A novel inhibitor of vacuolar ATPase, FR202126, prevents alveolar bone destruction in experimental periodontitis in rats.

An acidic microenvironment formed by vacuolar ATPase (V-ATPase) expressed in plasma membranes of osteoclasts is thought to be indispensable for bone resorption. This study examined the efficacy of a novel V-ATPase inhibitor, FR202126, in reducing alveolar bone loss caused by experimental periodontitis in rats. FR202126 inhibited H+ transport in plasma membrane vesicles of murine osteoclasts, whereas FR202126 exerted no effect on H+ transport of mitochondrial ATPase or gastric H+,K+-ATPase, indicating that FR202126 is a specific inhibitor of V-ATPase. As expected from the mechanism, FR202126 re

An ingested metallic wire migrating from stomach to pancreas treated by laparoscopic surgery: A case report.

INTRODUCTION: Foreign bodies inside the pancreas are rare and usually occur after the ingestion of sharp objects such as a fish bone, a sewing needle, or a toothpick. Furthermore, an ingested metallic wire migrating from stomach to pancreas is very rare. CASE PRESENTATION: We report a 36-year-old woman who was admitted to our hospital with "3-day history of dull progressive epigastric pain." Computed tomography of the abdomen revealed a linear, high-density body between the stomach wall and the pancreas. During the operation, a linear, hard, metallic wire was found in the adhesive tissue betw

Bone-supported arch bars are associated with comparable outcomes to Erich arch bars in the treatment of mandibular fractures with intermaxillary fixation.

PURPOSE: Titanium arch bars that are directly fixated to the maxilla and mandible with self-drilling locking screws combine features of Erich arch bars and bone-supported devices and present an alternative method of intermaxillary fixation (IMF) that possesses potential advantages over existing techniques. The objective of this study was to compare IMF using this device with Erich arch bars secured with circum-dental wires. MATERIALS AND METHODS: A retrospective cohort study was performed of patients who were surgically treated for mandibular fractures from 2012 through 2013. The primary pred

A mixed reality hand fracture simulator for virtual K-wire fixation.

PURPOSE: Proper management of hand fractures requires accurate reduction of bone fragments and stabilization with Kirschner wire (K-wire) fixation. Limited case exposure in residency and a shortage of feedback-rich skills assessment constrain deliberate practice. This work describes a mixed reality hand fracture simulator that integrates a realistic physical model with instrument tracking, real-time virtual image guidance, and quantitative performance feedback for training percutaneous K-wire fixation. METHODS: We developed a benchtop mixed reality (MR) simulator integrating an electromagneti

Buried or exposed Kirschner wire for the management of hand and forearm fractures: A systematic review, meta-analysis, and meta-regression.

BACKGROUND: The recommendation on whether to bury or expose the Kirschner wire (K-wire) for the management of fractures has still been controversial with inconsistent results in the published studies due to the potential issue associated with exposed K-wire is the heightened risk of infection, as it comes into direct contact with the external environment and air. This study aims to summarize the specific outcomes between buried and exposed K-wire for the management of hand and forearm fractures. METHODS: We conducted relevant literature searches on Europe PMC, Medline, Scopus, and Cochrane Li

Arthroscopic treatment for nonunion of the scaphoid.

Scaphoid nonunion has traditionally been treated by open surgery where the pseudarthrosis has been cleaned while either a structural wedged bone graft, or chips of cancellous bone has been used to fill the defect. K-wires or a screw has been used to stabilize the bone. Using the arthroscopic technique for treatment of nonunion of the scaphoid gives us small exposure to the joint, however with an excellent view of the bones, the articular surfaces and the intraarticular ligaments. The results from arthroscopic treatment for scaphoid nonunion with bone grafting using chips of cancellous bone are

Intramedullary fixation of unstable metacarpal fractures.

Intramedullary stabilization of metacarpal shaft and neck fractures is a relatively simple, cost-effective, and safe technique with good published outcomes (Fig. 9); however definite advantages over other techniques of fracture stabilization, or indeed simple early mobilization in some instances, have not been clearly demonstrated. A recent publication does suggest that the technique is comparable to percutaneous transverse fixation in the context of fifth metacarpal neck fractures.

An in vitro study in separating tensile loads during maxillo-mandibular fixation using wire and/or elastics.

Intermaxillary fixation (IMF) or maxillo-mandibular fixation (MMF) is a fundamental process in stabilizing the maxilla and mandible through dental maximum intercuspation (MIP) during the management of trauma, orthognathic surgery, and reconstruction. Despite the availability of several techniques in achieving adequate maxillo-mandibular fixation, concerns have been raised regarding the sufficiency of using only latex elastics to counter displacing forces during reduction and fixation. To address this, an in vitro study was conducted to compare the efficacy of three maxillo-mandibular fixation

The uncemented total hip arthroplasty. Intraoperative femoral fractures.

