[Clinical thinking and decision-making in practice. A pregnant Turkish woman with a liver tumor].

A 30-year-old Turkish woman suffered from pain in the right upper abdomen during the 32nd week of her third pregnancy. On ultrasound a liver tumour was diagnosed. The patient was followed without treatment during pregnancy. The outcome of the pregnancy was uncomplicated. The diagnosis of focal nodular hyperplasia was made post partum using MRI with contrast and a liver biopsy. The born son suffered the syndrome of Klippel-Trenaunay-Weber, also a vascular malformation. A liver tumour during pregnancy can influence the outcome of the pregnancy, but the pregnancy may also affect the progression o

[Clinical judgment and decision making in medical practice. A retiree with fatigue and foot drop].

A 64-year-old former civil servant consulted his general practitioner because of severe fatigue. Later he began to lose weight and gradually developed chronic sensorimotor polyneuropathy characterized by sensory nerve loss which started in his legs. After a year he needed a wheel chair and developed cachexia. IgG paraprotein was detected. Morbid-anatomical examination of enlarged supraclavicular lymph nodes revealed plasma cell angiofollicular hyperplasia, characteristic of Castleman's disease. Treatment with corticosteroids led to marked improvement of the patient's condition. He was able to

Clinical decision making and laparoscopy versus computer prediction in the management of the acute abdomen.

A prospective study has been undertaken of 321 patients with acute abdominal pain admitted to hospital under one surgical firm over a period of 21 months. During the first 10 months patients were classified on admission according to the perceived need for operation, with laparoscopy being performed on all those in whom the need for operation was uncertain. In addition, all women with suspected appendicitis underwent laparoscopy because previous studies by us and others have demonstrated a high error rate in this group. During the second 11 months of the study a similar system of classification

Assessing the clinical relevance of point-of-care ultrasound for hospitalists: Influence on clinical reasoning and decision-making.

INTRODUCTION: Point-of-care ultrasonography (POCUS) is increasingly recognized in internal medicine, yet its influence on hospitalists' clinical reasoning remains underexplored. OBJECTIVES: This study aimed to evaluate the influence of POCUS on diagnostic hypotheses, management strategies, and confidence levels among hospitalists in a transitional care unit with limited access to advanced imaging. METHODS: Prospective descriptive study in a transitional care unit enrolling 19 hospitalists. During routine care, clinicians used POCUS at their discretion and, immediately afterward, completed a

Patient-Based Decision-Making for Opioid Prescriptions after Ambulatory Breast Operation: A Randomized Clinical Trial.

BACKGROUND: Opioids are routinely prescribed after ambulatory breast surgery despite low reported pain, increasing risk of misuse and diversion. We conducted a randomized clinical trial to determine whether a patient opt-in prescribing strategy reduces opioid prescription, consumption, and excess without worsening postoperative pain or quality of life. STUDY DESIGN: Adults undergoing ambulatory breast surgery at a single tertiary center (2022 to 2024) were randomized 1:1 to Postoperative Opt-In Opioid Treatment (POINT) or routine opioid prescription. POINT patients received education and were

Analysis of low-benefit subgroups in breast-conserving surgery: Implications for clinical decision-making (Review).

Breast-conserving surgery (BCS) with radiotherapy is a standard treatment for early-stage breast cancer, offering survival outcomes equivalent to mastectomy while preserving breast appearance and improving quality of life. However, BCS suitability and effectiveness vary notably among patients due to differences in age, comorbidities, tumor biology, breast anatomy and psychosocial factors. Through a systematic review, the present study identified six patient subgroups that may derive limited benefit from BCS followed by radiotherapy. These constituted the elderly with multiple comorbidities, th

Unified genetic risk score for prostate cancer enables improved risk stratification for clinical decision-making.

BACKGROUND: Current clinical approaches to inherited prostate cancer (PCa) risk rely on binary classification of pathogenic variant (PV) carrier status without accounting for gene-specific heterogeneity or polygenic risk. We developed an integrated genetic risk model reflecting a continuum of inherited susceptibility. METHODS: In the UK Biobank (UKB; n=218 484), we evaluated associations of PVs in 11 clinically recommended genes and a polygenic risk score (PRS) with incident PCa using Cox models. A continuum model (GenProb-PCa) incorporating gene-specific PVs and PRS was developed and compare

Autoantibody repertoire in lupus nephritis: from pathogenic mechanisms to clinical integration for precision decision-making.

Lupus nephritis (LN) represents the most severe renal manifestation of systemic lupus erythematosus (SLE), and timely, risk-stratified intervention is essential to mitigate irreversible kidney damage and improve long-term clinical outcomes. The autoantibody repertoire not only constitutes a cornerstone of SLE diagnosis but also serves as a critical pathogenic link between systemic immune dysregulation and site-specific renal injury. This review synthesizes current evidence on established and emerging autoantibodies associated with LN, with an emphasis on their pathogenic contributions to three

Peritoneal cytology in pancreatic cancer: a comprehensive review on prognostic stratification and clinical decision-making.

Pancreatic cancer is characterized by its high malignancy, aggressive nature, and exceedingly poor prognosis. While surgical resection combined with systemic therapy offers the only potential for cure, the high recurrence rate significantly compromises long-term survival, even after radical surgery. The presence of peritoneal free tumor cells, detected via peritoneal cytology, represents a form of metastatic spread distinct from traditional distant metastases and exerts a similarly negative impact on patient outcomes. Peritoneal washing cytology is a practical and valuable diagnostic tool for

Two Decades of NPM1-Mutated Acute Myeloid Leukemia: From Molecular Insights to Clinical Decision-Making.

