A paracondylar process and early onset cervical osteoarthritis in a California Amerind adult.

Burial 65 is a 25-39-year-old male from a hunter-gatherer California Amerind collection with a large bone spur called a paracondylar process. The process measures 11.92mm in width and 13.32mm in length. It is located on the left anteromedial region near the foramen magnum and articulates with a pseudarthrosis to the atlas. The atlas, too, has a pseudarthrosis1 on the left transverse process. The paracondylar process may be the result of caudal occipitalization, which is a congenital variant that results from a lack of fusion of the occipital bone to the foramen magnum. Trauma is another possib

Unilateral lag screw fixation of isolated non-union atlas lateral mass fracture: a new technical note.

We describe a novel and new technique of posterior unilateral lag screw fixation of non-union atlas lateral mass fracture. A 46-year-old man presented with cervical pain and tenderness after a vehicle turn over accident and he was diagnosed to have left atlas lateral mass fracture. He was initially treated by immobilization using Minerva orthosis. About 2 months later, he developed severe neck pain and limitation of motion and thus he was scheduled for operation due to non-union atlas lateral mass fracture. A 28 mm lag screw was inserted under anterior-posterior and lateral fluoroscopic views.

Mortality From Combined Fractures of the Atlas (C1) and Axis (C2) in Adults.

Study design A retrospective case report of all upper cervical spine fractures diagnosed by CT imaging between 01/01/2013 and 31/12/2015 in NHS Greater Glasgow and Clyde, Scotland. Objective To compare the mortality following combined fractures of the atlas and axis to that of isolated fractures of either vertebra. Background The mortality from axis fractures is well documented in the literature. However, a combined fracture of the atlas and axis is seldom reported, leading to relatively unknown outcomes and mortality. Methods A total of 171 patients with atlas and/or axis fractures. Thirty-th

Combined occipitoatlantoaxial hypermobility with anterior and posterior arch defects of the atlas in Pierre-Robin syndrome.

Combined occipitoatlantoaxial instability of the cervical spine occurred in a 8-year-3-month old boy with Pierre-Robin syndrome. He also had failure of ossification of both the anterior and posterior arches of the atlas. An in situ fusion from the occiput to C2 restored cervical spine stability. This case is discussed in relation to other hypermobility and instability syndromes of the cervical spine.

[Axial mesenchyme structure in the anlage of the human vertebral column. Light microscopy study of a human embryo of 9 mm CRL (Carnegie stage 16)].

The anlage of the cervical vertebral column of a human embryo has been investigated (9 mm CRL, Carnegie stage 16). The nuclear density in the axial mesenchyme increases rhythmically from cranial to caudal. This phenomenon superimposes a metameric pattern on the blastema. Furthermore, cell formations are shaped by the orientation of the mesenchymal cells. The name 'formationes quasi distensae' is proposed for this system. The anlage of the atlas shows a distinct mesenchymal anlage of a vertebral body. The conclusion is drawn that the dens axis is predominantly formed out of the anlage of the at

[Are there therapy algorithms in isolated and combined atlas fractures?].

AIM: Injuries of the atlas are always a challenge in diagnostics and therapy. Different clinical manifestations, inconspicuous neurological results, uncertain findings of radiological diagnostics and possible accompanying injuries require individual therapeutic concepts. METHODS: Patients with injuries of C1 and C2 seen between 2001-2007 were evaluated and especially the morbidity and treatment of the C1-injured patients were verified. To systematise the injuries, a subdivision in isolated and combined trauma took place. Furthermore, the post-traumatic as well as post-therapeutic accompanying

[Isolated fractures of the atlas].

PURPOSE OF THE STUDY: To present the current tends in the diagnosis and management of isolated atlas fractures based on the retrospectively evaluated group of patients with this trauma. MATERIAL: In the period from 1995 to 2002, we treated 486 injuries to the cervical spine at our department. Out of these, 19 patients sustained an isolated fracture of the first cervical vertebra. This group consisted of 12 men and seven women; the average age was 46.6 years. Neurological findings in 18 patients were classified as Frankel E and, in one, as Frankel A. The causes of injury included a fall from h

Transoral approach to the cervical spine: report of four cases.

The transoral approach to the upper cervical spine is an established but little used route, offering excellent access with good wound healing, to lesions of the bodies of the atlas, axis and upper part of the third cervical vertebra. The authors report four cases which demonstrate the value of the procedure.

