Evaluation of newborns with skeletal dysplasias.

Skeletal dysplasia are a relatively common and distinct group of genetic disorders. Even though abnormalities of bone growth are the major clinical manifestations, the pathological process may involve in any body system. Many of these disorders have dire consequences such as neonatal death and most will have life-long physical and psychosocial morbidity associated with them. Recent advances in genetic research have identified the genes underlying the primary defects in several common skeletal dysplasias such as achondroplasia and diastrophic dysplasia. While these advances are clearly importan

Primary hyperparathyroidism in a 14 year old girl presenting with bone deformities.

A 14 year old girl with bilateral genu valgum of 6 years duration was brought for evaluation of primary hyperparathyroidism. She had clinical features of rickets such as frontal bossing, rachitic rosary, lumbar lordosis and fixed adduction deformity of the left leg. She had undergone osteotomy earlier for correction of these deformities without benefit. Laboratory investigations showed hypercalcaemia and hypophosphataemia, with elevated alkaline phosphatase and parathyroid hormone levels. A skeletal survey showed generalized decreased density of bone and brown tumours and, coincidentally, a ri

Sponastrime dysplasia: report on two siblings with metal retardation.

We report on two sisters in the third family affected with Sponastrime dysplasia. The clinical and radiological features are presented. The two patients showed severe mental retardation. The pattern of inheritance of this rare dysplasia is discussed.

Metaphyseal anadysplasia: a metaphyseal dysplasia of early onset with radiological regression and benign course.

We report on 4 boys (including 2 maternally related first cousins) with a metaphyseal dysplasia of early onset and regressive evolution. Diagnosis is possible in the first months. Distal metaphyses of long bones are very irregular. Femoral necks seem hypoplastic and the edges of the metaphyses are almost vertical; femoral shaft is bowed. Those anomalies disappear after 2 years. The main manifestations are slight shortness and a light varus deformity of the lower limbs. Stature is not affected. The upper tibial growth cartilage, studied in one case, showed wide proliferative and hypertrophic zo

[Kniest's disease].

A one year old child with Kniest's disease was admitted for reconstructive surgery of the bone deficit of a palatine cleft. This syndrome is recognizable at a very early age by the shortness of the limbs or the increase in size of the joints. Clinically it associates mild facial dystrophy and nanism from reduction in height of the trunk and the shortness and deviations of the limbs. Frequently associated are myopia, deafness and a palatine cleft. Radiological signs are reduced height of vertebral bodies, and a not infrequent finding is a cuneiform deformity of the first lumbar vertebrae. Epiph

Inhibition of partial closure of epiphyseal plate in rabbits by indomethacin.

The efficiency of indomethacin (10 mg/kg/day) in inhibiting recurrent partial closure of the epiphyseal plate was tested in rabbits. An epiphysiodesis was done laterally in the distal left femoral epiphyseal plate in 10 adolescent rabbits. This produced a valgus deformity in 14 of them. The bone bridge was then removed operatively and the rabbits were treated with either indomethacin or vehicle for 21 days postoperatively. Indomethacin plasma levels were about 180 ng/ml. The valgus deformity improved in indomethacin-treated rabbits, whereas it became worse in the controls (P = 0.004).

Parastremmatic dwarfism.

A girl aged ten, of Cape Coloured stock, with typical features of parastremmatic dwarfism has been investigated. The clinical manifestations included disproportionate dwarfism, limb deformity, a short stiff neck, and marked thoracic kyphosis. The radiographic changes were dramatic, the skeleton having a "flocky" appearance due to patches of radiolucency in an irregular lattice of sclerosis. The metaphyses and epiphyses of the long bones were grossly expanded, and the vertebrae were flattened and distorted. The clinical and radiographic features of five other previously reported individuals wit

Secreted BMP antagonists and their role in cancer and bone metastases.

Bone morphogenetic proteins (BMPs) are multifunctional secreted cytokines that act in a highly context-dependent manner. BMP action extends beyond the induction of cartilage and bone formation, to encompass pivotal roles in controlling tissue and organ homeostasis during development and adulthood. BMPs signal via plasma membrane type I and type II serine/threonine kinase receptors and intracellular SMAD transcriptional effectors. Exquisite temporospatial control of BMP/SMAD signalling and crosstalk with other cellular cues is achieved by a series of positive and negative regulators at each ste

Osteopetrosis and thalamic hypomyelinosis with synaptic degeneration in DAP12-deficient mice.

Deletions in the DAP12 gene in humans result in Nasu-Hakola disease, characterized by a combination of bone fractures and psychotic symptoms similar to schizophrenia, rapidly progressing to presenile dementia. However, it is not known why these disorders develop upon deficiency in DAP12, an immunoreceptor signal activator protein initially identified in the immune system. Here we show that DAP12-deficient (DAP12(-/-)) mice develop an increased bone mass (osteopetrosis) and a reduction of myelin (hypomyelinosis) accentuated in the thalamus. In vitro osteoclast induction from DAP12(-/-) bone mar

Lipopolysaccharide-induced osteoclastogenesis from mononuclear precursors: a mechanism for osteolysis in chronic otitis.

Osteoclasts are the only cells capable of carrying out bone resorption and therefore are responsible for the osteolysis seen in infectious diseases such as chronic otitis media and infected cholesteatoma. Pseudomonas aeruginosa is the most common organism isolated from these infectious middle ear diseases. In this study, we examined the mechanisms by which P. aeruginosa lipopolysaccharide (LPS) stimulates osteoclastogenesis directly from mononuclear osteoclast precursor cells. Osteoclast precursors demonstrated robust, bone-resorbing osteoclast formation when stimulated by P. aeruginosa LPS on

Evaluation of the Second Premolar's Bud Position Using Computer Image Analysis and Neural Modelling Methods.

Panoramic radiograph is a universally used diagnostic method in dentistry for identifying various dental anomalies and assessing developmental stages of the dentition. The second premolar is the tooth with the highest number of developmental abnormalities. The purpose of this study was to generate neural models for assessing the position of the bud of the second premolar tooth based on analysis of tooth-bone indicators of other teeth. The study material consisted of 300 digital pantomographic radiographs of children in their developmental period. The study group consisted of 165 boys and 135 g
