The nature of brain dysfunction in autism: functional brain imaging studies.

PURPOSE OF REVIEW: Functional magnetic resonance imaging studies have had a profound impact on the delineation of the neurobiologic basis for autism. Advances in fMRI technology for investigating functional connectivity, resting state connectivity, and a default mode network have provided further detail about disturbances in brain organization and brain-behavior relationships in autism to be reviewed in this article. RECENT FINDINGS: Recent fMRI studies have provided evidence of enhanced activation and connectivity of posterior, or parietal-occipital, networks and enhanced reliance on visuosp…

Human brain aminopeptidase A: biochemical properties and distribution in brain nuclei.

Aminopeptidase A (APA) generated brain angiotensin III, one of the main effector peptides of the brain renin angiotensin system, exerting a tonic stimulatory effect on the control of blood pressure in hypertensive rats. The distribution of APA in human brain has not been yet studied. We first biochemically characterized human brain APA (apparent molecular mass of 165 and 130 kDa) and we showed that the human enzyme exhibited similar enzymatic characteristics to recombinant mouse APA. Both enzymes had similar sensitivity to Ca(2+). Kinetic studies showed that the K(m) (190 mumol/L) of the human…

The blood-brain barrier as a regulatory interface in the gut-brain axes.

The blood-brain barrier (BBB) prevents the unrestricted movement of peptides and proteins between the brain and blood. However, some peptides and regulatory proteins can cross the BBB by saturable and non-saturable mechanisms. Leptin and insulin each cross the BBB by their own transporters. Impaired transport of leptin occurs in obesity and accounts for peripheral resistance; that is, the condition wherein an obese animal loses weight when given leptin directly into the brain but not when given leptin peripherally. Leptin transport is also inhibited in starvation and by hypertriglyceridemia. S…

[Brain hypothermia treatment for the management of severe pediatric brain injury].

In the management of severe pediatric brain injury, attention has previously been paid to brain edema, ICP elevation and low cerebral perfusion pressure (CPP). However, in the acute stage within 3-6 hours after trauma, brain hypoxia and hyperglycemia associated with diffuse brain injury are often observed. We have pointed out brain thermo-pooling (elevation of brain tissue temperature) and brain hypoxia caused by defective release of oxygen from hemoglobin (due to decrease in red blood cell enzyme (DPG)) as a new mechanism of brain injury. To treat these pathologic changes, we have developed a…

[Strategies for drug delivery to the brain across the blood-brain barrier].

Designing drugs (including peptides) with greater lipophilicity are still effective strategy for the enhancement of the permeability across the blood-brain barrier (BBB), which is considered to be lipoidal membrane barrier of endothelial cells which have tight junctions without fenestrations. However, for the development of brain-specific drug delivery for neuroactive drugs, it may be greater promising strategy to utilize the specific influx transporters existing at the BBB for hexoses, amino acids, monocarboxylic acids, amines or to inhibit the functions of the efflux transporters such as P-g…

[Postoperative brain infarction in a patient with the previous asymptomatic brain infarction].

We report a case of a postoperative brain infarction, in which an asymptomatic preoperative brain infarction was also revealed postoperatively. A 63-year-old man with bladder carcinoma was scheduled for the cystoplasty. The patient had no prominent preoperative abnormality. Anesthesia was maintained with isoflurane in N2O/oxygen combined with a spinal block and a continuous epidural block. Anesthesia lasted uneventfully for 16.5 hours. However, emergence from anesthesia was delayed. On the first postoperative day, motor aphasia and right hemiparalysis was confirmed. The computed tomography (CT…

A review of brain retraction and recommendations for minimizing intraoperative brain injury.

Brain retraction is required for adequate exposure during many intracranial procedures. The incidence of contusion or infarction from overzealous brain retraction is probably 10% in cranial base procedures and 5% in intracranial aneurysm procedures. The literature on brain retraction injury is reviewed, with particular attention to the use of intermittent retraction. Intraoperative monitoring techniques--brain electrical activity, cerebral blood flow, and brain retraction pressure--are evaluated. Various intraoperative interventions--anesthetic agents, positioning, cerebrospinal fluid drainage…

Nocardia as a pathogen of the brain: mechanisms of interactions in the murine brain--a review.

Nocardia asteroides strain GUH-2 invades the brain without inducing an early inflammatory response in normal mice. This strain can grow within the neurons as well as adjacent areas causing neurological damage without abscess formation. At low inoculum doses, this organism is gradually cleared from the brain after the initial burst of growth. Therefore, at two weeks, the brains of these mice appear to become sterile, but the animals begin to develop a variety of neurological signs including an L-dopa responsive headshake. It was found that mice that recovered from sublethal infection with N. as…

Quantitative morphology of the panda brain in comparison with the brains of the raccoon and the bear.

