

OBJECTIVE: We tested the safety, feasibility, and effectiveness of blood flow restriction-empowered low-intensity interval walking exercise (BFR-W) compared with conventional intensive overground walking (CON-W) at improving gait speed and functional capacity in patients with multiple sclerosis (MS) and severe gait disabilities. METHODS: 24 patients (58 ± 5 years; 7 males) with progressive MS (Expanded Disability Status Scale 5.5 - 6.5) were randomized to receive 12 rehabilitation sessions over 6 weeks. The BFR-W group (n = 12) performed interval walking (speed paced by a metronome that incre

Local relationships between restricted water diffusion and oxygen consumption in the ischemic human brain.

BACKGROUND AND PURPOSE: MR is widely used to depict still ischemic but viable tissue in acute stroke. However, the relationship between the apparent diffusion coefficient (ADC) and energy failure from reduced oxygen supply are unknown in man. METHODS: Acute carotid-territory stroke patients were studied prospectively with both diffusion tensor-imaging and back-to-back steady-state 15O-PET. Substantial numbers of voxels with oxygen extraction fraction >0.70 (ie, significant ongoing hypoxia) were identified in 3 patients (imaged at 7, 16 and 21 hours after stroke onset). In this voxel populatio

Preoperative fluid and electrolyte management with oral rehydration therapy.

PURPOSE: We hypothesized that oral rehydration therapy using an oral rehydration solution may be effective for preoperative fluid and electrolyte management in surgical patients before the induction of general anesthesia, and we investigated the safety and effectiveness of oral rehydration therapy as compared with intravenous therapy. METHODS: Fifty female patients who underwent breast surgery were randomly allocated to two groups. Before entry to the operation room and the induction of general anesthesia, 25 patients drank 1000 ml of an oral rehydration solution ("oral group") and 25 patient

Spatial Constraints Limit Natural Killer Cell Access to Germinal Centre HIV Reservoirs in Lymph Nodes.

BACKGROUND: Antiretroviral therapy effectively suppresses HIV-1 replication in blood but fails to eradicate viral reservoirs in lymphoid tissues, particularly in germinal centres (GCs) of lymph nodes (LNs). While CD8+ T cells are largely excluded from GCs, the role of natural killer (NK) cells, potential alternative effectors, remains poorly defined in human LNs during HIV infection. We investigated NK cell phenotype, function, and localization in paired LN and peripheral blood samples from South African women living with HIV (WLWH) and HIV-negative controls. METHODS: Using immunofluorescence

Functional rehabilitation strategies for enhancing neuromuscular control after ACL reconstruction: A systematic review.

BACKGROUND: Arthrogenic muscle inhibition (AMI) and altered neuromuscular control commonly arise after anterior cruciate ligament (ACL) injury and may persist both preoperatively and after anterior cruciate ligament reconstruction (ACLR), potentially impairing functional recovery. OBJECTIVE: To synthesise randomised controlled trial (RCT) evidence on functional rehabilitation interventions targeting neuromuscular control after ACLR, using electromyography (EMG)-derived outcomes. METHODS: PubMed, Cochrane Library, Embase, CINAHL, Web of Science, and SPORTDiscus were searched from January 1, 2

Arterial Spin Labeling Magnetic Resonance Imaging to Diagnose Contrast-Induced Vasospasm After Intracranial Stent Embolization.

BACKGROUND: Contrast-induced encephalopathy (CIE) is a rare and misdiagnosed complication of intravascular injection of contrast, responsible for arterial vasospasm and neurologic effects. Conventional radiologic findings are not in themselves specific for cerebral vasospasm diagnosis. We present a case in which arterial spin labeling (ASL) perfusion magnetic resonance imaging (MRI) was useful in early diagnosis of CIE. CASE DESCRIPTION: A 56-year-old woman was admitted for elective flow-diverter embolization of a recanalized left supra-ophthalmic internal carotid artery aneurysm; at 4 hours

Albumin reprograms the B cell transcriptional landscape and improves neutrophil antimicrobial function in patients with decompensated cirrhosis.

