Do all ischemic stroke subtypes benefit from organized inpatient stroke care?

BACKGROUND: There are limited data on the effectiveness of organized stroke care in different ischemic stroke subtypes in the real-world setting. We analyzed the effect of organized stroke care in all stroke subtypes in a longitudinal cohort study using data from the Registry of the Canadian Stroke Network. METHODS: Between July 2003 and September 2007, there were 6,223 consecutive patients with ischemic stroke subtype information by Trial of Org 10172 in Acute Stroke Treatment criteria. Subtypes were categorized as large artery atherosclerotic disease, lacunar, cardioembolic, or other. The a

Bacteroides-dominant gut microbiome of late infancy is associated with enhanced neurodevelopment.

Dysbiosis of gut microbiota has been retrospectively linked to autism spectrum disorders but the temporal association between gut microbiota and early neurodevelopment in healthy infants is largely unknown. We undertook this study to determine associations between gut microbiota at two critical periods during infancy and neurodevelopment in a general population birth cohort. Here, we analyzed data from 405 infants (199 females) from the CHILD (Canadian Healthy Infant Longitudinal Development) Cohort Study. Neurodevelopmental outcomes were objectively assessed using the Bayley Scale of Infant De

Long-term immunogenicity and efficacy of the quadrivalent human papillomavirus vaccine in women with HIV: a longitudinal cohort study.

BACKGROUND: Understanding the impact of the human papillomavirus (HPV) vaccine in women with HIV, who have higher rates of HPV infection and associated disease, is vital to achieving cervical cancer elimination. We aimed to assess the immunogenicity and efficacy of the quadrivalent HPV (qHPV) vaccine up to 9 years in a cohort of women with HIV. METHODS: This longitudinal multicentre cohort study recruited girls and women with HIV aged 9 years and older from 14 clinics providing HIV and gynaecological care across Canada. Participants were administered three doses of qHPV vaccine at 0, 2, and 6

Renal ultrasound after first febrile urinary tract infection in hospitalized children: The ROUTINE prospective observational study protocol.

BACKGROUND: Urinary tract infection (UTI) is a common reason for hospitalization in young children. American and Canadian guidelines recommend universal renal ultrasound after a first febrile UTI to identify genitourinary abnormalities that require further management, despite limited supporting evidence. The Renal ultrasOund after first febrile Urinary Tract INfEction (ROUTINE) study aims to (a) determine the incidence of clinically important and insignificant genitourinary abnormalities on renal ultrasound, (b) describe the cascade of healthcare interventions following the detection of abnorm

Chitinase-3-like Protein 1 Is Associated with Poor Virologic Control and Immune Activation in Children Living with HIV.

Perinatally infected children living with HIV (CLWH) face lifelong infection and associated inflammatory injury. Chitinase-like 3 protein-1 (CHI3L1) is expressed by activated neutrophils and may be a clinically informative marker of systemic inflammation in CLWH. We conducted a multi-centre, cross-sectional study of CLWH, enrolled in the Early Pediatric Initiation Canadian Child Cure Cohort Study (EPIC4). Plasma levels of CHI3L1, pro-inflammatory cytokines, and markers of microbial translocation were measured by enzyme-linked immunosorbent assays. Longitudinal clinical characteristics (viral l

Does effort-cost decision-making relate to real-world motivation in people living with HIV?

INTRODUCTION: Low motivation is frequent in older people with HIV, yet poorly understood. Effort-cost decision-making (ECDM) tasks inspired by behavioral economics have shown promise as indicators of motivation or apathy. These tasks assess the willingness to exert effort to earn a monetary reward, providing an estimate of the subjective "cost" of effort for each participant. Here we sought evidence for a relationship between ECDM task performance and self-reported motivation in a cross-sectional study involving 80 middle-aged and older people with well-controlled HIV infection, a chronic heal

The gut microbiome in pediatric-onset acquired demyelinating syndromes by myelin oligodendrocyte glycoprotein antibody status.

BACKGROUND: Alterations of the gut microbiome have been reported in central nervous system demyelinating diseases. While the gut microbiome in pediatric multiple sclerosis (MS) has been studied, the role of the gut microbiome in other pediatric-onset acquired demyelinating syndromes (ADS) remains unknown. We compared the gut microbiome composition between myelin oligodendrocyte glycoprotein antibody-positive (MOG+) and antibody-negative (MOG-) participants with pediatric-onset ADS. METHODS: Participants aged ≤21 years enrolled in the Canadian Pediatric Demyelinating Disease Network microbiome

Association between changes in parents' and children's adherence with the 24 h movement behaviour guidelines during the COVID-19 pandemic: a national longitudinal study.

