We report two cases of rare direct cervical vertebro-venous fistula (VVF) presenting with chronic radiculopathy as part of the symptom complex. We describe the MRI and intra-arterial angiography findings before and after successful embolisation. These demonstrate occlusion of the fistula with collapse and thrombosis of the draining extradural venous plexus and, in one case, resolution of MRI signal abnormality in the cervical spinal cord.

OBJECTIVE: To alert athletic trainers to the importance of recognizing the signs and symptoms of central cord syndrome.
BACKGROUND: A 15-year-old high school wrestler was found lying supine on the mat after sustaining a hyperextension injury to his neck while drilling during practice, complaining of numbness, tingling, and a burning sensation in all 4 extremities. Touching the extremities elicited an extreme burning sensation. After in-line stabilization of the cervical spine was performed, palpation of the spinous processes elicited tenderness and an increase in pain. Six weeks before the in

We have chosen six illustrations showing how much vital information can be obtained from median nerve SEPs during the first 24 hours in coma. With avulsion of brachial plexus roots there was loss of SEPs at the cervical cord and the scalp from the affected side. In a severe injury of the cervical cord there was preservation of brachial plexus potentials, while SEPs at the cervical cord were absent. After critical deterioration in a case of repeated subarachnoidal hemorrhage, scalp SEPs with very short latency occurred, which is a finding suggestive of destruction of cortical SEP generators her

An 11-year-old Belgian shepherd presented with progressive right thoracic limb lameness. Computed tomography and magnetic resonance imaging revealed a brachial plexus neoplasm and cytology was suggestive of a sarcoma. The patient underwent forequarter amputation and hemilaminectomy of the fifth cervical to first thoracic vertebrae with nerve root transection. Histopathology confirmed extraskeletal osteosarcoma. Twelve months post-operatively, the dog was exercising normally, and computed tomography did not identify local recurrence or metastatic disease. This is the first recorded case of cani

A thoracic outlet syndrome is only rarely caused by osseous anomalies. Instead, it is a soft-tissue disease, where the C8 and T1 roots or the proximal inferior trunk of the brachial plexus are compressed by fibromuscular anomalies between the inferior cervical spine and the first rib or pleura. Thoracic outlet syndrome is a clinical diagnosis. If surgery is recommended, it should be concentrated on the fibromuscular anomalies and not on the first rib. Transaxillary removal of the first rib carries an important risk of serious complications. Among these, brachial plexus injury is the most frequ

A 21-year-old man presented with right-hand intrinsic muscle wasting and right-hand tremor on neck flexion. MR scan of cervical spine was normal in the neutral position, but on neck flexion showed anterior shift and flattening of the cervical cord, with prominent posterior epidural space and engorged epidural venous plexus. We diagnosed Hirayama disease and his condition stabilised with a neck collar and physiotherapy.

Cervical lymphadenectomy is a common procedure for thyroid cancer. Some of the complications are congruent with the complications of thyroid surgery, in particular recurrent laryngeal nerve paresis and hypoparathyroidism as well as bleeding and wound infection. Specific complications of lateral cervical lymph node dissection are injuries to the accessory, phrenic and hypoglossal nerves, and the cervical plexus trunk and injuries, the salivary glands, and the lymphatic system, especially the ductus thoracicus. Most of these complications are very rare with an incidence of <1%. Profound anatomic…

First-bite syndrome consists of the appearance of pain in the parotid at the beginning of mastication, due to damage to the cervical sympathetic chain or the sympathetic plexus innervating the parotid gland. Clinical presentation in a patient who has undergone surgery of the parapharyngeal space suggests the diagnosis. We report here the case of a patient who presented first-bite syndrome after being operated on for a cervical sympathetic chain schwannoma.

The history of the patient is very important. In cervical syndroms typical pain in neck, shoulder and arm are present, often combined with occipital headache and paraesthesias. The symptoms are more pronounced during the night, while waking-up and after sudden head movements. Radiological changes mainly occur at C 4-C 7, but are not pathegnomonic. Hearing loss is rather frequent, usually one-sided and non fluctuating. The audiogram is not characteristic for its cervical origin. In 80% recruitment is present, tinnitus occurs in 30--60% of the patients, but displacusis and fullness of the ear do

Hirayama disease is a rare juvenile amyotrophy that is often misdiagnosed as an unrelated, relentlessly progressive disease. We present the case of an 18-year-old man who presented with weakness and atrophy of the right forearm and hand. Dynamic cervical magnetic resonance imaging was used, revealing the classic findings of epidural venous plexus dilation and anterior displacement of the dural sac. In addition, dilation of the external vertebral venous plexus was visualized. We discuss the clinical utility of dynamic magnetic resonance imaging and the underlying pathophysiology of these findin

