Congenital chylothorax presenting as hydrops fetalis. A case report.

A case of congenital chylothorax presented as polyhydramnios and massive fetal edema. The diagnosis was suspected prior to delivery on the basis of ultrasonography. Ultrasound was helpful in localizing the fluid for postnatal thoracentesis. Congenital chylothorax is part of the differential diagnosis of nonimmune hydrops fetalis.

Resolution of chylothorax after positive end-expiratory pressure ventilation.

Ligation of the thoracic duct has previously been recommended for adults with traumatic chylothorax when average daily chyle loss exceeds 1,500 mL/day over five days since such cases are usually refractory to medical management. We describe a case of traumatic chylothorax where chyle output exceeded 2 L/day for a week despite cessation of oral intake and institution of intravenous hyperalimentation. The chylothorax rapidly resolved when mechanical ventilation with positive end-expiratory pressure was begun for treatment of an acute respiratory distress syndrome. The artificial ventilation may …

[Traumatic chylothorax. A further indication for total parenteral nutrition].

Successful and rapid resolution of a serious cases of traumatic chylothorax by means of conservative management with TPN is reported. This method is clearly indicated as a mean of making up the calorie and amino acid need when litres of chyle are being lost every day. In addition, by resting the digestive tract it brings about a massive decrease in the production of lymph in the mesenteric district and thus leading to easier and earlier sufficiency of the collateral lymph drainage routes. The rapid decrease in quantity and the change over from chylous to serous chest drainage liquid after TPN …

Chylothorax as a Rare Complication of Hepatic Cirrhosis in the Absence of Ascites.

Chylothorax, a rare complication of hepatic cirrhosis, is often under-recognized in the absence of ascites or other classical signs of portal hypertension. The diagnosis can be challenging due to its infrequent presentation, which highlights the clinical significance of this case. A 57-year-old male with a history of decompensated liver cirrhosis presented with progressive dyspnea. Imaging showed a right-sided pleural effusion. Pleural fluid analysis confirmed chylothorax, with elevated triglycerides and normal cholesterol levels. The patient underwent chest tube placement, which resulted in t…

Chylothorax as the Initial Manifestation of Small Cell Lung Carcinoma.

Chylothorax, the accumulation of lymphatic fluid in the pleural space, is a rare condition often linked to trauma, surgery or malignancy. We present the case of a 63-year-old female with a significant smoking history who was diagnosed with chylothorax as the initial manifestation of small cell lung carcinoma (SCLC). Diagnostic workup, including pleural fluid analysis and imaging, confirmed the diagnosis and revealed mediastinal lymphadenopathy and bronchial narrowing. The patient underwent chemotherapy, immunotherapy and management of recurrent effusions but ultimately succumbed to pneumonia a…

Chylothorax as a rare complication of COVID-19: a case report.

Chylothorax is an uncommon cause of pleural effusion, most often associated with malignancy, trauma, or thoracic duct obstruction. Its occurrence following SARS-CoV-2 infection is exceptional. The present case report describes a 53-year-old woman who was admitted to the pneumology department complaining of chronic dry cough with a medical history of type II diabetes and SARS-CoV-2 infection one month ago. Physical examination revealed decreased vesicular breath sounds at the left lung base. A chest X-ray revealed a left pleural effusion and thoracentesis confirmed chylothorax. This case highli…

Rare Case of Progressive Osteolysis and Chylothorax in an Adolescent Male.

An 18-year-old man with no prior medical history presented to the tertiary care center with a 6-month history of progressively worsening bone pain. The pain was localized to the right posterior ribs, mid-thoracic spine, and right iliac bone and was described as deep, throbbing, and exacerbated by physical activity, which significantly limited his mobility. Concurrently, he reported recurrent episodes of dyspnea and chest tightness, particularly in the supine position. Physical examination revealed reduced tactile fremitus and diminished breath sounds over the bilateral lower lung fields, with

Thoracoscopic Management of Chylothorax by Application of a Fibrin Sealant Patch.

Thoracic duct ligation is a demanding procedure when the chyle leak and/or the duct itself are not identified. This report describes a new procedure using thoracoscopic closure of the chyle leak by application of a fibrin sealant patch. This strategy was successfully applied for closing (i) a small fistula due to a postoperative lesion of the lymphatic tributary vessels in one case, and (ii) a large fistula due to idiopathic rupture of the thoracic duct in another case.

Unilateral chylothorax in a renal transplant recipient: A case report.

Chylothorax is accumulated lymphatic fluid in the pleural space. It rarely happens in end-stage renal illness patients, but possible causes are hospital-related. This case describes a 40-year-old man experiencing unilateral chylothorax after a kidney transplant. Left pleural cavity drainage shows a white, milky, cloudy, and odorless fluid. Fluid analysis findings were consistent with chylous. A chest tube was inserted, resulting in complete evacuation of fluid and total lungs expansion. No fluid accumulation was observed upon tube removal. Chylothorax is a rare complication of renal transplant…

Constrictive physiology is not present in all dogs with idiopathic chylothorax.

