Giving Resources to lOw-income Children to Enhance RecoverY (GROCERY): Protocol for a randomized control trial.

BACKGROUND: Social and structural factors drive health disparities in children. Hospitalization of a child is stressful for families, especially for low-income families who may experience exacerbation of food insecurity and other financial hardships. There is a need to evaluate interventions to support low-income families after hospitalization to improve child health and family well-being and ultimately to reduce health disparities. OBJECTIVES: In this randomized control trial (RCT), we will compare the effectiveness of a post-discharge financial support intervention with standard resources o

Disseminated BCG infection in a child with multifocal osteomyelitis due to STAT1 LOF variant and primary immunodeficiency disease was significantly improved after anti-tuberculosis treatment: a case report.

BACKGROUND: This was a rare case where the diagnosis was not obvious during treatment, but the treatment was effective after diagnosis. An infant with recurrent fever was considered for systemic multiple osteomyelitis after two surgical biopsies. After a third operation to take a lymph node biopsy, the patient was finally diagnosed as having disseminated Bacille Calmette-Guerin(BCG) disease caused by BCG vaccination. After diagnosis, the child was effectively treated with anti-tuberculosis therapy. CASE DESCRIPTION: A 2-month-old female patient was hospitalized twice for fever and surface mas

Parental Coronavirus Disease 2019 Testing of Hospitalized Children: Rethinking Infection Control in a Pandemic.

BACKGROUND: Novel coronavirus disease 2019 (COVID-19) poses a challenge to infection control within hospital systems. Asymptomatic children and their caretakers carry the risk of silently spreading infection in pediatric emergency departments and hospital units. Our current knowledge is evolving, and infection control measures are frequently changing depending on new emerging data. METHODS: We conducted a point-prevalence study to assess SARS-CoV-2 Real-Time Polymerase Chain Reaction (SARS-CoV-2 RT-PCR) results of hospitalized children, their asymptomatic caretakers, and caretaker-child test

Pyloric stenosis after gastroschisis closure.

Hypertrophic pyloric stenosis is a very common surgical problem in infants. It occurs most often in otherwise well babies with normal gestation and birth history. Rarely, pyloric stenosis has been described in babies with history of prior abdominal surgery. Below we discuss the management of hypertrophic pyloric stenosis in a child who remained hospitalized, recovering from repair of a congenital abdominal wall defect.

[Pulmonary interstitial emphysema in a 15-month-old child treated with high frequency oscillatory ventilation].

Pulmonary interstitial emphysema (PIE) is a known complication of mechanical ventilation of preterm infants. PIE is associated with high mortality and treatment is controversial as both surgery and conservative treatment are possibilities. We present a case with a 15-month-old previously healthy child, hospitalized with severe respiratory distress. A computed tomography (CT) of the chest revealed localized PIE. He was successfully treated.

Breastfeeding: Neglect or Excessive Support? A Case Report of Child Abuse by a Negligent Heroin-Dependent Mother.

BACKGROUND: Breastfeeding is one of the best ways to promote, develop, and secure the health of infants. Child abuse is one of the most common and most important problems in the world, and one of the factors that increase its incidence is substance dependency of the parents. Breastfeeding beyond the normal age range can be harmful to the health of the mother and baby, and may represent a pathological parent-child relationship. CASE REPORT: A little girl, who was breastfed until the age of 8 years by her heroin-dependent mother, was hospitalized in a child and adolescent psychiatric ward due t

Pleural Effusions Identified by Point-of-Care Ultrasound Predict Poor Outcomes in Decompensated Cirrhosis.

Chronic liver disease (CLD) may be associated with pleural effusions (PEs). This article prospectively evaluates whether detection of PEs on thoracic ultrasound (TUS) at the bedside independently predicts mortality and length of stay (LOS) in hospitalized patients with a decompensated CLD. A total of 116 consecutive inpatients with decompensated cirrhosis underwent antero-posterior chest radiographs (CXR) and TUS to detect PEs. Their median age was 54 y (interquartile range, 47-62), 90 (70.6%) were male, and 61 (52.6%) fell into the Child-Pugh class C categorization. TUS identified PEs in 58 (

Molindone hydrochloride treatment of hospitalized children with conduct disorder.

Treatment-emergent symptoms and behavioral changes were assessed during an 8-week double-blind study comparing molindone and thioridazine in 31 aggressive hospitalized children (ages 6-11). Molindone was found to be as effective as thioridazine in this sample. Adverse effects differed (nonsignificantly) for the two groups, with more sedation in the thioridazine-treated children. Clinical records from 6 adolescent inpatients treated with molindone were reviewed as a comparison group. Molindone is a relatively safe neuroleptic for child and adolescent inpatients because of its short half-life an

Effects of parents' and pediatricians' worry concerning severe gastroenteritis in early childhood on later disturbances in the child's behavior.

Probands who had been hospitalized for gastroenteritis between the ages of 2 and 5 years were studied at the ages of 8 to 12 years. More conduct problems and a tendency to excessive dependence were found (1) in probands who were severely ill by objective criteria than were others and (2) in probands whose parents were specifically worried that their child might die as a result of the illness. General worry in the parents had different consequences. Other data suggest that pediatricians, because they are no longer worried about the illness after the child is discharged, may be failing to take a