The quantitative development of brain and brain components with respect to body-size is discussed comparatively for the Chinese Giant Panda, the Canadian Raccoon and the American Black Bear. The panda ranks highest in encephalization as well as in relative volumes of neocortex, cerebellum and six other parts. The raccoon exhibits the strongest development of olfactory elements. The bear stays below the other species, showing but a single index that is superior to the corresponding index in one of them only. These results must be considered within the context of recent debates on the systematic…

Changes of the blood-brain barrier in experimental metastatic brain tumors.

An experimental model for blood-borne cerebral metastases was developed by introducing Walker 256 carcinoma cells selectively into the intracranial internal carotid artery of rats. This model was used to study the regional capillary permeability of rat brain and metastatic brain tumors of various sizes with the aid of 14C alpha-aminoisobutyric acid (AIB) quantitative autoradiography. The regional capillary permeability varied with the anatomical location and size of the tumor. Intraparenchymal tumors less than 1 mm in diameter showed no increased permeability to AIB. As the tumors enlarged ove…

Experimental brain death. I. Morphology and fine structure of the brain.

The morphological characteristics of brain death were examined in baboons and cats after artificial cerebral ischaemia. All animals showed autolytic changes in the brain, ischaemic neuronal changes, midbrain haemorrhages, focal necrosis of the brain-stem, demarcation at C 1/C 2 cord segment, and displacement of cerebellar tissue. Ultrastructural examination revealed extreme brain oedema, autolytic changes, and complete obstruction of capillaries by astrocytic and endothelial swelling and intravascular blebs. These data indicate that brain death develops in several stages. If the process starts…

[Congenital brain malformations].

CLINICAL ISSUE: Malformations of the central nervous system belong to the most common developmental disorders in humans. The clinical presentation of brain malformations is nonspecific including developmental delay, hypotonia, and/or epilepsy. The great heterogeneity concerning etiology, mechanisms of development and morphology is challenging for diagnosis and classification of brain malformations. Thereby recognizing specific malformations is essential for optimal patient management and prognostic evaluation. The aim of this article is to give an overview of several clinically relevant brain …

Brain Trauma Imaging.

Imaging of mild traumatic brain injury (TBI) using conventional techniques such as CT or MRI often results in no specific imaging correlation that would explain cognitive and clinical symptoms. Molecular imaging of mild TBI suggests that secondary events after injury can be detected using PET. However, no single specific pattern emerges that can aid in diagnosing the injury or determining the prognosis of the long-term behavioral profiles, indicating the heterogeneous and diffuse nature of TBI. Chronic traumatic encephalopathy, a primary tauopathy, has been shown to be strongly associated with…

Brain arteriovenous malformations.

Brain arteriovenous malformations are an important cause of intracerebral hemorrhage in the young. Ruptured AVM's are often treated, as the risk of rebleeding is high. The treatment of incidentally discovered, unruptured AVMs is controversial as the morbidity and mortality of treatment may exceed that of the AVM's natural history. Management is multimodal and includes observation with follow up, as well as microsurgical resection, endovascular embolization, and stereotactic radiosurgery. Multidisciplinary teams are important in evaluating patients for treatment. The goal of treatment is comple…

Chronic Brain Failure.

Chronic brain failure, also known as dementia or major neurocognitive disorder, is a syndrome of progressive functional decline characterized by both cognitive and neuropsychiatric symptoms. It can be conceptualized like other organ failure syndromes and its impact on quality of life can be mitigated with proper treatment. Dementia is a risk factor for delirium, and their symptoms can be similar. Patients with dementia can present with agitation that can lead to injury. Logic and reason are rarely successful when attempting to redirect someone with advanced dementia. Interactions that offer a …

Nonmalignant Brain Tumors.

PURPOSE OF REVIEW: This article describes the diagnosis and management of meningioma, pituitary adenoma, craniopharyngioma, and glioneuronal tumors. RECENT FINDINGS: Both meningiomas and pituitary adenomas are common brain tumors. In many cases, these lesions are found incidentally on imaging when patients are being evaluated for a variety of symptoms and signs. While nonmalignant, these tumors are occasionally associated with significant morbidity due to location and resulting secondary symptoms. Rarely, these tumors can also transform into malignant variants. Surgical techniques allow for m…