OBJECTIVE: To determine whether subtotal pericardectomy affects recurrence and long-term outcomes in dogs with idiopathic chylothorax (IC). ANIMALS: 12 client-owned dogs diagnosed with IC between July 26, 2016, and March 23, 2023. METHODS: The diagnosis of constrictive physiology (CP) was established with cardiac catheterization and defined as elevated and equal diastolic pressures in all 4 cardiac chambers. Dogs were then entered into the constrictive physiology (CP) group or non-CP (NCP) group. All dogs received at least a thoracic duct ligation (TDL). The dogs in the CP group had a subtot…

Postesophagectomy chylothorax: a review of the risk factors, diagnosis, and management.

Chylothorax is a crucial postoperative complication of esophagectomy. Characterized by the leakage of chyle and lymphatic fluid through the thoracic duct, chylothorax could result in pleural effusion, respiratory distress, shortness of breath, cardiac arrhythmia, electrolyte imbalance, and malnutrition. Postesophagectomy chylothorax is associated with high morbidity and mortality, and its diagnosis and management require prompt and accurate identification of risk factors and treatment strategies. A variety of strategies are available to treat postesophagectomy chylothorax, ranging from conserv…

Transudative chylothorax in a liver cirrhosis patient: A case report.

Chylothorax defines chyle in the pleural space, usually from defects in thoracic duct. Chylothoraces are usually exudative, as defined by light's criteria but in rare instances, chylothoraces can be transudative. The leading cause of non-traumatic chylothorax is malignancy, but a non-traumatic chylothorax can be a rare manifestation of liver cirrhosis. In this case report, we present a case of an 82-year-old male with a history of non-alcoholic liver cirrhosis requiring multiple paracenteses for chylous ascites in the past, who was found to have a transudative non-traumatic chylothorax. His ch…

Evaluation of treatment options for postoperative and spontaneous chylothorax in adults.

OBJECTIVES: Both postoperative and spontaneous chylothorax remain therapeutic challenges without recommendations for a standardized treatment approach. Regardless of its aetiology, patients with chylothorax experience prolonged hospitalization and suffer from the associated complications or the invasive therapy administered. METHODS: We conducted a retrospective, observational review of adult patients with chylothorax treated between January 2010 and September 2019. The primary end point was successful management with sustained cessation and/or controlled chylous output. Therapy duration, inp…

Milky pleural effusion in a neonate and approach to investigating chylothorax.

Neonatal chylothorax is a rare presentation leading to significant respiratory distress, thus requiring timely diagnosis. A preterm neonate was resuscitated and ventilated, following which she clinically improved but subsequently developed respiratory distress with a right-sided pleural effusion. Interestingly, thoracentesis fluid appeared 'milky' with elevated triglycerides and lymphocytes, suggesting chylothorax. As fluid triglyceride level was lower than the established diagnostic criterion for chylothorax (1.24 mmol/L), a high fluid-to-serum triglyceride ratio was used as a surrogate diagn…

Resolution of refractory chylothorax with a combination of talc pleurodesis and CPAP.

Chylothorax is an uncommon cause of pleural effusion. Recurrent chylous effusions are often resistant to conservative treatment and many need surgical intervention. We report a 69-year-old woman with refractory idiopathic chylothorax resistant to medium-chain triglyceride diet and intermittent thoracentesis. Lymphangiography and lymphoscintigraphy failed to identify the site of leakage. We initiated continuous positive airway pressure (CPAP) 12 h before and 48 hours after talc pleurodesis. Chest drain was removed at day 3 and she was discharged at day 5. To our knowledge, this is the first cas…

Combination Therapy with Etilefrine and Pleurodesis for Refractory Congenital Chylothorax.

Etilefrine, a sympathomimetic agent, is reportedly effective against postoperative chylothorax. However, its effectiveness in treating congenital chylothorax was unknown. We report herein a case of refractory congenital chylothorax treated with etilefrine in a late preterm neonate with massive fetal chylous pleural effusion. The chylothorax was unresponsive to previous treatments, including dietary and pharmacological treatment and thoracic duct ligation. The pleural effusion decreased after intravenous etilefrine was begun on day of life (DOL) 84 and resolved after the addition of chemical pl…

Diagnosis and management of traumatic bilateral chylothorax: a clinical conundrum.

A 35-year-old man was admitted in the emergency department with a complaint of progressive dyspnoea and 4-day prior history of a road traffic accident. An initial evaluation was inconspicuous and he was discharged. Clinical examination and radiological imaging suggested bilateral pleural effusion for which bilateral intercostal drains were inserted. Further pleural fluid analysis aided the diagnosis of bilateral chylothorax. A conservative approach consisting of a fat-free diet, total parenteral nutrition and octreotide therapy was started. Upon failure of conservative management, thoracic duc…
