INTRODUCTION: In recent years, a number of studies have been carried out with the intention of isolating clinical dimensions in the psychopathology of bulimia nervosa. Although borderline personality has been considered a core element of the bulimic psychopathology by most of the authors, it has not been incorporated into any of these models. In this context, the present study was aimed at testing the consistence of the more complex model proposed until now, including in the analysis borderline personality as a clinical variable.

Although the pathogenesis and treatment of anorexia nervosa and bulimia nervosa have traditionally been discussed in psychological terms, these eating disorders are always accompanied by somatic symptoms, symptoms which may become very serious. Moreover, as the anorexic or bulimic patient seeking medical treatment does not as a rule reveal the self-inflicted nature of her symptoms to the doctor, it is vital that physicians recognize such symptoms as manifestations of eating disorders. The article consists in a review of somatic complications commonly accompanying anorexia and bulimia, and thei…

OBJECTIVES: The exact causes of hyperamylasemia detected in bulimia nervosa are unknown but it is presumed to be due either to repeated binging or to vomiting. This study set out to investigate the importance of vomiting in producing the raised serum amylase and to clarify whether the amylase in pancreatic or salivary.
METHODS: Patients suffering from hyperemesis gravidarum who were repeatedly vomiting in pregnancy but not binge eating had their total serum and pancreatic amylase measured. Bulimic patients and a control sample of nonvomiting pregnant women were similarly studied. An assessmen…

OBJECTIVE: The objective of this study was to evaluate the meal patterns and nutrient intake of women with bulimia nervosa (BN), and to compare them to clinical variables and recommended dietary allowances (RDA).
METHOD: The nutrient intake of 50 women with BN was analyzed from 14 day dietary records. The data were compared to the median intakes of a random sample of women and to the RDA. Clinical variables for the women with BN were also available.
RESULTS: Compared to nonbinge episodes per day, binge eating was significantly higher in energy from sucrose, fat, saturated fatty acids (SFA), …

Women with eating disorders have been disqualified as candidates for plastic surgery. We present a group of 6 young women with bulimia nervosa who presented with clinically symptomatic evidence of macromastia. All patients reported that dysfunctional eating habits, at least in part, where due to breast enlargement. Five patients underwent bilateral reduction mammaplasty. Patients were interviewed postoperatively and reported relief of physical symptoms and improvement in psychological well-being. Symptoms of eating disorders were completely eliminated or greatly reduced. This series has suppor…

OBJECTIVE: We examined the new subtyping criteria for bulimia nervosa by comparing purging and nonpurging bulimia nervosa patients with two similar subgroups: binge eating disorder patients and an eating disorder not otherwise specified (ED-NOS) group who compensates but does not binge eat or meet weight criteria for anorexia, which we labeled compensatory eating disorder (CED). Of particular interest was whether purging could differentiate a more "disturbed" group of patients, bulimic or otherwise.
METHOD: We compared these four subgroups on the Eating Disorders Inventory (EDI), the Hopkins …

OBJECTIVE: Recent research has suggested that a large proportion of patients with bulimia nervosa have seasonal (winter) worsening of mood symptoms similar to seasonal affective disorder (SAD). The objectives of this study were to determine the specificity of this finding in anorexia and bulimia nervosa, and to further delineate the seasonal mood and eating patterns in bulimia nervosa.
METHOD: A modified Seasonal Pattern Assessment Questionnaire (SPAQ) was administered to consecutive female patients assessed at an Eating Disorders Clinic with DSM-III-R diagnoses of bulimia nervosa (BN, N = 60…

Eating disorders remain an important health care problem for clinicians working with children and adolescents. Although the cause of these illnesses remains obscure, information about the effectiveness of different forms of treatment is available to guide the clinician. Individuals involved in the treatment of such patients should be aware of the means of diagnosing the conditions, the common complications of both anorexia and bulimia, the management of these, and the indications for specific treatment modalities. Although the outcome is variable, effective treatment delivered early in the cou…

Symptoms of anxiety can be prominent during treatment of bulimia nervosa. Our experience is that bulimics who abuse laxatives have the most prominent symptoms of anxiety. We conducted ratings of anxiety in 23 bulimics who purge with laxatives and 17 who purge by vomiting. We found that the laxative-abusing group had higher levels of state but not trait anxiety and that they were more likely to be treated with medication for anxiety during hospitalization. These data suggest an association between laxatives and anxiety in bulimia nervosa.

