Catastrophic injuries in high school and college baseball players.

BACKGROUND: There are few epidemiologic studies of catastrophic baseball injuries. PURPOSE: To develop a profile of catastrophic injuries in baseball players and to describe relevant risk factors. STUDY DESIGN: Retrospective cohort study. METHODS: The authors reviewed 41 incidents of baseball injuries reported to the National Center for Catastrophic Sports Injury Research from 1982 until 2002. RESULTS: There were an estimated 1.95 direct catastrophic injuries per year, or 0.43 injuries per 100,000 participants. The most common mechanisms of injury were a collision of fielders (9) or of a b

Mediastinal bronchogenic cyst manifesting as a catastrophic myocardial infarction.

Congenital bronchogenic cysts of the lung and mediastinum develop from the ventral foregut during embryogenesis. These cysts are often incidental radiologic findings in adults, but patients can be seen with symptoms of chest pain, cough, dyspnea, or any combination of these. Acute presentations are unusual and have rarely been reported. We present the unique case of a 36-year-old man seen with an acute coronary syndrome and sudden hemodynamic collapse. The patient sustained a massive and ultimately fatal myocardial infarction, compression of the left main coronary artery by a bronchogenic cyst

Catastrophizing, anxiety and pain during dental hygiene treatment.

OBJECTIVES: This study examined the relations between catastrophizing, dental anxiety, and pain during dental hygiene treatment. METHODS: Participants were 78 (32 men, 46 women) consecutive referrals to the Dalhousie University Dental Clinic. All patients were scheduled for a scaling procedure performed by senior dental hygiene students. Following treatment, patients completed the Pain Catastrophizing Scale and the Dental Anxiety Scale - Revised, and were asked to rate the degree of pain they experienced during the scaling procedure. RESULTS: Regression analyses revealed that age and the rum

Factor structure, reliability, and validity of the Pain Catastrophizing Scale.

The Pain Catastrophizing Scale (PCS; Sullivan et al., Psychol. Assess. 7, 524-532, 1995) has recently been developed to assess three components of catastrophizing: rumination, magnification, and helplessness. We conducted three studies to evaluate the factor structure, reliability, and validity of the PCS. In Study I, we conducted principal-components analysis with oblique rotation to replicate the three factors of the PCS. Gender differences on the original PCS subscales were also analyzed. In Study II, we conducted confirmatory factor analyses to evaluate the adequacy of fit of four alternat

Catastrophic consequences secondary to psychotropic drugs, Part 1.

The use of psychotropic drugs has dramatically changed the lives of many; yet as often occurs with technological advances, negative outcomes can be encountered. Potentially fatal reactions to neuroleptic drugs and serotonin-enhancing agents (e.g., antidepressants) have been presented in Part 1, with the intent of alerting and informing psychiatric nurses. Part 2 will complete this series on catastrophic consequences of psychotropic drugs with a discussion of agranulocytosis and lithium toxicity.

The impact of catastrophic beliefs on functioning in chronic fatigue syndrome.

This study investigated the association between catastrophic beliefs and disability in the context of Chronic fatigue syndrome (CFS). A sample of 282 CFS sufferers were asked about the consequences of pushing themselves beyond their present physical state. Responses were coded into catastrophic or non-catastrophic categories. While not differing on the length of illness or psychological adjustment, subjects demonstrating catastrophic responses evidenced significantly higher levels of fatigue and were more disabled in terms of their ability to work both in their normal occupation and around the

Catastrophic visual loss due to Cryptococcus neoformans meningitis.

We have reviewed our experience with 17 of our own patients with cryptococcal meningitis and 32 cases from the literature. Although this complication is an uncommon event, patients with cryptococcal meningitis may develop visual loss in the absence of other ocular lesions (endophthalmitis or cryptococcomas in the visual pathway) that could explain the visual symptoms. There are 2 distinct patterns of visual loss: rapid visual loss and slow visual loss. Rapid visual loss is characterized by onset of profound visual loss over a period as short as 12 hours before or early in the course of therapy

The relation of clinical catastrophes, endogenous oxalate production, and urolithiasis.