NPM1-mutated acute myeloid leukemia (AML) represents one of the most frequent and biologically distinct AML entities, characterized by the aberrant cytoplasmic localization of the NPM1 mutant protein. The recent advances in molecular biology and translational research have progressively redefined the clinical management of this disease. This narrative review summarizes the evolution of NPM1-mutated AML over the last two decades, focusing on the transition from immunohistochemical observations to modern genetics-based and measurable residual disease (MRD)-driven approaches. We discuss the biolo

Enhanced clinical decision-making to optimize targeted gene panel testing for inborn errors of immunity.

BACKGROUND: Targeted gene panels (TGPs) are frequently used to assess inborn errors of immunity (IEI) owing to the expanding knowledge of monogenic causes. Refining their clinical indications may enhance test yields. OBJECTIVE: To assess clinical factors associated with the diagnostic results of IEI from TGP in pediatrics. METHODS: A retrospective study of pediatric patients with suspected IEI who underwent Invitae TGP over a 5-year period at a tertiary hospital in the U.S. RESULTS: Of the 350 patients (median age 5.1 years, IQR [1.6,10.9]; 56% male; 85% White), 8.3% had diagnostic genetic

Clinical decision-making in gastric taeniasis: risks of endoscopic removal and role of staged therapy.

This case describes a rare presentation of gastric taeniasis in a young woman with chronic atrophic gastritis and subcutaneous lesions consistent with cysticercosis. Stool microscopy confirmed Taenia spp eggs, though species identification was limited by the absence of morphological or molecular confirmation. While clinical evidence strongly suggested Taenia solium, this limitation is acknowledged. The case underscores the importance of clinical judgement in avoiding endoscopic removal, given risks of incomplete extraction, rupture and anaphylaxis. A staged pharmacologic regimen was employed:

Clinical and Dermoscopic Determinants of Surgical Decision-Making in Skin Cancer Screening: A Retrospective Analysis.

Background and Objectives: Deciding which pigmented or non-pigmented skin lesions require surgical excision, as opposed to continued observation, is a common challenge in everyday dermatologic practice. Although dermoscopy is well established as a tool that improves melanoma detection accuracy compared with naked-eye examination, real-world evidence on how demographic profile, patient history, and dermoscopic impression jointly influence the surgical decision remains limited. This study examined the clinical and dermoscopic factors associated with a positive surgical decision (PSD) in a large

[Clinical decision-making for immediate restoration of terminal dentition: determination and transfer of jaw relations].

Immediate implant-supported fixed restoration in edentulous jaws demonstrates a success rate comparable to that of conventional implant restoration. However, this approach still presents a certain degree of technique sensitivity. In the field of immediate implant-supported fixed restoration in dentistry, a repeatable and stable jaw relation is the prerequisite for the design and fabrication of prostheses. It also reduces chairside denture placement and occlusal adjustment time and lowers the risk of occlusion-related complications. For patients with terminal dentition, the precise transfer of

How Cardiovascular Physical Examination Impacts Clinical Decision-Making in Various Scenarios of Cardiac Valvular Diseases.

BACKGROUND: Complementary diagnostic examinations have replaced the importance of Physical Examination (PE) in Clinical Decision-making (CDM). 1. Medical clinical practice has lost confidence in Physical Examination (PE) compared to complementary diagnostic examinations (CE). More importance has been deposited on Echocardiography (ECHO) in cardiac valvular dysfunction scenarios. No studies have measured the impact of cardiac PE in Clinical Decision-making (CDM). 2. Discrepancies between PE findings and ECHO reports are common and may impact CDM in different forms. 3. PE has proven essential to

Machine learning in lupus nephritis: bridging prediction models and clinical decision-making towards personalized nephrology.

BACKGROUND: Lupus nephritis (LN) is one of the most severe manifestations of systemic lupus erythematosus (SLE), affecting up to 65% of patients and contributing significantly to morbidity and mortality. The heterogeneous clinical course of LN-characterized by alternating flares and remissions-stems from complex immunological, genetic, endocrine, and environmental factors. Current management strategies rely on immunosuppressants and corticosteroids, yet predicting disease progression, treatment response, and relapse risk remains challenging. OBJECTIVE: This review synthesizes current evidence

Clinical Value of ChatGPT for Epilepsy Presurgical Decision-Making: Systematic Evaluation of Seizure Semiology Interpretation.

BACKGROUND: For patients with drug-resistant focal epilepsy, surgical resection of the epileptogenic zone (EZ) is an effective treatment to control seizures. Accurate localization of the EZ is crucial and is typically achieved through comprehensive presurgical approaches such as seizure semiology interpretation, electroencephalography (EEG), magnetic resonance imaging (MRI), and intracranial EEG (iEEG). However, interpreting seizure semiology is challenging because it heavily relies on expert knowledge. The semiologies are often inconsistent and incoherent, leading to variability and potential

Role of Potential Biomarkers in Aortic Aneurysms: Does It Hold Promise for Clinical Decision Making?

Aortic aneurysms (AAs) are a life-threatening disease with a mortality rate of up to 80% when they rupture. AA has a multifactorial etiology, including smoking, advanced age, and family history, and has multifaceted pathophysiological mechanisms underlying its formation, mainly including inflammation of the aortic wall, reduction of medial smooth muscle cells, and degradation of the extracellular matrix. It is also a progressive disease. Their treatments are limited to open surgical repair and endovascular aneurysm repair. There is no effective drug treatment. The diagnosis of AA is usually ma