Fifty-five eating-disordered women and 16 normal controls participated in this study to determine whether olfactory function is altered in patients with food-restricting anorexia, anorexia with bulimic features, and bulimia nervosa. Olfactory function was assessed using the University of Pennsylvania Smell Identification Test and by determining phenyl ethyl alcohol odor detection thresholds. Only the very low-weight anorexics showed impairments in their identification and detection of odors. This group's olfactory function did not improve from admission to discharge despite significant weight …

This study examined two aspects of psychodynamic theory concerning bulimia nervosa, that bulimic patients ascribe to a traditionally feminine sex-role and that bulimic women have difficulty differentiating emotional from visceral sensations. 18 bulimic and 18 control women were administered the Bem Sex-role Inventory and the Eating Disorder Inventory. Contrary to dynamic theory, bulimic women were not more likely classified as traditionally feminine than control subjects; however, significantly more controls than bulimic women were classified as androgynous. Analysis of scores on the Eating Di…

Since the delineation of bulimia nervosa as a distinct syndrome in 1979, a variety of etiological models and related treatments have evolved. Methodological advances in evaluation have been reflected in recent outcome studies. There is now extensive evidence for the effectiveness of various short-term psychotherapies for bulimia nervosa. However, there is no convincing support for the specificity of any one form; all have a salubrious effect. Cognitive-behavioral therapy has been the most extensively researched. A parallel literature examines antidepressant pharmacotherapy for this disorder, a…

Bulimia nervosa often follows a chronic and relapsing course. To understand relapse in this disorder, we highlight cognitive processes underlying the binge/purge cycle. Links are drawn between cognitions, causal perceptions, and the binge/purge cycle in a reformulation of the abstinence violation effect with a special focus on attributions. This reformulation is then applied to the lapse-relapse transition in bulimia nervosa. Finally, theoretical and clinical implications of the reformulation are presented.

The abnormalities in eating behavior associated with bulimia nervosa suggest that patients with this illness may have a disturbance in satiety. The present study employed a six-meal protocol to assess satiety in both binge and non-binge eating episodes in women with bulimia nervosa and normal controls by examining whether an increase in the size of a soup preload led to a decrease in the amount of food consumed in a subsequent test meal. In control subjects, the increase in preload size was associated with an increase in fullness and a reduction in consumption of the non-binge test meal. Patie…

Outcome predictors of a cognitive behavioural group treatment for bulimia nervosa were examined. Pre-treatment self-esteem, binge frequency and ineffectiveness, but not duration of disorder, significantly predicted outcome on at least one measure (binge frequency or overall eating pathology) at post-treatment and/or three-month follow-up. The results are related to previous studies and to theories of maintenance of the disorder.

Medical complications of anorexia and bulimia nervosa may include metabolic, renal, gastrointestinal, cardiovascular, pulmonary, endocrinologic, neurologic, hematologic, immunologic, dental, and/or dermatologic effects. Patient evaluation and differential diagnosis must keep all of these possibilities in mind if the adolescent with an eating disorder is to be successfully treated.

Bulimia nervosa is a fascinating but challenging disorder to treat. There is no single treatment approach that suits all patients. Some patients make it clear that they would never consider taking drugs, others that group therapy is totally unacceptable and some, however hard they try, just never seem to get the hang of a cognitive behavioural approach. The principles that I have described above relate specifically to bulimic behaviour and attitudes. It is self-evident that other aspects of the patient functioning must not be ignored. Interpersonal, work and social problems may all need to be …