A dose-related toxicity syndrome of renal, cerebral, and liver dysfunction; metabolic acidosis; and deposition of calcium oxalate crystals in tissues is reported in association with various apparently unrelated treatments for a wide range of diseases. The parenteral nutrient xylitol, the hyperosmolar agent glycerol, the polysorbate emulsifiers (e.g., in vitamin E preparations), the anesthetic methoxyflurane, and possibly the experimental hypoglycemic agent dichloroacetate all produce a toxicity syndrome very similar to that of ethylene glycol poisoning. In long-term, high-dose oral toxicity st

Potentially catastrophic bleeding disorders. Approach to diagnosis and management.

Clinical and laboratory evaluation of severe bleeding can detect the presence of an intrinsic or acquired coagulation disorder. The three most common inherited coagulation disorders are factor VIII deficiency (hemophilia A), factor IX deficiency (hemophilia B), and von Willebrand's disease. Vitamin K deficiency, liver disease, and disseminated intravascular coagulation are the most common acquired disorders. A thorough clinical history is crucial to diagnosis. Screening tests that measure prothrombin time, partial thromboplastin time, thrombin time, and platelet count permit initial classifica

Dyspneic-fear and catastrophic cognitions in hyperventilatory panic attacks.

The tenability of cognitive explanations of the experience of fear during panic attacks (viz. Ley's misattribution-of-symptoms hypothesis and Beck's and Clark's catastrophic-misinterpretation-of-symptoms hypotheses) is seriously questioned by findings from three independent lines of research: (a) Wolpe and Rowan's observation that catastrophic cognitions follow fear, (b) Rachman, Levitt and Lopatka's reports of panic attacks without fearful cognitions, and (c) reports of panic attacks during sleep occurring predominately during non-dreaming stages of sleep. Recognition of these findings led Le

From catastrophizing to catalyzing: does pain catastrophizing modulate the beneficial impact of open-label placebos for chronic low back pain? A secondary analysis.

Chronic back pain (CBP) is a global health problem with significant health and economic consequences. Traditional analgesics are often no better than placebo, highlighting the need for biopsychosocial approaches. Open-label placebos (OLPs), administered with patient consent, offer a promising alternative. Existing research has mainly focused on the effects of OLP treatments on patient-reported outcomes. In a previous randomized controlled trial (RCT), we investigated whether an OLP treatment improves subjective and objective outcomes such as spinal mobility in CBP patients. The analysis showed

The clinical significance of low complement levels in patients with catastrophic antiphospholipid syndrome: A descriptive analysis of 73 patients from the "Catastrophic antiphospholipid syndrome registry".

OBJECTIVES: To explore the prevalence and clinical significance of low complement levels in patients with catastrophic antiphospholipid syndrome (CAPS). METHODS: We reviewed data from the "CAPS Registry" on C3 and/or C4 complement plasma protein levels during acute CAPS episodes. Patients were classified into those with low and normal complement levels. Data on clinical presentation, with special focus on thrombotic microangiopathy (TMA) features, diagnosis of systemic lupus erythematosus (SLE), and antiphospholipid antibody (aPL) profile were reviewed. The chi-square exact test was performed

Changes in pain catastrophizing predict later changes in fibromyalgia clinical and experimental pain report: cross-lagged panel analyses of dispositional and situational catastrophizing.

INTRODUCTION: Fibromyalgia (FM), characterized by wide-spread diffuse pain and sensory abnormalities, is associated with elevated indices of distress and pain-related catastrophizing compared to both pain-free samples and those with chronic pain conditions. Catastrophizing is a pervasive negative mental set, and is a strong predictor of negative pain-related outcomes such as clinical pain intensity, and physical disability. Situational catastrophizing, measured in the context of experimentally-induced pain, is strongly related to enhanced pain sensitivity, a core aspect of the pathophysiology

Do firms with unique competencies for rescuing victims of human catastrophes have special obligations? Corporate responsibility and the AIDS catastrophe in sub-Saharan Africa.

Firms possessing a unique competency to rescue the victims of a human catastrophe have a minimum moral obligation to devote substantial resources toward best efforts to aid victims. The minimum amount that firms should devote to rescue is the largest sum of their most recent year's investment in social initiatives, their five-year trend, their industry's average, or the national average. Financial exigency may justify a lower level of investment. Alternative social investments may be continued if they have an equally compelling rationale. These duties apply to the global pharmaceutical compani
