Cauda Equina Syndrome in a Lactating Mother - A Safe Treatment Approach.

Cauda equina syndrome is widely considered as a surgical emergency. The cause of cauda equina syndrome usually is a large central lumbar disc herniation, prolapse or sequestration. Decompression at the earliest has been suggested by many authors but the planning of surgical management becomes challenging when the patient is a breastfeeding mother. Fear of harmful effects of the drugs (administered in the mother) on the infant, always confuses clinicians regarding the treatment approach. So the multidisciplinary approach is necessary with involvement of anaesthetist, paediatrician and also a gy…

[Dorsal extrusion of intervertebral disc as a cause of cauda equina syndrome].

We have presented a case of rare dorsally sequestrated lumbar disc herniation manifesting as cauda equina syndrome. The patient was admitted to the Neurological Department of Canton Hospital Zenica due to urinary retention and weakness in both lower extremities. Magnetic resonance imaging showed a compressing mass located in the dorsal extradural space at the L2-L3 level. An extruded intervertebral disc was found intraoperatively. The decompression was followed by good recovery.

Cauda equina syndrome caused by a migrated bullet in dural sac.

Cauda equina syndrome (CES) is a serious neurological condition and the most common cause is a central disc herniation. Migration of a bullet down the spinal canal is uncommon. In this report, the authors present an unusual case of cauda equina syndrome caused by a migrated bullet in dural sac. The patient's cauda equina syndrome was related to possible compression of cauda equina resulting from both bone fragments in L1 level and bullet itself in S2 level. The patient's symptoms and neurological deficits resolved considerably after surgery. In our opinion, it is important to pay attention to …

Medical screening and evacuation: cauda equina syndrome in a combat zone.

STUDY DESIGN: Resident's case problem. BACKGROUND: Cauda equina syndrome (CES) is a rare, potentially devastating, disorder and is considered a true neurologic emergency. CES often has a rapid clinical progression, making timely recognition and immediate surgical referral essential. DIAGNOSIS: A 32-year-old male presented to a medical aid station in Iraq with a history of 4 weeks of insidious onset and recent worsening of low back, left buttock, and posterior left thigh pain. He denied symptoms distal to the knee, paresthesias, saddle anesthesia, or bowel and bladder function changes. At the…

[Use of rechargeable stimulators in 2 cases of cauda equina syndrome].

The first of the 2 cases of cauda equina syndrome we report occurred following phenolization of sacral roots after a series of operations for Hirsprung disease. The second occurred after trauma from a bullet that hit the spine. Fecal and urinary incontinence and neuropathic pain in the lower extremities were present in both cases. After various treatments failed, both patients were given dual spinal stimulation for integrated treatment of both incontinence and neuropathic pain. We applied a novel approach, connecting both stimulators to a single rechargeable generator, reducing medium- and lon…

[Dibucaine for spinal anesthesia is a probable risk for cauda equina syndrome].

A 64-year-old man was scheduled for transure thral resection of the prostate. The patient's medical history showed borderline diabetic state and two uncomplicated surgeries under spinal anesthesia. Spinal anesthesia was performed at the L 3/4 interspace using hyperbaric 0.24% dibucaine 2.2 ml, which was followed by general anesthesia because the anesthesia level had spread only to the lower left side of the body. On the next day, he complained of difficulty of defecation and urination combined with hypesthesia around the anus, which was diagnosed as cauda equina syndrome. The symptoms had not …

Vacuum disk and facet phenomenon in a dog with cauda equina syndrome.

A 7-year-old castrated male Rottweiler was evaluated for an 18-month history of bilateral hind limb lameness, signs of pain on rising, and mild ataxia. A gas-filled lumbosacral disk space (vacuum disk phenomenon) was revealed by use of computed tomography. Additional smaller gas bubbles were in the periphery of the disk and between the degenerated L5-L6 dorsal articular facets (vacuum facet phenomenon). Vacuum disk and facet phenomena have been described in humans with degenerative disk and vertebral articular facet joint disease; gas attracted from surrounding tissues accumulates within cleft

[Autonomic neuropathy with cauda equina syndrome in a patient with diabetes mellitus].