Severe traction injuries may damage the brachial plexus at any level from the spinal cord to the axillary outlet. Investigation aims to determine the level of the injury for each of the nerves, trunks or cords, with particular reference to obtaining firm evidence of any intradural (pre-ganglionic) damage. We report the results of computerised axial tomography of the cervical spine in comparison with conventional myelography and with surgical exploration of the plexus. CT scanning with contrast enhancement greatly improves diagnostic accuracy, particularly at C5 and C6 root levels.

CONTEXT: Cervical root avulsion secondary to traumatic plexus injury is a rare etiology of superficial siderosis (SS) of the central nervous system (CNS). We describe the case of an isolated progressive compressive myelopathy revealing this complication and discuss the pathogenesis of such a presentation, its clinical and imaging peculiarities with a literature review.
FINDINGS: We report on the case of a 48-year-old man with history of left brachial plexus injury at the age of 2 years. Since the age of 38 years, he had presented with a progressive paraplegia, bladder and erectile dysfunction

It has been estimated that 30% to 80% of the population has at least one tension-type headache each year. Tension-type headaches can be located in any region of the head, which can confuse a differential diagnosis with temporomandibular joint disorders and cervical dysfunction. The roles of temporomandibular joint disorders and cervical dysfunction in tension-type headache are evaluated. Definitions, pathophysiology, and treatment considerations for each are discussed.

BACKGROUND: Tetraplegia caused by cervical spinal cord injury is devastating for patients and represents a significant public health problem in both developed and developing countries. Improved functional outcomes after nerve transfers are increasingly reported in the literature, but thus far, no options exist for injuries above the C5 level.
CASE DESCRIPTION: We report the cases of 2 patients with C4 spinal cord injury, American Spinal Injury Association A, who underwent successful bilateral spinal accessory nerve transfers, on 1 side to the triceps nerve with long intervening sural graft an

OBJECTIVE: The purpose was to identify the prevalence, causative factors, injury types, and associated injury patterns in multitrauma patients who sustained brachial plexus injuries.
METHODS: A retrospective review of a prospectively collected and computerized database and a chart review were performed.
RESULTS: Brachial plexus injuries were identified in 54 of 4538 (1.2%) patients presenting to a regional trauma facility. Young male patients predominated. Motor vehicle accidents were the most frequent cause overall, but only 0.67% of such accidents resulted in plexus injuries. Conversely, 4

Lipomatous tumours are uncommon in the paediatric population and lipomas arising in the cervical/supraclavicular region are particularly rare. Tumours of this location may affect surrounding vasculature as well as the brachial plexus. Complete surgical excision remains the mainstay of treatment for symptomatic lipomas. Surgery allows for symptomatic relief as well as histological confirmation of the diagnosis, as excluding malignancy is an important component of management in lipomatous tumours. We describe the case of a patient in early childhood who presented with a slowly progressive mass al

Currently the posterior approach undertaken to perform cervical hemilaminectomy and foraminotomy provides sufficient exposure to treat the majority of lateral soft-disc herniations or osteophytes causing radiculopathy. Limitations imposed by the surgical field, however, often necessitate excessive retraction of the nerve root and epidural venous plexus, which may potentially exacerbate a preexisting radiculopathy or increase intraoperative blood loss. Partial resection of the inferior pedicle augments exposure and enlarges the neural foramen, thus facilitating decompression while minimizing ma

INTRODUCTION: Kennedy's disease (KD) is a rare X-linked recessive motor-neuron neuropathy also known as Spinal and Bulbar Muscular Atrophy (SBMA).
CASE REPORT: We report a case of Kennedy's disease (KD) with acute laryngeal stridor and dysphagia whose anesthesiologic management required a tracheotomy. The surgical procedure was conducted under local anesthesia performed by ultrasound-guided superior laryngeal nerve block and superficial cervical plexus block using 2% lidocaine. The surgical procedure was well tolerated without any discomfort reported from the patient.
CONCLUSIONS: In KD loca…

The shoulder is the most mobile joint in the body. Because it serves as a way station for the nerves supplying the upper limb, it creates a potential for nerve lesions that may be caused or significantly influenced by the complex dynamics of the shoulder girdle. This article presents the most commonly encountered lesions as well as an algorithm for their diagnosis and treatment.
