The case history of a 27 year old male ex-intravenous drug user who presented with haemoptysis and indistinct lung shadowing is reviewed. During a 3 week period of investigation his lung shadowing increased dramatically and it became apparent that he had metastases from chorionic gonadotropin-producing testis cancer which had undergone spontaneous regression of the primary tumour. Fifteen women with unruptured tubal pregnancies diagnosed by laparoscopy and ultrasound were treated with danazol 400 mg, injected directly into the affected tube. Study inclusion criteria were (i) a rising or plateauing level of serum beta human chorionic gonadotropin (hCG), (ii) a tubal swelling less than 3 x 3 cm, (iii) an intact tubal serosa and (iv) no active bleeding. OBJECTIVE: To compare a local injection of hyperosmolar glucose and salpingostomy for the laparoscopic treatment of tubal pregnancy in terms of immediate success and postoperative tubal patency.
DESIGN: Prospective. PATIENTS, SETTING: Forty women with an unruptured tubal pregnancy were enrolled from among 117 women with ectopic pregnancies (EPs) admitted consecutively to the university clinic. 250 mg hydroxy-progesteronecaproate was used for intramuscular administration in 25 patients, between 11 and 15 week's gestation, presenting either with a history of recurrent abortion or being admitted because of threatened abortion. In an immunohistochemical study of the presence of the beta subunit of human chorionic gonadotrophin (hCG) in ovarian neoplasms it was found that hCG was detectable in 42% of ovarian epithelial tumors, an incidence of hCG synthesis which very closely approximates that found in radioimmunoassay studies. There was no relationship between the capacity of an ovarian neoplasm to synthesize hCG and the histologic type of the tumor, the degree of malignancy of the neoplasm, or the prognosis.

A simple, sensitive and reliable non-radioactive method for the detection of hCG in concentrated urine for the diagnosis of early pregnancy is reported. A new method is described for calculating maternal serum marker distribution parameters which will improve risk estimation when screening for Down's syndrome. We report the outcome of the treatment with intratubal methotrexate (25 mg) administered via laparoscopy in 8 patients with ectopic pregnancy who where treated according to a preestablished protocol. A successful treatment was achieved in 6 patients (75%). Hysterosalpingography was performed in 5 of the patients, and in 4 of them we found bilateral tubal patency. Currently 3 patients have a normal intrauterine pregnancy.

Syncytiotrophoblastic cells were identified in a teratocarcinoma of the testis by light microscopy using the semi-thin sectioning technique. An electron microscope study confirmed them to be syncytial. Many irregularly shaped nuclei lying in a honeycombed cytoplasm were observed, while the free margins contained microvilli. These cells are morphologically similar to syncytiotrophoblasts of the normal placenta. The presence of syncytiotrophoblastic cells in testicular tumours means a worse prognosis than would otherwise be concluded on the basis of the particular tumour concerned.

Ectopic pregnancies are the leading cause of maternal mortality in the first trimester, with an incidence of 5%-10% of all pregnancy-related deaths. Diagnosis of ectopic pregnancies is difficult due to clinical mimics and non-specific symptoms of abdominal pain and vaginal bleeding. BACKGROUND: Cervical pregnancy is a rare type of ectopic pregnancy. The management of cervical pregnancy is challenging because of the rarity of the condition, late presentation, which is associated with increased risk of failed medical treatment, and excessive post-evacuation bleeding that may require hysterectomy. There is no good evidence in the literature regarding the pharmacological management of living cervical ectopic pregnancy of more than 9 + 0 weeks of gestation, and there is no standard protocol on methotrexate doses in these cases.

UNLABELLED: Point-of-care testing for β-hCG has been widely advocated to allow rapid diagnosis/exclusion of pregnancy in the emergency department. A quantitative blood β-hCG assay has the additional benefit of being able to monitor the viability of pregnancy, using serial measurements, to determine the appropriate expected increase in β-hCG levels over time (e.g. ectopic pregnancy), and aiding in determining if an intrauterine gestational sac should be visible on sonographic imaging.