BACKGROUND & AIMS: Patients with acutely decompensated (AD) cirrhosis are immunocompromised and particularly susceptible to infections. This study investigated the immunomodulatory actions of albumin by which this protein may lower the incidence of infections. METHODS: Blood immunophenotyping was performed in 11 patients with AD cirrhosis and 10 healthy volunteers (HV). Bulk and single-cell RNA sequencing (scRNA-seq) and flow cytometry were performed in peripheral blood mononuclear cells (PBMCs) from 20 patients with AD cirrhosis and 34 HV exposed to albumin. Albumin's effects on degranulatio

[The use of the osteopathic correction for the combined treatment and rehabilitation of the patients presenting with the vertebral artery syndrome].

BACKGROUND: The pathological changes in the cervical spine frequently result in the disturbances of the blood supply in the vertebral-basiliar system. The degenerative-dystrophic processes in the cervical spine occupy the leading place in the pathogenesis of the vertebral artery syndrome (VAS) causing disorders of the microcirculation and innervation, as well as the restriction of mobility and the development of somatic dysfunctions. It can be assumed that the manual methods applied to manage this condition can be effective in the removal the functional abnormalities in the blood flow. AIMS:

Delayed selective cerebral hypothermia decreases infarct volume after reperfused stroke in baboons.

BACKGROUND: Hypothermia is known to provide neuroprotection from focal ischemia. However, lethal cardiovascular complications resulting from total body cooling have greatly restricted hypothermia as a therapy for stroke. This study determined whether selective cerebral cooling induced after reversible cerebral artery occlusion would decrease the infarct volume. METHODS: Under general anesthesia, 8 baboons were subjected to 1-hour simultaneous occlusion of the left internal carotid artery and anterior cerebral arteries by transorbital surgical approach. Four animals were treated with selective

Nanoparticle-mediated delivery of placental gene therapy via uterine artery catheterization in a pregnant rhesus macaque.

Nanoparticles offer promise as a mechanism to non-invasively deliver targeted placental therapeutics. Our previous studies utilizing intraplacental administration demonstrate efficient nanoparticle uptake into placental trophoblast cells and overexpression of human IGF1 (hIGF1). Nanoparticle-mediated placental overexpression of hIGF1 in small animal models of placental insufficiency and fetal growth restriction improved nutrient transport and restored fetal growth. The objective of this pilot study was to extend these studies to the pregnant nonhuman primate and develop a method for local deli

MCP-1-CCR2-M2 macrophages axis contributes to diffuse large B-cell lymphoma progression and inhibits antitumor immune response.

Diffuse large B-cell lymphoma (DLBCL) is an aggressive hematological malignancy with restricted effective therapy choices. Neither the recruitment of monocytes nor the functioning of the physiological mechanisms of macrophage polarization can be achieved without the involvement of the MCP-1/CCR2 axis. We investigated the feasibility of treatment targeting MCP-1-CCR2-macrophages axis in DLBCL. MCP-1, CD68, CD163 expression was analyzed by immunohistochemistry in 143 DLBCL patients tissues, and MCP-1 concentration and CD14 + CCR2 + monocytes in peripheral blood were analyzed in another cohort by

Unique cell surface expression of receptor tyrosine kinase ROR1 in human B-cell chronic lymphocytic leukemia.

PURPOSE: Gene expression profiling identified receptor tyrosine kinase ROR1, an embryonic protein involved in organogenesis, as a signature gene in B-cell chronic lymphocytic leukemia (B-CLL). To assess the suitability of ROR1 as a cell surface antigen for targeted therapy of B-CLL, we carried out a comprehensive analysis of ROR1 protein expression. EXPERIMENTAL DESIGN: Peripheral blood mononuclear cells, sera, and other adult tissues from B-CLL patients and healthy donors were analyzed qualitatively and quantitatively for ROR1 protein expression by flow cytometry, cell surface biotinylation,
