Are we ready to treat hepatitis C virus in individuals with opioid use disorder: assessment of readiness in European countries on the basis of an expert-generated model.

Individuals with a history of injecting drugs have a high prevalence of chronic hepatitis C (HCV) infection. Many have a history of opioid use disorder (OUD). Despite novel treatments with improved efficacy and tolerability, treatment is limited in the group. A faculty of experts shared insights from clinical practice to develop an HCV care-readiness model. Evidence and expert knowledge was collected. Ten experts developed a model of three factors (with measures): 'healthcare engagement', 'guidance' and 'place'. Overall, 40-90% of individuals with OUD engage with drug treatment services. Ten o

Improving Health Equity and Outcomes for Children and Adolescents: The Role of School-Based Health Centers (SBHCs).

School-based health centers (SBHCs) provide a critical point of access to youth in low-resource communities. By providing a combination of primary care, reproductive health, mental health, vision, dental, and nutrition services, SBHCs improve the health, wellbeing, and academic achievement of the students they serve. SBHCs operate in collaboration with schools and community primary care providers to optimize the management of chronic health conditions and other health concerns that may result in suboptimal scholastic achievement and other quality of life measures. Sexual adjustment counseling for women with chronic pelvic pain.

Sexual concerns are common in women with chronic pelvic pain and often remain unresolved when pain improves. Therefore, to restore pelvic function, treatment should address sexuality in addition to pain. In this article, I describe sexual challenges experienced by women with chronic pelvic pain, introduce a modified sexuality counseling model, and suggest sexuality resources and training for gynecologic nurses and other health care providers who are ideally positioned to offer sexuality counseling to this population.

Patient- and provider-level determinants of self-assessed health and well-being among adults with chronic conditions in Slovenian primary care.

BACKGROUND: Chronic diseases significantly impact patients’ quality of life and healthcare systems. In Slovenia, understanding the factors influencing self-assessed health and well-being can support more effective primary care policies. The purpose of this study was to identify patient- and provider-level factors associated with general health and well-being among Slovenian adults with chronic conditions. METHODS: This cross-sectional study was conducted as part of the OECD-led PaRIS initiative. Data were collected from 2133 patients aged ≥ 45 years with at least one chronic condition and 70 p

Challenges and outcomes of pharmacist billing and revenue in a hospital-based primary care setting.

BACKGROUND: Embedded ambulatory care pharmacists improve chronic disease outcomes and medication optimization; however, lack of sustainable reimbursement models remains a barrier to widespread implementation, particularly in hospital-based outpatient department (HOPD) primary care settings where traditional incident-to billing is unavailable. Emerging billing pathways, including chronic care management (CCM), and Medicare Annual Wellness Visits (AWVs), may improve financial sustainability. OBJECTIVE: Characterize pharmacist billing activities across payer types in a traditional university-bas

Expanding Perinatal Care Beyond Obstetrics: The National Veterans Affairs Maternity Care Coordinator Program.

Pregnant and postpartum individuals in the United States experience high rates of adverse pregnancy outcomes, despite multiple obstetric initiatives to address this public health crisis. Perinatal care that meaningfully addresses long-term factors that impact pregnancy outcomes, such as chronic mental and physical conditions, adverse social and environmental factors, structural racism, birth-related morbidity and trauma, and transitions to parenting, must go beyond obstetric care sites to include health care providers who deliver care across the lifespan. Veteran's Health Administration (VHA),

Building on the 2025 national academies pharmacy workshop: ensuring access to community pharmacist care.

Growing medication use, chronic disease burden, and primary care provider shortages have intensified the need for additional models of care delivery in the United States. Community pharmacists are uniquely positioned to help address these challenges through medication management, chronic disease management, and timely access to care. Evidence demonstrates that pharmacist services improve medication use and therapeutic outcomes while lowering overall health care costs. Despite these benefits, pharmacists remain underutilized as patient care providers, largely due to inconsistent reimbursement a

Practices to improve transitions of care: a national perspective.

Improving transitions of care has significant importance to our health care system. While care transitions has been studied and researched by many individuals over the past 20 years, more work is needed to further improve the process. Those beginning to focus on transitions need not begin from scratch, but can use information and research from national and regional collaborative models, as well as other tools and resources to enhance the quality of transitions programs.

Adolescents' transition to self-management of a chronic genetic disorder.

Self-management of chronic illness requires acquisition of self-care skills such as seeking knowledge, adhering to recommendations, practicing healthy behaviors, and life-long self-surveillance. This article describes the core problem and psychosocial processes by which parents transfer, and children take on, the responsibility for managing a chronic genetic condition. Individuals with Marfan syndrome (MFS), their parents, and health care providers were the sources of empirical data. A sample of 108 providers, parents, and individuals with MFS were recruited through a genetics clinic and the N

Drawing a Line Between Integrative and Functional Medicine.

Integrative Medicine (IM) and Functional Medicine (FM) are frequently conflated but represent fundamentally different models of care. IM is an academic discipline grounded in evidence-based medicine, whole-person care, and interdisciplinary collaboration, incorporating evidence-informed complementary, traditional, and Indigenous healing practices within conventional standards of diagnosis and management. FM is a distinct clinical model built on the premise that chronic disease reflects disturbances in biological function, seeking individualized "root causes" through systems-based assessment, s

Precentral stimulation for chronic pain.

A decade of clinical experience has suggested that precentral stimulation is an option for patients with deafferentation as well as other chronic pain syndromes. Permanent complications are uncommon. More scientific evidence is warranted to understand the precise mechanisms for this treatment modality. A larger organized clinical trial is desired to establish the efficacy of precentral stimulation.

Palliative care policy and practice in Kerala, India: Implications for Sustainable Development Goal 3-Health and well-being.

The higher prevalence and incidents of chronic and life-threatening illnesses and the aging population living with morbidity and life-limiting conditions necessitate the need for care provisions like palliative care globally. Palliative care strives for the quality of life of patients with terminal illnesses and serious health conditions and their families. However, globally, access to palliative care remains very limited. The situation is similar in India, except for Kerala, where palliative care access to the needy population is almost universal. Kerala's community palliative care model is g

An enhanced harmonic densely connected hybrid transformer network architecture for chronic wound segmentation utilising multi-colour space tensor merging.

Chronic wounds and associated complications present ever growing burdens for clinics and hospitals world wide. Venous, arterial, diabetic, and pressure wounds are becoming increasingly common globally. These conditions can result in highly debilitating repercussions for those affected, with limb amputations and increased mortality risk resulting from infection becoming more common. New methods to assist clinicians in chronic wound care are therefore vital to maintain high quality care standards. This paper presents an improved HarDNet segmentation architecture which integrates a contrast-elimi

Underlying Mechanisms of Osteoporosis in the Context of Multimorbidity: Clinical Challenges and Management Strategies.

Osteoporosis and chronic conditions such as type 2 diabetes mellitus, cardiovascular disease, heart failure, and chronic kidney disease share several common biological mechanisms, including chronic inflammation, oxidative stress, hormonal dysregulation, and metabolic alterations. In this context, multimorbidity presents an increasing clinical challenge, particularly in older populations, where osteoporosis remains frequently underdiagnosed and undertreated. This review aims to explore the complex interplay between skeletal fragility and cardiometabolic diseases, emphasizing the role of nutriti
