Vaginal delivery compared with caesarean section in early preterm breech delivery: a comparison of long term outcome.

OBJECTIVE: To determine the optimum mode of delivery of the early preterm fetus in breech presentation. DESIGN: Retrospective comparison of two cohorts of preterm breech fetus. SETTING: Two tertiary care centres: at one centre the preferred management for preterm breech presentation was vaginal delivery; at the other centre, the preferred method was caesarean section. POPULATION: All singleton infants delivered after breech presentation from 1984 through 1989, at a gestational age of 26 to 31 weeks. Those with lethal congenital abnormalities, placenta praevia, placental abruption, fetal dea…

Recommendations of the FIGO Committee on Perinatal Health on guidelines for the management of breech delivery.

The FIGO Committee on Perinatal Health: Prof Künzel (Giessen), Prof. Bréart (Paris), Prof. Davidson (Los Angeles), Prof. Eskes (Nijmegen), Prof. Huch (Zürich), Prof. Odendaal (Tijgerberg), Prof. Patel (Dundee), Prof. Takeda (Tokyo) and Prof. Yu (Clayton), has discussed the management of the breech in gestation and during labor for the developed and developing countries on the background of perinatal and maternal morbidity and mortality. The recommendations are an expression of the opinion of the committee which have been presented to the Executive Board of FIGO for information on 12-13 Novembe

Developmental dysplasia of the hip in twins: the importance of mechanical factors in the etiology of DDH.

BACKGROUND: Developmental dysplasia of the hip (DDH) is a disease of multifactorial etiology. Genetic and intrauterine mechanical factors in … with 420 hips) of whom 48 were a cephalic breech, 35 were a cephalic-cephalic, and 22 were a breech-breech presentation. The presentation was defined by regular prenatal ultrasounds and the obstetric records at birth. The average gestational age was 36.7 weeks and average weight at birth was 2290 g. Control group 1 consisted of 274 single gestation children, with a c …

Prediction of obstetric outcome in vaginal breech birth using ultrasound pelvimetry in nulliparous women-A feasibility study.

INTRODUCTION: Magnetic resonance (MR) pelvimetry is widely used in planning vaginal breech birth and may support women's informed decision-making regarding their preferred mode of birth. This feasibility study aimed to assess whether transperineal ultrasound (TPU) could measure the maternal pelvis as accurately as MR pelvimetry and thus predict the outcome of vaginal breech birth. MATERIAL AND METHODS: In this prospective cohort study, nulliparous women with a singleton fetus in breech presentation received TPU for the measurement of the anteroposterior mid-pelvic diameter (AMD). These measur…

Femoral fracture during breech vaginal delivery: A case report.

Femoral fracture is a rare but significant foetal injury, more frequent and likely to happen when the foetus is malpositioned or in a breech presentation. Cesarian section does not appear to be protective and all recent publications report cases occurring during cesarian section. We report a case that occurred in a vaginal delivery of a single footling breech presentation. This complication allows us to remind that femur fracture is a complication of breech delivery whatever the modality. The prognosis is good with early diagnosis.

A survey of pregnant women's attitude towards breech delivery and external cephalic version.

A structured interview survey was carried out in 150 women who came for their first antenatal visit in a university hospital in Hong Kong between June and July 1998. Their opinions and perceptions of fetal and maternal safeties on different modes of delivery for both cephalic and breech presentation, and external cephalic version (ECV) were surveyed. Their decisions on the management of term breech-presenting pregnancy were examined. Most women (92%) preferred vaginal delivery to Caesarean delivery (CS) in case of cephalic presentation, mainly because it was a natural way of parturition. They …

Intracranial hemorrhage in low-birth-weight twins during neonatal period.

Since twins have a higher incidence of premature birth, fetal distress, asphyxia and other pre- and intrapartum high-risk factors, it is rea … rst week of life in the breech group than in the cephalic presentation group. The adverse effect of breech presentation on intracranial hemorrhage did not appear to be mediated by the well recognized risk factors such as lower gestational age, lower Apgar scores, and higher incidence of respiratory distress syndrome and assisted ventilation. Since most of the twins w …

Timing of head circumference measurement in newborns.

AIM: The objective of this study is to compare head circumferences (HCs) measured on the day of delivery and on the third day after delivery. METHOD: A total of 499 children born in the maternity department were recruited for the study. HC was measured in centimeters immediately after birth and on the third day after delivery. RESULTS: There are statistically significant differences between HC at the time of birth and on the third day. Presentation and method of delivery were significantly associated with differences in HC. For presentation, we found that breech newborns more commonly have s

Fetal position and skull shape.

The skull shapes of 615 consecutively born babies weighing over 2500 g were examined visually and by palpation. A characteristic deformity of the occiput was found which was strongly associated with breech presentation; it is suggested that this be termed hyptiocephaly. Severe forms of hyptiocephaly might partially explain the high incidence of occipital osteodiastasis in breech babies. A lesser occipital deformity was associated with occipito-posterior presentation. It was found that facial asymmetry declined with increasing parity. Statistical trends suggest that all of these abnormalities o…

Twin entanglement revisited.

A fear of interlocking twins is one factor that has led to a high Cesarean section (C/S) rate in breech (A)/vertex (B) (Br(a)/Vtx(b)) twins. We sought to estimate the frequency of occurrence of twin entanglement, and of interlocking Br(a)/Vtx(b) twins in vaginal deliveries. 541 twins and 48,195 deliveries were retrospectively studied for the period 1987-1995. The incidence of Br(a)/Vtx(b) deliveries was noted, and the number of deliveries marked by interlocking and collision of fetuses. The mode of delivery, reason for C/S, and sources of perinatal mortality were noted. Only 43 deliveries were

[The effect of breach presentation on the development of the hips].

Authors examined, using sonographic methods in 100 children with breech-presentation, which of the intrauterine postures means a major danger in the development of hip dysplasia. It was found that acetabular alterations, requiring therapy or not, develop more frequently in groups in which the hips were in total flexion, the knees in semiflexion and the lower extremities in major external rotation. Based on their results they state that not the simple breech position is the greatest danger in the development of dysplasia and dislocation in babies born with breech presentation.

Breech births in twin pregnancy: an analysis of Apgar score and perinatal mortality from a Nigerian sample.

Perinatal mortality (PNM) rates are reported for 146 twin-1 and 192 twin-2 breech births among 622 consecutive twin pairs delivered at the U … th first and second twin with birthweight 3.0 kg and above. Factors such as low birthweight, breech/breech presentation, breech extraction and retained second twin breech contributed significantly to the high PNM rates. More favorable PNM rates were recorded among a limited number of breech infants delivered by primary cesarean section for breech/breech or first twin …

OBJECTIVE: To assess training in breech delivery in the United Kingdom at the level of registrar. METHOD: Three hundred and nineteen registrars in obstetrics and gynecology in the United Kingdom replied to a postal questionnaire. RESULTS: The various indications of cesarean section in breech presentation answered by respondents were: pre-term labor, pelvimetric measurement of inlet/outlet less than 11.5 cm, primigravida, no spontaneous labor, estimated fetal weight less than 2.5 kg or more than 3.5 kg, non-progression of labor in active phase, non-delivery after one hour of active pushing in…
