Anesthesia in a patient with Job's syndrome (hyper IgE). Case report.

BACKGROUND AND OBJECTIVES: Job's syndrome (JS), one of the presentations of the Hyper IgE Syndrome, is a rare immunodeficiency. It includes cutaneous abscesses, recurring pneumonias, pneumatoceles, eosinophilia, hyperimmunoglobulinemia E (> 2,000 IU.mL(-1)), and craniofacial and bone growth changes. This report describes the disease and its anesthetic management. CASE REPORT: The patient is a 13 year old black male, 40 kg, ASA II, with Job's Syndrome diagnosed 6 months prior to this admission. The patient was admitted for elongation of the right femur. He denied use of drugs and prior surgeri

Congenital anomalies: a 25-year overview.

Expansion of the discipline of hand surgery and heightened interest in congenital problems have resulted in major advances in the treatment of congenital hand anomalies over the past 25 years. Increased experience with congenital anomalies of the hand has expanded the hand surgeon's knowledge of patterns and relationships between different anomalies resulting in new methods of classification and more logical approaches to treatment. The principles of treatment of the more common anomalies, such as syndactyly, established by prior generations of hand surgeons have been refined in details of tec

[Progressive lengthening of the femur using Wagner's technic. Apropos of 48 cases].

Forty eight progressive femoral lengthenings using the Wagner technique have been performed on patients aged between 6 and 18 years (average 12 years). The main aetiology of the shortening was congenital deformity (25 cases). The mean lengthening obtained was 5.2 cm and the greatest was 11.2 cm. Lengthening obtained was more than 20 p. 100 in six cases with an extreme of 24 p. 100. The final discrepancy was always less than 25 mm. In 6 cases this was corrected by the addition of a contralateral epiphysiodesis or tibial lengthening. Weight bearing was resumed after an average of 5-6 months. Man

Fatal complication of femoral elongation in an achondroplastic dwarf. A case report.

Excessive elongation of both femora in a 12-year-old achondroplastic dwarf resulted in fixed extension and contracture of both knees. An attempt to shorten these overly elongated femora surgically was complicated by the unanticipated release of massive fat emboli, which resulted in the patient's death.

Ilizarov talus bone lenghtening combined with multiple functional free flaps for reconstruction of a mangled hindfoot: A case report.

INTRODUCTION: Mangled hindfoot injuries with calcaneal defects and extensive soft tissue loss are foot-threatening, carrying high risks of infection and osteomyelitis. Currently, there is no standardized reconstructive protocol, though various methods have been applied. This case highlights a staged strategy combining vascularized flaps and the Ilizarov technique for such injuries, with an emphasis on controlling infection risks and reducing the need for bone grafting. CASE PRESENTATION: A 38-year-old male with a right Gustilo-Anderson type IIIC calcaneal fracture (high-temperature crush inju

Changes in muscles and tendons due to neural motor disorders: implications for therapeutic intervention.

Patients with an upper motor neurone syndrome (CP) suffer from many disabling primary symptoms: spasms, weakness, and loss of dexterity. These primary 'neurogenic' symptoms often lead to secondary disabilities, muscle contractures, and tertiary effects, bone deformations. A common symptom of CP is hypertonia, with the consequence that the involved muscles remain in an excessively shortened length for most of the time. As a normal reaction of the muscle tissue, the number of sarcomeres is reduced and the muscle fibers shorten permanently: a contracture develops. A possible second type of contra

Removal of broken PRECICE Stryde intramedullary lengthening nails.

This paper describes two different techniques for removal of broken Precice Stryde intramedullary bone lengthening nails, which unlike trauma nails are solid containing mechanical components. Consequently, surgeons face unique challenges when these implants brake within medullary canal. Here, we present our surgical approach for removal of three broken implants. In one patient (46kg) both Ø10mm femoral Stryde implants (max. weight allowance 68kg) broke through the proximal locking screw hole preoperatively on the right side and intraoperatively on the left side (413 and 504 days after index su

Imaging of musculoskeletal disorders in children.

A review of the year's literature on musculoskeletal disorders in infants and children includes many advances in imaging principles, especially MR imaging pulse sequences. We have not attempted to review these advances because the principles are essentially the same in children as in adults. We have confined this review to several topics including the importance of differentiating the findings in osteogenesis imperfecta from those in the abused child, limb-lengthening procedures in infants and children, the treatment of slipped upper femoral capital epiphysis, spinal anomalies, and skeletal ab

An Efficient and Reproducible Protocol for Distraction Osteogenesis in a Rat Model Leading to a Functional Regenerated Femur.