Many cross-sectional studies have examined associations between parents' and children's movement behaviours, focusing on single behaviours, despite these behaviours being interdependent. This longitudinal study examined associations between parents' and children's adherence to the Canadian 24 h movement guidelines. An online national survey was conducted with parents of 7-12-year-olds (N = 2257; 51.6% boys) in December 2020, and every 6 months until June 2022 (four waves). Multinomial regression models embedded in generalized estimating equations were used to model associations between changes

Therapies Used by Children With Primary Ciliary Dyskinesia: A Natural History Study.

INTRODUCTION: Primary ciliary dyskinesia (PCD) management has not been systematically evaluated and is largely empirical. METHODS: Pediatric participants with PCD were enrolled in a prospective, longitudinal, multicenter, observational study. Therapies were recorded at annual visits and categorized by type. Age-related trends in prevalence of therapies were described by serial cross-sectional analyses. Generalized estimating equations analyzed covariates affecting prevalence of certain therapies and whether these covariates impacted oral antibiotic courses. RESULTS: A total of 137 participan

Psychological Resilience and Adolescent Sport-Related Concussion.

OBJECTIVES: Our primary objective was to investigate the trajectory of resilience from preinjury to acutely after sport-related concussion (SRC; within 10 days of injury) and to time of return to play (RTP) in adolescents. We further sought to determine whether this trajectory differed between sexes. Secondarily, we investigated the associations between resilience and post-SRC symptom burden and length of recovery. SETTING: A substudy of the multicenter pan-Canadian SHRed Concussions (Surveillance in High Schools and Community Sport to Reduce the Risk of Concussions and their Consequences) pr

Machine Learning Models Can Predict Tinnitus and Noise-Induced Hearing Loss.

OBJECTIVES: Despite the extensive use of machine learning (ML) models in health sciences for outcome prediction and condition classification, their application in differentiating various types of auditory disorders remains limited. This study aimed to address this gap by evaluating the efficacy of five ML models in distinguishing (a) individuals with tinnitus from those without tinnitus and (b) noise-induced hearing loss (NIHL) from age-related hearing loss (ARHL). DESIGN: We used data from a cross-sectional study of the Canadian population, which included audiologic and demographic informati

Clinical and Community Factors' Correlation With NT-proBNP in First Nations: A Cross-sectional Analysis of the CAHHM-FN Cohort.

BACKGROUND: In Canada, Indigenous populations face inequities in cardiovascular care, and have disparate heart failure (HF) outcomes. N-terminal pro-B-type natriuretic peptide (NT-proBNP) is a biomarker that has been reported to predict subclinical left ventricular (LV) dysfunction, clinical HF, and cardiovascular events. Because of disparities in HF outcomes and care between Indigenous and non-Indigenous people in Canada, in this study we sought to understand the unique sociocultural, clinical, and community factors that are associated with circulating NT-proBNP levels. METHODS: We conducted

Secretory IgA modifies the association between early-life gut microbiota trajectories and childhood nonatopic wheeze.

AIMS: Wheezing in childhood is common and evidence is accumulating for the role of the gut microbiome in the development of atopic wheeze. Changes to the early-life gut microbiota and secretory IgA (SIgA) production have been linked to childhood disease; however, their connection to nonatopic wheeze is unknown. The objectives of the present study were to evaluate the relationships between early-life gut microbiota trajectories, SIgA and childhood nonatopic wheeze. METHODS: Early-life gut microbiota, SIgA and child outcome data were collected as part of the Canadian Healthy Infant Longitudinal

The influence of physical activity and sex on carotid artery longitudinal wall motion in younger healthy adults.

Carotid artery longitudinal wall motion (CALM) is a novel preclinical marker for atherosclerosis that describes the axial anterograde and retrograde motion of the intima-media complex. While regular physical activity and sex are known to independently influence arterial stiffness, their roles on axial arterial wall behaviour are unknown. The purpose of this study is to examine whether physical activity and sex impact CALM. We hypothesized that CALM retrograde displacement and total amplitude would be greater in females and active individuals, as a function of arterial stiffness. Fifty-seven yo

Trajectories of Depression, Post-Traumatic Stress, and Chronic Pain Among Women Who Have Separated From an Abusive Partner: A Longitudinal Analysis.

This longitudinal study explored changes in women's health after separation from an abusive partner by characterizing the trajectories of their mental health (depression and post-traumatic stress disorder [PTSD]) and physical health (chronic pain) over a 4-year period. We examined how the severity of intimate partner violence (IPV) affected these trajectories, controlling for selected baseline factors using 5 waves of data collected from a community sample of 309 English-speaking, Canadian women. IPV severity was measured using the Index of Spouse Abuse where women were asked to consider the e
