Pathological fracture induced by Halicephalobus gingivalis (Nematoda: Rhabditida) in a horse limb.

Halicephalobus gingivalis is a free-living nematode that occasionally causes infections in horses. We report a rare case of limb fracture of horse caused by infection with H. gingivalis. An 8-year-old mare was referred to the Veterinary Hospital of the Federal University of Lavras with claudication grade 5 of the right hind limb, that had been started 3 months ago. The patient had aseptic arthritis in the tarsal joint and edema that extended to the quartile. The radiographic examination showed punctate osteolysis with exacerbation of bone trabeculation along the calcaneus, talus, proximal epip

Optimal entry point for antegrade and retrograde femoral intramedullary nails.

Surgical management of femoral shaft fractures with intramedullary nails has become the standard of care, with multiple options for entry point described, including piriformis entry, trochanter entry and retrograde femoral nails. Our present review describes the surgical anatomy of the proximal and distal femur and its relation to different entry points for intramedullary femoral nails. In addition, we reviewed relative indications for each technique, difficulties associated and possible complications.

Predicting Deformity Correction of Growth Modulation in Late-onset Tibia Vara.

BACKGROUND: Growth modulation in late-onset tibia vara (LOTV) has been reported to yield variable results. We hypothesized that parameters of deformity severity, skeletal maturity, and body weight could predict the odds of a successful outcome. METHODS: A retrospective review of tension band growth modulation for LOTV (onset ≥8 y) was performed at 7 centers. Tibial/overall limb deformity and hip/knee physeal maturity were assessed on preoperative anteroposterior standing lower-extremity digital radiographs. Tibial deformity change with first-time lateral tibial tension band plating (first LTT

A Novel Technique to Close Large Defects in the Foot Secondary to Infection Using Widely Available and Inexpensive Tools: A Case Report.

One of the challenges after central ray resection is a large soft-tissue defect. Many authors have reported the use of external fixators as a means of narrowing the forefoot. Ours is the first article to report an interesting case using widely available and inexpensive tools such as Kirschner and cerclage wires as an external fixation means of narrowing the forefoot after a complete second-ray resection and extensive soft-tissue debridement for a severe diabetic foot ulcer. This simple yet inexpensive technique is easy to perform for any foot and ankle surgeon at any hospital or surgical cente

Impact of periprosthetic femoral fracture fixation plating constructs on local stiffness, load transfer, and bone strains.

Secondary femoral fractures after the successful plate-screw fixation of a primary Vancouver type B1 periprosthetic femoral fracture (PFF) have been associated with the altered state of stress/strain in the femur as the result of plating. The laterally implanted condyle-spanning plate-screw constructs have shown promises clinically in avoiding secondary bone and implant failures as compared with shorter diaphyseal plates. Though the condyle-spanning plating has been hypothesized to avoid stress concentration in the femoral diaphysis through increasing the working length of the plate, biomechan

New Technologies in Pediatric Deformity Correction.

The ability to correct limb deformities is one of the core elements of pediatric orthopedics. The term, orthopedics, is derived from the Greek language and means straightening (ortho) children (paidos). New advances in the evaluation and management of children with limb alignment or limb length issues are constantly appearing. This review highlights some of the recent technologies that have been developed to improve the care of these children.

Restricting Lower Limb Flail is Key to Preventing Fatal Pelvic Blast Injury.

Pelvic vascular injury in the casualty of an explosive insult is a principal risk factor for increased mortality. The mechanism of injury has not previously been investigated in a physical model. In this study, a small-animal model of pelvic blast injury with a shock-tube mediated blast wave was utilised and showed that lower limb flail is necessary for an unstable pelvic fracture with vascular injury to occur. One hundred and seventy-three cadaveric mice underwent shock-tube blast testing and subsequent injury analysis. Increasingly displaced pelvic fractures and an increase in the incidence

Use of a cross-leg free muscle flap to reconstruct an extensive burn wound involving a lower extremity.

A young patient sustained a high-voltage burn with extensive destruction of the soft tissue in his left lower extremity. Occlusion of the anterior and posterior tibial arteries, loss of toe extensors and the superficial and deep peroneal nerves were noted, besides the exposure of the lower end of the tibia and metatarsal bones. In the absence of proper recipient vessels, a cross-leg free latissimus dorsi muscle flap with overlying skin and depending on the vessels of the contralateral foot was used successfully for reconstruction of the defect. The pedicle was divided 3 weeks after microvascul

Reconstruction following oncological iliosacral resection.

AIMS: The sacroiliac joint (SIJ) is the only mechanical connection between the axial skeleton and lower limbs. Following iliosacral resection, there is debate on whether reconstruction of the joint is necessary. There is a paucity of data comparing the outcomes of patients undergoing reconstruction and those who are not formally reconstructed. METHODS: A total of 60 patients (25 females, 35 males; mean age 39 years (SD 18)) undergoing iliosacral resection were reviewed. Most resections were performed for primary malignant tumours (n = 54; 90%). The mean follow-up for surviving patients was ni

Hyperplastic callus formation in osteogenesis imperfecta following intramedullary rodding.

Hyperplastic callus formation has been reported in patients with osteogenesis imperfecta following fractures but not following intramedullary rodding. The present case is that of a child who developed massive hyperplastic callus following osteotomy and intramedullary rodding of all lower extremity bones. Noted complications of intramedullary rodding include refracture, delayed union, nonunion, and pin migration, and it is our belief that hyperplastic callus formation should be considered another potential complication.

Rare presentation of bilateral supracondylar femoral fractures in an individual with chronic spinal cord injury.

INTRODUCTION: Osteoporosis is a disease characterized by low bone mass and fracture of the bones irrespective of the aetiology. Insufficient data is available on the management of osteoporosis specific to individuals with spinal cord injury (SCI). CASE REPORT: The case describes a 24-year-old gentleman with a chronic spinal cord injury of 3-year duration, who was poorly evaluated and underwent unsupervised trial of therapeutic standing, resulting in a symmetrical supracondylar femoral fragility fracture. The assessment and management of the fractures and subsequent hemarthroses are also descr

Changing epidemiology of lower extremity fractures in adults over a 15-year period - a National Hospital Discharge Registry study.

BACKGROUND: Demographic changes led to an increasingly ageing population in Germany and thus to possible changes in the frequency of fractures. The primary aim of this study was to report changes in fracture rates of the lower extremities in Germany in 2002 compared to 2017 and to evaluate those changes. METHODS: Inpatient data from the German National Hospital Discharge Registry (ICD10) for 2002 and 2017 were evaluated. Changes in total counts and incidence rates were analysed for fractures in the following locations: femoral neck, pertrochanteric, subtrochanteric, distal femur, femoral shaf