This protocol describes the use of a newly developed external fixator for distraction osteogenesis in a rat femoral model. Distraction osteogenesis (DO) is a surgical technique leading to bone regeneration after an osteotomy. The osteotomized extremities are moved away from each other by gradual distraction to reach the desired elongation. This procedure is widely used in humans for lower and upper limb lengthening, treatment after a bone nonunion, or the regeneration of a bone defect following surgery for bone tumor excision, as well as in maxillofacial reconstruction. Only a few studies clea

Does An Osteotomy Performed in Congenital Pseudarthrosis of the Tibia Heal?

BACKGROUND: Shortening and deformity of the tibia commonly occur during the treatment of congenital pseudarthrosis of the tibia (CPT). The role of osteotomies in lengthening and deformity correction remains controversial in CPT. This study evaluates the approach to and outcome after osteotomy performed in CPT. METHODS: We performed an IRB approved retrospective review of consecutive patients with CPT treated at our institution from 2010 through 2019. Patients who underwent osteotomies were included in this study. RESULTS: Nine patients (10 osteotomies-5 proximal metaphyseal and 5 diaphyseal)

The tension-stress effect on the genesis and growth of tissues: Part II. The influence of the rate and frequency of distraction.

To assess the influence of both the rate and the frequency of distraction on osteogenesis during limb elongation, a canine tibia was used with various combinations of distraction rates (0.5 mm, 1.0 mm, or 2.0 mm per day) and distraction frequencies (one step per day, four steps per day, 60 steps per day). The distractions were performed after both open osteotomy and closed osteoclasis. Histomorphic and biochemical studies were conducted on the elongated osseous tissue, fascia, skeletal muscle, smooth muscle, blood vessels, nerves, and skin. It was determined that distraction at a rate of 0.5 m

The callotasis method of limb lengthening.

Limb lengthening by the callotasis technique of De Bastiani and use of the Orthofix external fixator was performed on 25 femurs and tibias of 23 patients between June 1989 and March 1995 in our clinic. Twenty femoral and five tibial segments were lengthened. Simultaneous lengthening of the femur and tibia was performed in two limbs. The mean duration of follow-up was 35.4 months (range, 12 to 81 months). Age at surgery ranged from 9 to 25 years (16.4 years on average). Poliomyelitis was the most common cause (69.6%) of the leg length discrepancy. The maximal achieved lengthening was 8 cm and t

Consent in Limb Lengthening Surgery: Predicting the True Incidence of Material Risk.

AIM: The consent process involves supported decision-making between the surgeon and the patient. Both potential benefits and material risks of the procedure require explanation, with adequate time for reflection. The complexity of limb reconstruction surgery includes the potential for multiple types of complications. In an attempt to delineate the material risks in lower limb lengthening, a literature review was undertaken to ascertain the published rates of complications. MATERIALS AND METHODS: A review of articles from 2003 to 2023 via PubMed and Google Scholar, including keywords 'lengthen

The effectiveness of post-lengthening plating after pediatric limb lengthening surgery: a comparative retrospective study.

OBJECTIVE: The primary objective of this retrospective comparative study was to compare clinical outcomes of post-lengthening plating, also called plating after lengthening (PAL), with external fixation alone in pediatric limb lengthening. METHODS: Plating after lengthening was applied to 41 limbs of 39 pediatric patients who underwent limb lengthening between 2020 and 2024. As a control group, 35 patients treated with only an external fixator during the same period were included. In both groups, the external fixator index, bone healing index, complication rates, and pain at final follow-up (

The lengthening of short upper extremity amputation stumps.

BACKGROUND: Short upper extremity amputation stumps are difficult to fit with an appropriate-level prosthesis. Fitting at a more proximal level generally results in decreased function. Options in the past have included stump and prosthetic modification, both of which provide limited improvement in function. Anecdotal reports of lengthening short amputation stumps have been published. This article reviews our experience with lengthening short upper extremity amputation stumps in children. METHODS: All patients who underwent an upper extremity stump lengthening at Shriners Hospital Los Angeles
