OBJECTIVES: In this study, we investigated the clinical feasibility of using umbilical cord blood as an alternative to neonatal blood for measuring serum albumin and immunoglobulin G (IgG) levels in newborns, including preterm newborns.

In Switzerland the amount of blood donations is determined by the demands for plasma; as a result there is a surplus of erythrocyte concentrates for which more uses should be found. Trials in a regional general hospital proved that these concentrates can be advantageously employed in the field of surgery and anasthesia: due to close cooperation between the clinical department and the blood donor sevice the demand for erythrocyte concentrates rose from 5 per cent to 80 per cent within 5 years.

Recently we experienced one incident that the patient with A blood type reacted as AB type on macroscopic observation when the blood type was tested immediately after the infusion with Dextran70 in saline. The patient was confirmed as having the A type by reexamination later. Microcirculation was studied in experiments on rats whose blood had been completely replaced by perfluorotributylamin emulsion. After blood replacement the microcirculatory channel was actively perfused with the blood substitute. It was shown that the emulsion circulation rate exceeded the initial rate of the blood flow, that high level of nutritive vessels perfusion was maintained and the physiological parameters of reactivity and functional motility of microvessels remained unchanged. It has been shown in clinical experiments that plasma substitutes generally have a specific-colloidal influence on special parts of the clotting system. These changes are predominant after infusion of dextran, since on one hand there is an inhibition of platelet function, and on the other hand a decrease in the activity of plasmatic clotting factors.