In significant number of diabetic patients are observed numerous complications during the disease onset and course. One of the most frequent is diabetic polyneuropathy that can, during its course, assume such types and imitate various diseases that can by the intensity of revealance be prominent and couse significant differential-diagnostic difficulties, especially if some forms of polyneuropathy are more rarely considered. The patient with poorly regulated diabetes, in whom the couse of disturbances was unknown for the longer period of time was presented in this study. Leading clinical phenom

Cauda equina syndrome. Implications for the orthopaedic nurse in a clinical setting.

Cauda equina syndrome (CES) is a presentation of signs and symptoms that indicate nerve compression involving one or more of the nerve roots in the lumbar spine. The Clinical Nurse Specialist working in a general orthopaedic practice must become familiar with the presentation of CES to appropriately assess the patient and alert the physician. Prompt and accurate care of the patient is critical when cauda equina syndrome is suspected to minimize permanent nerve damage. This article describes the syndrome, its signs and symptoms, basic triage techniques, plan of care, and recovery for the patien…

MRI demonstration of arachnoiditis in cauda equina syndrome of ankylosing spondylitis.

The cauda equina syndrome is a rare but well-recognised complication of longstanding ankylosing spondylitis, usually presenting when the joint disease is quiescent. The clinical and radiological findings in a patient with only a 3-year history, in whom the onset of joint and neurological symptoms was apparently simultaneous, are presented. MRI revealed characteristic expansion of the lumbar spinal canal with scalloping of the pedicles, laminae and spinous processes, related to numerous posterior dural diverticula. The quantity and extent of such diverticula are unusual. We demonstrated adheren…

Magnetic resonance imaging of the cauda equina in Guillain-Barré syndrome.

We report three patients (two children and one adult) with Guillain-Barré syndrome and magnetic resonance imaging evidence of gadolinium enhancement of the cauda equina and lumbar nerve roots. All three patients exhibited symmetric ascending paralysis and areflexia, and two (one child, one adult) suffered urinary incontinence and retention. Similar enhancement has been observed in patients with chronic inflammatory demyelinating polyneuropathy and suggests proximal nerve inflammation. Magnetic resonance imaging in Guillain-Barré syndrome and chronic inflammatory demyelinating polyneuropathy ma

Cauda equina syndrome in patients undergoing manipulation of the lumbar spine.

Cauda equina syndrome has been implicated as a potential complication of spinal manipulation. A review of the literature from 1911 to 1989 revealed ten reported cases of cauda equina syndrome in patients undergoing manipulation without anesthesia. This article presents three new cases where a temporal association was found between the onset of cauda equina symptoms and lumbar manipulation. The type of manipulation administered and the relationship between the treatment and symptoms is reviewed. In each of these cases both the chiropractic practitioner and the emergency room physician failed to…

Incidence and long-term patient reported outcome measures of patients with radiological cauda equina compression but without clinical features of cauda equina syndrome.

BACKGROUND: Cauda equina syndrome (CES) is a rare spinal emergency. Paradoxically, in the British NHS, suspected CES requiring an emergency magnetic resonance imaging constitutes one of the commonest reasons for acute referrals to neurosurgery/spine surgery. Further acute referrals also occur with the reverse scenario, when radiological evidence of cauda equina compression is not accompanied by the clinical red flags of the syndrome. The understanding of CES, particularly in cases with radiological compression but lacking clinical symptoms, remains limited. This study aims to explore the incid…

Ischaemic cauda equina syndrome: an atypical presentation of acute aortic occlusion .

Ischaemic cauda equina syndrome (ICES) is an uncommon but severe condition resulting from impaired blood supply to the cauda equina, often due to vascular events such as acute aortic occlusion (AAO). While AAO usually affects distal organs, it may compromise the lumbar radicular arteries, causing spinal cord ischaemia. This case describes a middle-aged patient who presented with back pain, urinary incontinence and paraplegia. Initial lumbar spine MRI was unremarkable; however, a loss of flow void in the abdominal aorta led to CT angiography, which revealed extensive infrarenal aortic thrombosi…