Intraoperative fractures of the proximal femur occurred in 40 (38 patients) of 630 (6.3%) biological ingrowth total hip arthroplasties performed between January 1984 and July 1986. Twenty-three of these fractures occurred during 131 revision arthroplasties (17.6%) and 17 during 499 primary arthroplasties (3.5%). All but two of the fractures were recognized during surgery. Thirty-seven were treated with either Parham bands or cerclage wires. Bone graft was added to the fracture site in 31 fractures. All of the fractures healed. Three of the patients failed to achieve stable fixation and have re

Intramedullary Bone Transport Nail for the Treatment of Segmental Tibial Bone Defect After Acute Traumatic Bone Loss.

Medullary bone transport is a valuable technique to perform distraction osteogenesis with all-internal components. This technique has notable advantages over other techniques which rely on external pins, wires, and frames. Preoperative templating and surgical planning are critical to the success of this procedure to ensure that movement of the segment across the defect can be performed based on the specifications of the nail. The case of a young patient with a 67 mm diaphyseal tibial defect after an open tibial shaft fracture who had successful reconstruction with a medullary bone transport na

Strength evaluation of lateral calcaneal wall with circular external fixation application in cadaveric specimens.

Circular external fixation is a useful treatment option for the correction of complex trauma, extremity deformity, osteomyelitis, and reconstruction of the foot and ankle. The goal of the present study was to determine the degrees of bolt rotation required to create enough wire tension to cause structural failure of the lateral calcaneal wall when stressed with both olive and smooth wires in a cadaveric model. Ten fresh, thawed, below-the-knee specimens were tested at the San Diego Cadaveric Academic Research Symposium. The mean bolt rotation required to pull an olive wire through the lateral

An optimized method for establishing experimental rat periodontitis using "double-ligature" technique.

Establishing stable and dependable animal models of experimental periodontitis is critical in advancing our understanding of the etiology, clinical diagnosis, and treatment of periodontitis. However, conventional silk ligation methods for inducing periodontitis in rats have limitations, with silk thread detachment being a major concern. In this study, we established a reliable rat periodontitis model using a novel "double-ligature" technique combining silk sutures with orthodontic wires, augmented by a high-sugar diet. Thirty rats were randomized into five groups: control, wire-only, silk-only

Faster healing of radial neck fractures in children treated with the Metaizeau technique compared with percutaneous Kirschner wire fixation: a population-based study.

BACKGROUND: There is increasing experience in the treatment of radial neck fractures using the minimally invasive Metaizeau technique with retrograde elastic stable intramedullary nailing (ESIN) as an alternative to percutaneous Kirschner wire (K-wire) fixation. The main objective of this study was to compare fracture healing between these two techniques. METHODS: This population-based, single-centre study included all 53 consecutive and eligible patients aged < 16 years with radial neck fractures who were operated on at Oulu University Hospital (OUH), Finland, between 2000 and 2020. OUH is t

Temporary Dorsal Staple Fixation of Scapholunate Interosseous Ligament Repair and Reconstruction.

There is a wide variety of techniques to address scapholunate interosseous ligament injury, including both repair and reconstruction of the ligament. What many of these techniques have in common is the protection of the repair or reconstruction by the use of Kirschner wires (K-wires) temporarily placed across the scapholunate and sometimes scaphocapitate articulations to provide immobilization. There are multiple potential downsides to K-wire utilization, including possible interference with the repair or reconstruction, distraction of the scapholunate articulation as the K-wire is passed, occ

[Misunderstanding about the tension band principle in the treatment of patella fractures].

Kirschner's wire tension band was the first choice in the treatment of patella fracture. But the principle of tension band had been misunderstood for long time. In the traditional principle, it was believed that tension band could change the separating force between fracture fragments into compressing stress when exercising. Eccentric fixation and articular surface cracking were disadvantageous. These could make the fracture fragments separate either at resting or exercising, which cause the gaps without pressure. Thus excercising could not change the separating force into compressing stress.

Metaphyseal osteotomy for the treatment of tailor's bunions.

There have been many surgical approaches described for the surgical treatment of tailor's bunions. The authors have been using a procedure that addresses both an elevated fourth to fifth intermetatarsal angle and an elevated lateral deviation angle, for the past 3 years. The metaphyseal osteotomy provides reliable correction of these two abnormalities and allows for uncomplicated rigid fixation.

Structures at Risk During Volar Percutaneous Fixation of Scaphoid Fractures: A Cadaver Study.

BACKGROUND: Fracture of the scaphoid bone can be treated with cast immobilization or surgery. Historically, surgery was reserved for displaced fractures. However, because weeks of cast immobilization may result in stiffness, loss of strength, loss of bone density and an inability to work or participate in recreational activities for a prolonged period, operative treatment of non-displaced fractures has become increasingly common. Several surgical techniques for fixation have been described, but their risks and benefits have not yet been clearly elucidated. In a study in cadavers, we investigat