[Roentgenologic diagnosis of the cervical spine in childhood].

The degree of lordosis and mobility of the cervical spine, the "formation of steps" at the top end of the cervical spine, the Atlas-Dens distance, the diameter of the bony cervical spine canal and the breadth of the retropharyngeal and retrotracheal space were measured on 360 lateral radiograms of the cervical spine in the median position, at maximum flexion and maximum extension. The subjects were 120 children aged three to 14 years. The discussion of the results draws attention to their practical value and to the importance of functional radiographical examination of the cervical spine durin

A congenital anomaly of the atlas as a diagnostic dilemma: a case report.

OBJECTIVE: The purpose of this case report is to draw attention to the differences between a Jefferson fracture and a congenital anomaly of the anterior and/or the posterior arch of the atlas. CLINICAL FEATURES: A 42-year-old woman visited the chiropractic practice complaining of headache, neck pain, dizziness, and numbness in both of her arms after she fell vertically and directly on her head twice on a playground. Before this fall, she had no such complaints. After taking x-rays of the cervical spine, a Jefferson fracture was suspected. INTERVENTION AND OUTCOME: After computed tomography a

Traumatic injuries of the c1/c2 complex: computed tomographic imaging appearances.

Cervical spine fractures occur in 1 to 3% of trauma cases and account for two-thirds of spinal cord injuries. Injury to the C1/C2 complex accounts for 19 to 25% of cervical spine injuries. Fractures of C2 occur most frequently, 55% of which involve the odontoid peg. The unique anatomical relationship between the atlas and axis produces a variety of injury patterns not seen elsewhere in the spine. Numerous developmental anomalies also occur which can mimic trauma. Well-described fracture patterns include posterior arch fractures of the atlas, Jefferson type fractures, fractures of the odontoid

Transposition of the vertebral artery after a unilateral c1 lateral mass fracture: a case report and review of the literature.

In any cervical spine trauma involving dislocation of the facet joints or transverse foramen, a concomitant injury of the vertebral arteries must be excluded. Early diagnosis of an arterial occlusion is important because therapeutic intervention can prevent further complications, such as irreversible vertebrobasilar ischemia. Conservative and surgical treatment strategies are chosen depending on patient symptoms, fracture morphology, and grade of vascular injury. The benefit of anticoagulant medication after blunt injuries of the vertebral arteries in preventing thromboembolic events remains a

Unilateral atlantal mass hypertrophy in acromegaly. Case report.

The authors report an extremely rare case of a patient with acromegaly who had unilateral enlargement of an atlas facet resulting in cord compression and progressive quadriparesis. Although unilateral atlas facet enlargement has been identified in the literature in cases of spondylosis and as a component of congenital malformation, its association with acromegaly has not previously been reported. Resection of the offending facetal bone and atlantoaxial fixation resulted in rapid neurological recovery.

[Fracture-separation of the atlas. A case].

A case of fracture-separation of the atlas is presented. The diagnosis, suspected on the antero-posterior open-mouth film and the frontal tomograms, was confirmed by computed tomography which showed the double fracture of the anterior and posterior arches of the atlas and demonstrated that the transverse ligament, factor of stability of this fracture, was intact.

[Cranio-vertebral junction anomalies and osseous dysplasias (author's transl)].

The cartilaginously preformed occipital region of the cranium is ontogenetically a part of the backbone. The cranio-vertebral junction is unique among the other junctional areas of the backbone: 1. It has not yet been proved that different combinations of cranial and caudal junction anomalies are not combined with cervical junction anomalies. 2. The assimilations of the atlas which are considered to be progressive and regressive variants and the so-called manifestations of the occipital vertebra, i.e. diametrically opposed members of a developmental line occur together with a frequency that is

Atlanto-axial approach for cervical myelography in a Thoroughbred horse with complete fusion of the atlanto-occipital bones.

A 2-year-old Thoroughbred gelding with clinical signs localized to the first 6 spinal cord segments (C1 to C6) had complete fusion of the atlanto-occipital bones which precluded performing a routine myelogram. An ultrasound-assisted myelogram at the intervertebral space between the atlas and axis was successfully done and identified a marked extradural compressive myelopathy at the level of the atlas and axis, and axis and third cervical vertebrae.
